Multicentric Study on the Diagnostic of Neuromuscular Diseases in Children with High Creatinine Phosphokinase Levels.

Children (Basel)

November 2024

Department of Neuropediatrics, Centro Materno-Infantil do Norte, Centro Hospitalar Universitário de Santo António (CHUdSA), Largo da Maternidade de Júlio Dinis 45, 4050-651 Porto, Portugal.

Background: Neuromuscular diseases (NMDs) are rare, predominantly hereditary, with progressive course disorders. Furthermore, diagnosis can be delayed by years after symptoms emerge, resulting in missed opportunities for modifying disease progression, specific therapeutic approaches, and counseling. Some NMDs have high levels of creatine phosphokinase (CK).

A 73-year-old man presented with nausea, abdominal discomfort, and distention persisting for the past five days. He had previously been diagnosed with stage III peripheral CD4+ T cell lymphoma and had initiated chemotherapy comprising vincristine two weeks prior to presentation. An evaluation revealed diffuse colon distention and pneumatosis intestinalis without mechanical obstruction, consistent with acute colonic pseudo-obstruction.

The original LaPrade technique for anatomic reconstruction of the posterolateral corner of the knee uses two separate allografts. More recently, a modification of this technique, using an adjustable-length suspension device with a cortical button for tibial fixation, allows anatomic reconstruction with a single semitendinosus autograft. This modification is of utmost relevance when sources of allograft are not available for multiligament knee reconstruction.

Background: Carotid endarterectomy (CEA) carries a risk of perioperative stroke, particularly during carotid cross-clamping. While neurological monitoring is reliable for patients under regional anesthesia, alternative options are needed for those unsuitable for regional anesthesia. Near-infrared spectroscopy (NIRS) is commonly used during CEA, but its diagnostic accuracy varies, particularly under general anesthesia.

Introduction: Knowledge about the effect of disease modifying treatment (DMT) in late-onset multiple sclerosis (LOMS, onset ≥50 years-old) is scarce. This study aims to evaluate the association between DMT use and multiple sclerosis (MS) evolution in a LOMS cohort.

Methods: This multicentre, retrospective and observational study included LOMS patients with ≥2 years of follow-up.

It is essential to comprehend the clinical manifestations of hereditary hemorrhagic telangiectasia (HHT), also known as Osler-Weber-Rendu syndrome. This autosomal dominant vascular disorder presents with distinct symptoms, including mucocutaneous telangiectasia, epistaxis, gastrointestinal bleeding, and iron deficiency anemia. Furthermore, arteriovenous malformations (AVMs) commonly occur in the pulmonary, hepatic, and cerebral circulations.

In this article, we aim to present the case of a five-year-old child with traumatic spondylolisthesis of L5-S1 associated with a sacral fracture, manifesting as a spinopelvic dissociation. The goal of surgical treatment is to reduce the deformity and provide posterior fixation to restore the sagittal alignment of the lumbosacral junction. One-year follow-up showed good clinical and radiological outcomes.

Article Synopsis
  • Interstitial lung disease (ILD) is prevalent in rheumatoid arthritis (RA) and linked to higher mortality; this study assesses antifibrotics nintedanib and pirfenidone's real-world effectiveness and safety in RA-ILD patients.
  • A multicenter observational study included 40 RA-ILD patients, primarily treated with nintedanib; data on demographics, pulmonary function tests (PFTs), and adverse events (AEs) were collected and analyzed.
  • Results showed that antifibrotics halted the decline in forced vital capacity (FVC) and total lung capacity (TLC) after initiation, but diffusion capacity continued to decline, with 40% of patients experiencing adverse events.

Article Synopsis
  • * The study employed a SEIR model to analyze transmission dynamics and found that MSM with high sexual activity contributed significantly more to mpox spread than those with low sexual activity, but vaccinated individuals were less likely to spread the virus.
  • * Findings suggest that ongoing vaccination efforts, increased awareness in at-risk communities, and regular genomic surveillance are essential for managing future mpox outbreaks and potential threats from new viral clades.

Article Synopsis
  • About one-third of Portuguese patients with congenital hypogonadotropic hypogonadism (CHH) have a genetic cause linked to 10 different genes, with a small percentage showing oligogenic inheritance.
  • CHH leads to delayed puberty and infertility due to issues with hormones like GnRH, LH, and FSH.
  • In a study involving 81 patients and 263 controls, genetic screening helped identify pathogenic variants, but many variants classified as uncertain complicate the understanding of the disease causes.

Contextual Hospital Conditions and the Risk of Nosocomial SARS-CoV-2 Infection: A Matched Case-Control Study with Density Sampling in a Large Portuguese Hospital.

J Clin Med

September 2024

Departamento de Ciências da Saúde Pública e Forenses e Educação Médica, Faculdade de Medicina da Universidade do Porto, 4200-319 Porto, Portugal.

Knowledge of the role of hospital conditions in SARS-CoV-2 transmission should inform strategies for the prevention of nosocomial spread of this pathogen and of similarly transmitted viruses. This study aimed to identify risk factors for nosocomial acquisition of SARS-CoV-2. We ran a nested case-control study with incidence density sampling among adult patients hospitalized for >7 days (August-December 2020).

Prognostic factors associated with disability in a cohort of neuromyelitis optica spectrum disorder and MOG-associated disease from a nationwide Portuguese registry.

J Neurol Sci

September 2024

Neurology Service, Department of Neurosciences, Centro Hospitalar Universitário de Santo António, Porto, Portugal; Unit for Multidisciplinary Research in Biomedicine, Instituto de Ciências Biomédicas de Abel Salazar da Universidade do Porto, Porto, Portugal.

Introduction: Neuromyelitis optica spectrum disorders (NMOSD) and MOG-associated disease (MOGAD) are an increasingly recognized group of demyelinating disorders of the central nervous system. Previous studies suggest that prognosis is predicted by older age at onset, number of relapses, the severity of the first attack and autoantibody status.

Objective: To study prognostic factors associated with disability progression and additional relapses in the 3-year follow-up of a national NMOSD/MOGAD cohort.

Background: Laryngeal involvement is rare in tuberculosis, representing around 1% of all cases of this infection worldwide. Given the larynx' location in the airway, this form of tuberculosis is of particular importance because it is highly contagious. With our hospital being in a high tuberculosis burden area, we propose to characterize the clinical presentation, evolution, and laryngoscopy findings of a series of laryngeal tuberculosis cases in order to reduce misdiagnosis.

Article Synopsis
  • - Bacterial tracheitis (BT) is a rare but serious condition that can lead to severe airway obstruction, primarily exhibiting symptoms like stridor, fever, and rapid breathing, with a recent increase in respiratory distress cases.
  • - A case study of a five-year-old boy highlights rapid clinical deterioration despite treatment attempts, including corticosteroids and nebulized epinephrine, ultimately requiring intubation due to respiratory failure.
  • - The case emphasizes the need for healthcare professionals to recognize classic and evolving symptoms of BT and be prepared for immediate invasive interventions, as effective treatment can lead to significant recovery.

The perianal disease affects up to one-third of individuals with Crohn's disease (CD), causing disabling symptoms and significant impairment in quality of life, particularly for those with perianal fistulising CD (PFCD). The collaborative effort between gastroenterologists and surgeons is essential for addressing PFCD to achieve fistula closure and promote luminal healing. Limited fistula healing rates with conventional therapies have prompted the emergence of new biological agents, endoscopic procedures and surgical techniques that show promising results.
