Carcinoid syndrome is a constellation of signs and symptoms caused by different hormones produced by carcinoid tumors. Very rarely, those tumors can metastasize to the heart and cause cardiac involvement of the tumor. This study presents a very rare case of secondary cardiac tumor affecting the left ventricle from a metastatic carcinoid tumor originating from the small intestine without carcinoid valvular heart disease.

Article Synopsis
  • - Increasing reports indicate that COVID-19 may impact the pancreas, leading to conditions like pancreatitis caused by high triglyceride levels (hypertriglyceridemia).
  • - A case study of a 51-year-old woman revealed severe abdominal pain and elevated triglycerides, with a subsequent COVID-19 diagnosis on day 2, requiring intensive treatment for both pancreatitis and COVID-19.
  • - The treatment successfully lowered triglyceride levels, highlighting the need for further research to understand the relationship between COVID-19, triglycerides, and pancreatic health, as complications can be serious.

Introduction Preterm prelabor rupture of membrane (PPROM) contributes to increasing rates of preterm birth, causing greater health risks for newborns. While the mechanisms driving PPROM are not well understood, one hypothesis is that it is due to systemic inflammation, which can be caused by obesity defined as a BMI [Formula: see text]30 kg/m. The specific aim of the study was to compare neonatal outcomes after PPROM between patients who were obese vs not obese in early pregnancy at a tertiary medical center serving an Appalachian population.

  • Primary renal lymphoma (PRL) is a rare form of non-Hodgkin's lymphoma affecting the kidneys without spreading to other areas.
  • A 66-year-old woman experienced shortness of breath and acute renal failure, leading to a diagnosis of primary renal diffuse large B-cell lymphoma after detailed imaging and a renal biopsy.
  • The patient was treated with R-CHOP chemotherapy, leading to a full recovery of kidney function and highlighting the need for prompt recognition and treatment of PRL to improve patient outcomes.

Alveolar soft part sarcoma (ASPS) is a rare malignant tumor that manifests as a slow-growing soft tissue mass and frequently presents with distant metastasis. The prognosis is variable, and complete remission of metastatic disease has rarely been reported. Our patient was diagnosed with metastatic ASPS at the age of 17, with a primary forearm lesion and metastasis to the lungs.

Biliary Dyskinesia-Controversies, Diagnosis, and Management: A Review.

JAMA Surg

September 2024

Department of Surgery, West Virginia University/Charleston Division, Charleston Area Medical Center Institute for Academic Medicine, Charleston.

Importance: Biliary dyskinesia is a disorder characterized by biliary pain, a sonographically normal gallbladder, and a reduced gallbladder ejection fraction on cholecystokinin-cholescintigraphy (CCK-HIDA) scan. Laparoscopic cholecystectomy remains a common treatment for biliary dyskinesia despite a lack of high-quality evidence supporting the practice. The following review summarizes the current biliary dyskinesia outcomes data, the diagnostic strategies and their limitations, biliary dyskinesia in the pediatric population, the emerging phenomenon of the hyperkinetic gallbladder, and suggestions for addressing identified knowledge gaps.

Impact of emergency department boarding on patients outcomes in hip fractures.

Am J Emerg Med

September 2024

Institute of Academic Medicine, Charleston Area Medical Center, Charleston, WV, United States of America. Electronic address:

Background: Boarding time in the Emergency Department (ED) is an area of concern for all patients and potentially more problematic for the hip fracture population. Identifying patient outcomes impacted by ED boarding and improving emergent care to reduce surgical delay for this patient population is a recognized opportunity. The objective of this study is to examine the impact of ED boarding in relation to patient outcomes in the surgical hip fracture population.

  • Intradiverticular transitional cell carcinoma (TCC) of the bladder is difficult to diagnose and treat due to its unique location in bladder diverticula, necessitating early detection for better patient outcomes.
  • Tailored treatment plans, including radical surgeries and adjuvant chemotherapy, are crucial because of the aggressive nature of this cancer, along with strict monitoring after treatment to manage high recurrence rates.
  • Future research goals include identifying biomarkers, comparing treatment effectiveness, and exploring innovative therapies like immunotherapy, with the aim of improving patient survival rates and quality of life.

Article Synopsis
  • The study looked at how different surgeons perform two types of surgeries for carotid artery problems: carotid endarterectomy (CEA) and transcarotid artery revascularization (TCAR).
  • It analyzed patient data from 2021 to 2023 to see how the number of surgeries a surgeon does impacts the risks of problems like strokes or injuries after the surgery.
  • Results showed that surgeons who primarily do TCAR or CEA have different rates of complications, suggesting that experience with one type affects outcomes for patients.

Endogenous opioids and their associated receptors form a system that maintains survival by positively reinforcing behaviors that are vital to life. Cancer and cancer treatment side effects capitalize on this system pathogenically, leading to maladaptive biological responses (e.g.

The function of endogenous opioids spans from initiating behaviors that are critical for survival, to responding to rapidly changing environmental conditions. A network of interconnected systems throughout the body characterizes the endogenous opioid system (EOS). EOS receptors for beta-endorphin, enkephalin, dynorphin, and endomorphin underpin the diverse functions of the EOS across biological systems.

Background: The frequency of distal lower extremity bypass (LEB) for infrapopliteal critical limb threatening ischemia (IP-CLTI) has significantly decreased. Our goal was to analyze the contemporary outcomes and factors associated with failure of LEB to para-malleolar and pedal targets.

Methods: We queried the Vascular Quality Initiative infrainguinal database from 2003 to 2021 to identify LEB to para-malleolar or pedal/plantar targets.

Tissue Ablation: Applications and Perspectives.

Adv Mater

August 2024

Laboratory for Patient Inspired Engineering, Mayo Clinic, 13400 East Shea Blvd, Scottsdale, AZ, 85259, USA.

Tissue ablation techniques have emerged as a critical component of modern medical practice and biomedical research, offering versatile solutions for treating various diseases and disorders. Percutaneous ablation is minimally invasive and offers numerous advantages over traditional surgery, such as shorter recovery times, reduced hospital stays, and decreased healthcare costs. Intra-procedural imaging during ablation also allows precise visualization of the treated tissue while minimizing injury to the surrounding normal tissues, reducing the risk of complications.

Objective: Retrograde open mesenteric stenting (ROMS) is an alternative to mesenteric bypass in patients with acute mesenteric ischemia (AMI) with variable reported 30-day mortality rates. Large studies evaluating patient outcomes following ROMS are scarce. Our study aims to assess the results of this approach among patients presenting with AMI.

Hyperkalemic periodic paralysis is a rare medical condition characterized by periods of extreme muscle weakness or paralysis. While most cases of hyperkalemic periodic paralysis are associated with a genetic channelopathy, cases of secondary hyperkalemic periodic paralysis can pose a challenge for medical personnel in terms of timely recognition. Identification of this medical emergency early in its course is essential to preventing cardiac and neurological sequelae.

The erector spinae plane (ESP) block, initially designed for thoracic analgesia, has evolved into a versatile regional anesthesia technique with literature support for success in numerous contexts. In this case report, we highlight the successful application of ESP to provide postoperative analgesia for pediatric Dega osteotomy involving both the femoral head and acetabulum, in a patient with numerous neurological comorbidities that would have weighed against some more traditional regional anesthesia techniques. This case further highlights the versatility of ESP, demonstrating its use in blocking lumbar nerve roots in a pediatric patient with complex neurological challenges.
