We present an elite footballer in his 20s with a large and symptomatic osteochondral lesion of the weight-bearing area of the medial femoral condyle, who was proposed for a single plug fresh osteochondral allograft transplantation. The lesion was due to repetitive micro traumas and had become highly symptomatic being responsible for making the football practice at a professional level almost impossible, reaching a point that keeping an elite sports career was compromised. Six months after surgery, the athlete returned to sports and, at a 3-year follow-up, still participated in elite high-level football, with a Tegner Activity Scale 10/10, IKDC 93.

EULAR points to consider for patient education in physical activity and self-management of pain during transitional care.

Ann Rheum Dis

November 2024

Department of Education, Faculty of Education Sciences, CIBIS (Centro de Investigación para el Bienestar y la Inclusión Social) Research Centre and SPORT Research Group (CTS-1024), University of Almería, Almería, Spain.

Article Synopsis
  • A EULAR task force developed guidelines for educating young patients with juvenile-onset rheumatic and musculoskeletal diseases about managing physical activity and pain during their transition from pediatric to adult healthcare.
  • They formulated two overarching principles highlighting the need for personalized care and seven points to consider, emphasizing the importance of patient education for a smoother transition.
  • The guidelines are backed by moderate to weak levels of evidence and are intended to standardize care in Europe while encouraging further research and policy development in this area.

Background: To address the societal burden of low back pain (LBP), several health systems have adopted Models of Care (MoCs). These evidence-informed models aim for consistent care and outcomes. However, real-world applications vary, with each setting presenting unique challenges and nuances in the primary healthcare landscape.

Objective: To compare spinal symptoms and spinal/hip mobility at baseline and 2 years in early axial spondyloarthritis (axSpA) and non-axSpA chronic back pain (BP) patients.

Methods: Baseline and 2 years data of the SPondyloarthritis Caught Early cohort were analysed. Outcomes assessed: overall BP, BP at night, morning stiffness (MS) intensity, MS duration, occiput-to-wall distance (OWD), cervical rotation, chest expansion, lateral spinal flexion (LSF), modified Schober test (mSchober), intermalleolar distance (IMD) and Bath Ankylosing Spondylitis Metrology Index (BASMI).

  • A team updated the 2017 EULAR treatment recommendations for systemic sclerosis (SSc) to include new evidence and therapy options.
  • They conducted a systematic literature review and developed 22 recommendations across 8 clinical areas, focusing mainly on skin fibrosis and interstitial lung disease (ILD).
  • The updated guidelines now suggest new treatments like mycophenolate mofetil and rituximab and emphasize a research agenda for future therapies targeting various aspects of SSc.

Objective: Management of endometrial cancer is advancing, with accurate staging crucial for guiding treatment decisions. Understanding sentinel lymph node (SLN) involvement rates across molecular subgroups is essential. To evaluate SLN involvement in early-stage (International Federation of Gynecology and Obstetrics 2009 I-II) endometrial cancer, considering molecular subtypes and new European Society of Gynaecological Oncology (ESGO) risk classification.

  • Autism is a common condition influenced by both single gene issues and multiple genes, and many autistic people need better healthcare that genomics can help provide.
  • The European Autism GEnomics Registry (EAGER) aims to collect info about autistic people who have had their entire DNA sequenced to help with future research and trials.
  • EAGER will involve 1,500 participants from 13 places in 8 countries who will share genetic samples and fill out surveys to help researchers understand the link between genetics and health.

Article Synopsis
  • Models of care (MoCs) for low back pain (LBP) aim to bridge the gap between evidence and practice in primary healthcare, enhancing the effectiveness of treatments.
  • A scoping review of 29 studies identified 11 MoCs, mostly implemented in high-income countries, with a focus on structured care approaches and involvement of general practitioners and physiotherapists.
  • While education and exercise were common components across the MoCs, the details concerning intervention content and follow-up protocols were often insufficiently reported.

Objectives: To investigate uniformity and pitfalls in structured radiological staging of rectal cancer.

Methods: Twenty-one radiologists (12 countries) staged 75 rectal cancers on MRI using a structured reporting template. Interobserver agreement (IOA) was calculated as the percentage agreement between readers (categorical variables) and Krippendorff's α (continuous variables).

Platelet-rich plasma (PRP) has emerged as a promising therapy in regenerative medicine. However, the lack of standardization in PRP preparation protocols presents a challenge in achieving reproducible and accurate results. This study aimed to optimize the PRP preparation protocol by investigating the impact of two different anticoagulants, sodium citrate (SC) and ethylenediaminetetraacetic acid (EDTA), and resuspension media, plasma versus sodium chloride (NaCl).

Article Synopsis
  • This text outlines a systematic literature review that evaluates the efficacy and safety of various pharmacological treatments for psoriatic arthritis (PsA), focusing on disease-modifying antirheumatic drugs (DMARDs).
  • A thorough search of databases like Medline and EMBASE helped to identify relevant studies published between 2019 and 2022, analyzing both treatment effectiveness and potential adverse effects.
  • The review found that different classes of drugs, including conventional and biologic DMARDs, were effective in alleviating PsA symptoms and improving quality of life, while also confirming that safety concerns align with previous findings from 2019.

Objective: New modes of action and more data on the efficacy and safety of existing drugs in psoriatic arthritis (PsA) required an update of the EULAR 2019 recommendations for the pharmacological treatment of PsA.

Methods: Following EULAR standardised operating procedures, the process included a systematic literature review and a consensus meeting of 36 international experts in April 2023. Levels of evidence and grades of recommendations were determined.

Objectives: To assess, in spondyloarthritis (SpA), the discriminative value of the Outcome Measures in Rheumatology (OMERACT) ultrasound lesions of enthesitis and their associations with clinical features in this population.

Methods: In this multicentre study involving 20 rheumatology centres, clinical and ultrasound examinations of the lower limb large entheses were performed in 413 patients with SpA (axial SpA and psoriatic arthritis) and 282 disease controls (osteoarthritis and fibromyalgia). 'Active enthesitis' was defined as (1) power Doppler (PD) at the enthesis grade ≥1 plus entheseal thickening and/or hypoechoic areas, or (2) PD grade >1 (independent of the presence of entheseal thickening and/or hypoechoic areas).

Neonatal jaundice is a frequently observed occurrence in full-term newborns and typically manifests between 48 and 96 hours following birth. Early-onset jaundice is primarily induced by pathological factors, namely sepsis, hemolysis and an excessive accumulation of bilirubin resulting from the breakdown of red blood cells. We present a case involving a full-term newborn with an uneventful perinatal history, who exhibited jaundice within the initial day of life and was subsequently admitted to the neonatal intensive care unit to commence intensive phototherapy.

A woman in her late 50s with recent onset of hypertension, diabetes, lumbar pain and unintentional weight loss was diagnosed with a cortisol and androgen-producing adrenal mass. Despite this, serum adrenocorticotropic hormone (ACTH) concentration was inappropriately elevated, which was investigated thoroughly. Investigations included a brain magnetic resonance imaging to exclude concomitant pituitary adenoma, a corticotropin-releasing hormone stimulation test and a gallium- and 18F-FDOPA PET scan, both excluding ectopic ACTH production.
