Introduction: Patients with head and neck cancer (HNC) have an elevated incidence of cachexia and malnutrition due to the tumor's location interfering with oral feeding. Concurrent chemoradiation (CCRT) can have an emetic effect and cause dysphagia and oral mucositis. Adequate nutrition improves immunity, raises the response to therapy, reduces adverse effects, and improves survival.

Article Synopsis
  • - The study investigates how prosthesis-patient mismatch (PPM) after transcatheter aortic valve implantation (TAVI) affects patient outcomes, particularly focusing on balloon-expandable and self-expandable valves in patients with small aortic annuli.
  • - Data was analyzed from a retrospective registry of 628 patients with severe aortic stenosis, revealing that those with severe PPM had a higher risk of all-cause mortality compared to those with less than moderate PPM, particularly after implantation of certain valve types.
  • - The findings conclude that patients with small aortic annuli and severe PPM face increased early mortality risk, while the use of supra-annular valves (SAV) appears to protect against severe PPM

Article Synopsis
  • - Multiple trichoepithelioma syndrome is a rare skin condition characterized by multiple benign skin lesions, typically requiring surgical excision for treatment, with diagnosis usually confirmed after pathology tests.
  • - In a specific case, a 36-year-old male patient visited a clinic for the first time, presenting with numerous skin lesions on his head and neck, leading to surgical intervention and consistent pathology results showing trichoepitheliomas without other types of tumors.
  • - The patient's tumors were larger and involved critical areas of the face, necessitating a more intricate surgical approach rather than the simple excision commonly performed for smaller lesions.

Objective: To evaluate the risk factors for lymph node metastasis (LNM) after a non-curative (NC) gastric endoscopic submucosal dissection (ESD) and to validate and eventually refine the eCura scoring system in the Western setting. Also, to assess the rate and risk factors for parietal residual disease.

Design: Retrospective multicentre multinational study of prospectively collected registries from 19 Western centres.

Article Synopsis
  • There has been a significant increase in the use of transcatheter aortic valve implantation (TAVI) to treat symptomatic severe aortic stenosis, prompting the need for streamlined procedures in specialized Heart Valve Centres.
  • The 2022 European TAVI Pathway Survey collected input from 147 TAVI centres in 26 countries, revealing details about their procedural practices and patient management.
  • Findings indicate that while minimalist TAVI methods are widely adopted, many patients still experience longer hospital stays, suggesting room for improvement in discharge processes.

Article Synopsis
  • Valvular heart disease (VHD) is a leading cause of heart failure and is often challenging to manage, especially in patients receiving conservative treatment.
  • Recent advancements in catheter-based interventions have expanded treatment options for both high-risk and lower-risk patients, potentially benefiting a broader range of individuals with VHD.
  • This clinical consensus statement aims to evaluate the evidence for managing cardiogenic shock related to left-sided VHD and propose a practical approach for using less invasive transcatheter interventions in challenging situations where traditional surgery poses high risks.

Brain metastasis in prostate cancer is quite a rare entity, especially when it manifests at diagnosis. The symptoms are usually non-focal and vary based on the location affected. It is almost always associated with a poor prognosis, with an overall survival of less than a year.

Article Synopsis
  • A study involving 157 patients focused on comparing calibrated IBS and 2D strain-derived IBS to the actual MF observed through cardiac imaging and biopsies, analyzing the data for correlation.
  • The results showed no significant correlation between IBS values and measures of myocardial fibrosis, indicating that IBS may not be a reliable indicator of fibrosis in patients with severe AS.

Article Synopsis
  • The study aimed to evaluate the distribution of Candida species and their antifungal resistance in cases of candidaemia across Europe, analyzing data collected from 41 centers in 17 countries between 2018 and 2022.
  • The most common isolates identified were C. albicans, C. glabrata, and C. parapsilosis, with notable geographical variations in their prevalence and antifungal resistance patterns.
  • Resistance to fluconazole was observed in certain species, particularly C. glabrata and C. parapsilosis, with rare cases of echinocandin resistance found, including specific genetic mutations in resistant isolates.

Background: Implant-based submuscular breast reconstruction (SBR) can be performed with the aid of acellular dermal matrices (ADM) for implant coverage on their inferolateral pole, aiming at providing a biological interface for hiding the implant and therefore reducing the risk of complications. The purpose of this study is to assess the long-term post-operative outcomes obtained using the SBR-specific Native® ADM (DECO med s.r.

Contextual factors influencing the use of coercive measures in Portuguese mental health care.

Int J Law Psychiatry

November 2023

Lisbon Institute of Global Mental Health, Lisbon, Portugal; Comprehensive Health Research Centre (chrc), NOVA Medical School, NOVA University of Lisbon, Lisbon, Portugal.

The use of coercive measures in mental health care is an important indicator of the quality of care being provided, and non-patient-related factors are increasingly recognized to contribute to their use. The study aimed to explore the perspectives of mental health care professionals who have first-hand experience with the use of coercion on the contextual factors that influence the use of coercion in the Portuguese mental health care. Five focus group discussions were conducted among 23 doctors and 17 nurses from five psychiatric departments in urban and rural regions of Portugal.

Torque teno virus (TTV) was recently identified as a potential biomarker for the degree of immunosuppression, and potentially as a predictor of rejection and infection in solid organ transplant patients. We evaluated TTV viral load in kidney transplant (KT) patients during the first year post-transplant to examine overall kinetics and their relationships with deleterious events, including episodes of infection and the formation of de novo donor-specific antibodies (DSAs). In a single-center, prospective observational cohort study, 81 KT patients were monitored at baseline, week 1, and month 1, 3, 6, 9 and 12, post-KT, and whenever required by clinical events.

Non-invasive brain stimulation techniques (NIBS) have been widely used in both clinical and research contexts in neuropsychiatry. They are safe and well-tolerated, making NIBS an interesting option for application in different settings. Transcranial magnetic stimulation (TMS) is one of these strategies.

Cholesteryl hemiazelate identified in CVD patients causes in vitro and in vivo inflammation.

Oxidation of PUFAs in LDLs trapped in the arterial intima plays a critical role in atherosclerosis. Though there have been many studies on the atherogenicity of oxidized derivatives of PUFA-esters of cholesterol, the effects of cholesteryl hemiesters (ChEs), the oxidation end products of these esters, have not been studied. Through lipidomics analyses, we identified and quantified two ChE types in the plasma of CVD patients and identified four ChE types in human endarterectomy specimens.

The field of Cardio-Oncology has grown significantly, especially during the last decade. While awareness of cardiotoxicity due to cancer disease and/or therapies has greatly increased, much of the attention has focused on myocardial systolic disfunction and heart failure. However, coronary and structural heart disease are also a common issue in cancer patients and encompass the full spectrum of cardiotoxicity.
