Research Question: Shared decision-making has become a hallmark of quality care and is increasingly spotlighted in practice guidelines. Little is known about women's views for treatment of less active ectopic pregnancy. What are the preferences of women for less active ectopic pregnancy treatment-related attributes?

Design: A discrete choice model with 8 attributes depicting ectopic pregnancy treatment including varying levels of first-line treatment effectiveness, length of hospitalization, cost, length of sick leave, of convalescence, need for surgical management, for emergency care during convalescence and for tube removal was used.

  • Women's preoperative hopes and fears about cystocele repair often differ from those of their surgeons, highlighting a gap in understanding.
  • A study analyzed responses from 265 women, revealing prevalent hopes related to prolapse repair (60%) and fears of prolapse relapse (38%).
  • Surgeons' expectations aligned closely with women's, but only 60% of women reported prolapse repair as a key expectation, suggesting the need for enhanced communication between patients and surgeons regarding individual concerns and goals.

Article Synopsis
  • Ulipristal acetate (UPA) is a medication used for emergency contraception and treating uterine fibroids, leading researchers to investigate changes in miRNA expression in the endometrium after at least 3 months of UPA treatment.
  • A comparative study analyzed miRNA expressions in tissue samples from women who underwent UPA treatment versus control groups of UPA-naïve women in different menstrual phases using microarray and pathway analysis.
  • The results showed that UPA affected 59 miRNAs related to cell cycle, cancer, and inflammation, with distinct expression profiles in the basal and superficial endometrial layers, indicating a need for further large-scale studies on the long-term effects of SPRM on endometrial miRNA.

  • Due to instances of liver toxicity linked to UPA, new recommendations from the PRAC and EMA now require assessing liver function before treatment and monitoring during it.
  • Access to repeated UPA treatments is now limited to women deemed "non-eligible" for surgery, prompting a discussion on what 'non-eligibility' actually means.

Digital diabetes: Perspectives for diabetes prevention, management and research.

Diabetes Metab

September 2019

Inserm UMR1153, Paris Descartes University, AP-HP, centre of research in epidemiology and statistics, Sorbonne Paris Cité (CRESS), 75004 Paris, France.

Digital medicine, digital research and artificial intelligence (AI) have the power to transform the field of diabetes with continuous and no-burden remote monitoring of patients' symptoms, physiological data, behaviours, and social and environmental contexts through the use of wearables, sensors and smartphone technologies. Moreover, data generated online and by digital technologies - which the authors suggest be grouped under the term 'digitosome' - constitute, through the quantity and variety of information they represent, a powerful potential for identifying new digital markers and patterns of risk that, ultimately, when combined with clinical data, can improve diabetes management and quality of life, and also prevent diabetes-related complications. Moving from a world in which patients are characterized by only a few recent measurements of fasting glucose levels and glycated haemoglobin to a world where patients, healthcare professionals and research scientists can consider various key parameters at thousands of time points simultaneously will profoundly change the way diabetes is prevented, managed and characterized in patients living with diabetes, as well as how it is scientifically researched.
