Telomerase is reactivated by genomic TERT rearrangements in ~30% of diagnosed high-risk neuroblastomas. Dismal patient prognosis results if the RAS/MAPK/ALK signaling transduction network also harbors mutations. We present a liquid biopsy-based monitoring strategy for this particularly vulnerable pediatric patient subgroup, for whom real-time molecular diagnostic tools are limited to date.

Article Synopsis
  • Many kidney transplant patients are classified as having no rejection after biopsy, but some may actually have undetected rejection activities.
  • A study of over 5,000 biopsies found that more than half were marked as no rejection, yet many showed signs of subthreshold T cell-mediated rejection (TCMR) and antibody-mediated rejection (ABMR).
  • Patients with these hidden rejection activities are at increased risk of future graft issues, as higher levels of TCMR and ABMR correlate with worse kidney function and higher chances of rejection in subsequent biopsies.

Objective: This prespecified exploratory analysis evaluated the association of gene expression signatures, tumor mutational burden (TMB), and multiplex immunohistochemistry (mIHC) tumor microenvironment-associated cell phenotypes with clinical outcomes of pembrolizumab in advanced recurrent ovarian cancer (ROC) from the phase II KEYNOTE-100 study.

Methods: Pembrolizumab-treated patients with evaluable RNA-sequencing (n = 317), whole exome sequencing (n = 293), or select mIHC (n = 125) data were evaluated. The association between outcomes (objective response rate [ORR], progression-free survival [PFS], and overall survival [OS]) and gene expression signatures (T-cell-inflamed gene expression profile [TcellGEP] and 10 non-TcellGEP signatures), TMB, and prespecified mIHC cell phenotype densities as continuous variables was evaluated using logistic (ORR) and Cox proportional hazards regression (PFS; OS).

The oxygen dissociation curve of blood in COVID-19-An update.

Front Med (Lausanne)

February 2023

Department of Molecular and Cellular Sport Medicine, Institute of Cardiovascular Research and Sport Medicine, German Sport University Cologne, Cologne, Germany.

An impressive effect of the infection with SARS-Co-19 is the impairment of oxygen uptake due to lung injury. The reduced oxygen diffusion may potentially be counteracted by an increase in oxygen affinity of hemoglobin. However, hypoxia and anemia associated with COVID-19 usually decrease oxygen affinity due to a rise in [2,3-bisphosphoglycerate].

Chronic Spontaneous Urticaria: The Role and Relevance of Autoreactivity, Autoimmunity, and Autoallergy.

J Allergy Clin Immunol Pract

August 2023

Institute of Allergology, Charitè, Medical University of Berlin, Berlin, Germany; Fraunhofer Institute for Translational Medicine and Pharmacology ITMP, Allergology and Immunology, Berlin, Germany.

Chronic spontaneous urticaria (CSU) is a frequent and often severely disabling disease. A large number of studies were performed during the last 2 decades to clarify its pathogenesis. These studies shed light on the underlying autoimmune mechanisms of CSU pathogenesis and have led us to understand that different mechanisms may exist and, sometimes, coexist behind the same clinical presentation.

Article Synopsis
  • The study aimed to evaluate if patients with BRCA1/2-mutated or homologous recombination deficient (HRD) ovarian cancers benefited from the immune therapy atezolizumab in the phase III IMagyn050 trial.
  • Out of evaluated samples, 22% had BRCA1/2 mutations, and 46% were HRD, with most tumors showing low tumor mutation burden (TMB), and progression-free survival (PFS) was better in BRCA2-mutated and HRD tumors.
  • The trial concluded that neither BRCA1/2 mutation nor HRD provided significant advantages from atezolizumab, indicating low TMB levels and suggesting that genomic instability does not enhance sensitivity to immune checkpoint inhibitors

Background: Anaplastic large cell lymphoma (ALCL) is an aggressive non-Hodgkin T cell lymphoma commonly driven by NPM-ALK. AP-1 transcription factors, cJUN and JUNb, act as downstream effectors of NPM-ALK and transcriptionally regulate PDGFRβ. Blocking PDGFRβ kinase activity with imatinib effectively reduces tumor burden and prolongs survival, although the downstream molecular mechanisms remain elusive.

Beta-blockers have solid documentation in preventing cardiovascular complications in the treatment of hypertension; atenolol, metoprolol, oxprenolol and propranolol demonstrate proven cardiovascular prevention in hypertension mega-trials. Hypertension is characterised by activation of the sympathetic nervous system from early to late phases, which makes beta-blockers an appropriate treatment seen from a pathophysiological viewpoint, especially in patients with an elevated heart rate. Beta-blockers represent a heterogenous class of drugs with regard to both pharmacodynamic and pharmacokinetic properties.

Article Synopsis
  • The study investigates the immune environment in high grade serous ovarian carcinoma (HGSOC) patients to find alternative immune targets after PD-1/PD-L1 inhibitors did not yield favorable results.
  • Analysis of tumor samples from 103 HGSOC patients showed that higher levels of intratumoral CD3 T lymphocytes and HLA-E expression were linked to better progression-free and overall survival rates.
  • The findings suggest that targeting the HLA-E/CD94-NKG2A/2C pathway could be a promising strategy, especially for patients with genomic instability indicated by homologous recombination deficiency (HRD).
