Background: The GLARIUS trial, which investigated the efficacy of bevacizumab (BEV)/irinotecan (IRI) compared with standard temozolomide in the first-line therapy of O6-methylguanine-DNA methyltransferase (MGMT)-nonmethylated glioblastoma, showed that progression-free survival was significantly prolonged by BEV/IRI, while overall survival was similar in both arms. The present report focuses on quality of life (QoL) and Karnofsky performance score (KPS) during the whole course of the disease.

Methods: Patients (n = 170) received standard radiotherapy and were randomized (2:1) for BEV/IRI or standard temozolomide.

Fibroblast growth factor-23 (FGF23) is a bone-derived hormone regulating vitamin D hormone production and renal handling of minerals by signaling through an FGF receptor/αKlotho (Klotho) receptor complex. Whether Klotho has FGF23-independent effects on mineral homeostasis is a controversial issue. Here, we aimed to shed more light on this controversy by comparing male and female triple knockout mice with simultaneous deficiency in Fgf23 and Klotho and a nonfunctioning vitamin D receptor (VDR) (Fgf23/Klotho/VDR) with double (Fgf23/VDR, Klotho/VDR, and Fgf23/Klotho) and single Fgf23, Klotho, and VDR mutants.

Objectives: Despite increasing evidence supporting selective caries tissue removal, the technique is not adopted by most dentists, one possible reason being that patients might reject it. We aimed to assess patients' preferences for selective versus complete excavation, and to identify predictors of this preference.

Methods: A sequential mixed-methods approach was taken.

Background: Female pattern hair loss (FPHL) is a common non-scarring alopecia characterized by widening of the midline hair part at the crown (vertex). In 1977, Ludwig developed a scale that graded the degree of visible vertex hair thinning from I (least severe) to III (most severe). However, by the time patients exhibit the full manifestations of 'Ludwig I', they have already lost a significant volume of hair.

Background: Periodontal scaling might cause musculoskeletal disorders, and scaling instruments might not only have different effectiveness and efficiency but also differ in their ergonomic properties. The present study assessed ergonomic working patterns of experienced (EO) and less experienced operators (LO) when using hand and powered devices for periodontal scaling and root planning.

Methods: In an experimental study using periodontally affected manikins, sonic (AIR), ultrasonic (TIG) and hand instruments (GRA) were used by 11 operators (7 EO/4 LO) during simulated supportive periodontal therapy.

How sick is the defendant? Forensic and emergency medical aspects at trial.

Leg Med (Tokyo)

January 2013

University Medical Centre Charité - University of Berlin, Institute of Legal Medicine and Forensic Sciences, Turmstrasse 21, Berlin, Germany.

The evaluation of a defendant's ability to stand trial is a core task in the field of legal medicine as a supportive instrument of jurisprudence. Before the trial, the court-appointed expert informs him-/herself regarding existing somatic and psychological illness(-es) within the context of preparing an expert evaluation, then composing the written expert evaluation, which reflects the condition of the subject at the time of the examination and arrives at an assessment of the ability to stand trial. The decompensation or aggravation of illnesses - in particular in stressful situations, as is to be expected for the defendant in the courtroom - can thus be taken into consideration such that alongside the current evaluation of the defendant's health condition, emergency medical treatment may also become necessary in court at times.

Department of Paediatric Oncology and Haematology, Charité - University of Berlin, Berlin, Germany.

Thrombocytopenia is the most common haematological abnormality in newborns admitted to neonatal care units and serves as an important indicator of underlying pathological processes of mother or child. In most cases thrombocytopenia is mild to moderate and resolves within the first weeks of life without any intervention. However, in some neonates thrombocytopenia is severe or may reflect an inborn platelet disorder.

University Medical Centre Charité-University of Berlin, Institute of Legal Medicine and Forensic Sciences, Turmstr. 21, Building N, 10559 Berlin, Germany.

Post-mortem animal depredation is not an uncommon phenomenon in routine forensic autopsies. We present three cases of complete post-mortem decapitation by domestic German shepherd dogs. In two cases, the head had been bitten off, defleshed and left lying near the body, while in one case it had been completely devoured by two dogs; only small skull fragments and crowned teeth could be found.
