Background: CIN2 is considered a biologically equivocal lesion falling between low- and high-grade cervical dysplasia, but it is often managed with cervical conization as a high-grade lesion. However, since cervical conization can lead to an increased risk of adverse obstetric events, it might be interesting to identify, by colposcopy, a subgroup of women with a low risk of "occult" CIN3 who could be managed with a "wait and see" approach.

Methods: All the women with CIN2 cervical biopsy from 1999 to 2019 were retrospectively identified.

High-risk HPV-positive and -negative high-grade cervical dysplasia: Analysis of 5-year outcomes.

Gynecol Oncol

April 2021

Gynecological Oncology Unit, Fondazione IRCCS Istituto Nazionale dei Tumori di Milano, Milan, Italy.

Article Synopsis
  • The study aimed to assess the outcomes of high-risk HPV-positive and negative women with high-grade cervical dysplasia through a retrospective analysis of medical records from 2010 to 2014.
  • A total of 2,966 women were included, with 85% being HR-HPV-positive; results indicated that HR-HPV-positive patients had a significantly higher risk of recurrence after conization compared to HR-HPV-negative patients.
  • The findings suggest that while HR-HPV-negative high-grade dysplasia accounts for 15% of cases, these patients generally have better outcomes, highlighting the need for further research to validate these results.

In the context of the COVID-19 pandemic, patients need to be evaluated within 2-4 weeks in the following cases: cytology result of "squamous cell carcinoma," "atypical glandular cells, favor neoplastic," "endocervical adenocarcinoma in situ," or "adenocarcinoma"; histopathological diagnosis of suspected invasion from cervical/vaginal biopsy, or invasive disease after a cervical excision procedure, vaginal excision, or vulvar biopsy/excision; sudden onset of strongly suggestive symptoms for malignancy. Digital imaging technologies represent an important opportunity during the COVID-19 pandemic to share colposcopic images with reference centers, with the aim of avoiding any concentration of patients. All patients must undergo screening for COVID-19 exposure and should wear a surgical mask.
