Background: Renal replacement therapy (RRT) is essential in the presence of life-threatening complications associated with acute kidney injury (AKI). In the absence of urgent indications, the optimal timing for RRT initiation is still under debate. This meta-analysis aims to compare the benefits between early and late RRT initiation strategies in critically ill patients with AKI.

Article Synopsis
  • * Group 4 (G4) MB, the most common subtype, is characterized by somatic mutations affecting the core binding factor alpha (CBFA) complex, including changes to genes like CBFA2T2 and OTX2.
  • * Research indicates that G4 MB cells resemble early progenitor cells in the cerebellar region but are stalled in development; targeting OTX2 may help overcome this block and allow these cells to mature normally.

Article Synopsis
  • The COVID-19 pandemic has significantly impacted healthcare systems globally, leading to changes in interventional cardiology practices, particularly in Portugal.
  • A study analyzed data from the Portuguese Registry on Interventional Cardiology to compare the volume of percutaneous coronary interventions (PCIs) performed during the initial outbreak from March to May 2020 against data from the same period in the past three years (2017-2019).
  • Results indicated a 36% decrease in total PCI procedures during the first COVID-19 wave, with notable reductions of 25% for STEMI, 20% for NSTE-ACS, and 59% for chronic coronary syndromes.

Kidney Cancer Biomarker Selection Using Regularized Survival Models.

Cells

July 2022

INESC-ID, Instituto Superior Técnico, Universidade de Lisboa, Rua Alves Redol 9, 1000-029 Lisbon, Portugal.

Clear cell renal cell carcinoma (ccRCC) is the most common subtype of RCC showing a significant percentage of mortality. One of the priorities of kidney cancer research is to identify RCC-specific biomarkers for early detection and screening of the disease. With the development of high-throughput technology, it is now possible to measure the expression levels of thousands of genes in parallel and assess the molecular profile of individual tumors.

Screening a Targeted Panel of Genes by Next-Generation Sequencing Improves Risk Stratification in Real World Patients with Acute Myeloid Leukemia.

Cancers (Basel)

June 2022

Instituto de Medicina Molecular João Lobo Antunes, Faculdade de Medicina, Universidade de Lisboa, 1649-028 Lisboa, Portugal.

Article Synopsis
  • This study examines the impact of next-generation sequencing (NGS) on the risk assessment of acute myeloid leukemia (AML) patients, focusing on a cohort of 268 newly diagnosed individuals.
  • Initially, 23% of patients could not be categorized into risk groups, but this number dropped to 10% after incorporating NGS data, highlighting NGS's role in improving risk stratification.
  • The findings reveal not only a refinement in risk classifications—resulting in 62 patients being better assessed—but also unexpected clinical outcomes tied to specific gene mutations, emphasizing the need for further research in this area.

Objective: To evaluate the influence of patient characteristics on hyperlactatemia in an infected population admitted to intensive care units and the influence of hyperlactatemia severity on hospital mortality.

Methods: A post hoc analysis of hyperlactatemia in the INFAUCI study, a national prospective, observational, multicenter study, was conducted in 14 Portuguese intensive care units. Infected patients admitted to intensive care units with a lactate measurement in the first 12 hours of admission were selected.

Article Synopsis
  • Recent studies showed a rise in cerebral venous thrombosis (CVT) cases during the COVID-19 pandemic, prompting this evaluation of hospitalization and mortality rates comparing the pandemic year (2020) to the year prior (2019).
  • A cross-sectional study analyzed data from 171 stroke centers worldwide, focusing on CVT admissions and associated mortality from January 2019 to May 2021, revealing no significant differences in overall CVT volume or mortality between 2019 and 2020.
  • However, the first five months of 2021 saw a notable increase in CVT admissions compared to 2019 and 2020, with higher mortality rates linked to COVID-19-positive patients and instances of vaccine-induced immune

Invasive and Subcutaneous Infections Caused by Filamentous Fungi: Report from a Portuguese Multicentric Surveillance Program.

Microorganisms

May 2022

Reference Unit for Parasitic and Fungal Infections, Department of Infectious Diseases, National Institute of Health, Dr. Ricardo Jorge, 1649-016 Lisbon, Portugal.

Invasive fungal infections (IFI) have significantly increased over the past years due to advances in medical care for the at-risk immunocompromised population. IFI are often difficult to diagnose and manage, and can be associated with substantial morbidity and mortality. This study aims to contribute to understanding the etiology of invasive and subcutaneous fungal infections, their associated risk factors, and to perceive the outcome of patients who developed invasive disease, raising awareness of these infections at a local level but also in a global context.

The role of endobronchial ultrasound (EBUS) and trans-esophageal endobronchial ultrasound (EUS-B) in lung cancer is well established and scientifically validated. There is increasing data that endosonography is a crucial tool for the diagnosis of central lung lesions, and mediastinal staging and restaging of non-small cell lung cancer patients. The present article reviews the technical aspects of EBUS and EUS-B and focus on the last published research regarding its value in lung cancer.
