Ultra-overt therapy: a novel medical approach centered on patient consciousness.

Within the realms of human and artificial intelligence, the concepts of consciousness and comprehension are fundamental distinctions. In the clinical sphere, patient awareness regarding medication and its physiological processes plays a crucial role in determining drug efficacy and outcomes. This article introduces a novel perspective on prescription practices termed "Ultra-Overt Therapy" (UOT).

Human T-cell lymphotropic virus screening in France: Missed opportunities? A retrospective multicenter study of serological testing in hospital laboratories.

Objectives: Human T-lymphotropic virus 1 infection is endemic in the French Antilles, French Guiana, and sub-Saharan Africa, the origin of many immigrants currently living in France. There are no national screening recommendations outside of the regulatory obligations concerning donations of blood, tissue, gametes, or milk to a lactarium. This study aimed to investigate the screening and diagnostic practices for this infection in France.

Prognosis of variceal and non-variceal upper gastrointestinal bleeding in already hospitalised patients: Results from a French prospective cohort.

Objectives: Patients who develop upper gastrointestinal bleeding (UGIB) while in hospital appear to have a poor prognosis. Our study aims at analysing the difference in outcome between in-patients (IPs) and out-patients presenting with variceal and non-variceal UGIB.

Methods: We conducted a multicentre prospective study by collecting data about variceal and non-variceal UGIB cases through 46 hospitals in France between November 2017 and October 2018.

A blood smear examination led to the diagnosis of severe bacterial infection in a splenectomized patient. The knowledge of a relevant clinical information (recent dog bite) could have led to immediate presumptive identification of the causative agent, Capnocytophaga canimorsus.

[Second- or third-line treatment with erlotinib in EGFR wild-type non-small cell lung cancer: Real-life data].

Introduction: The benefit of tyrosine kinase inhibitors for patients with an EGFR wild-type non-small cell lung cancer (NSCLC) remains controversial.

Methods: The survival of patients with an EGFR wild-type NSCLC who received second- or third-line erlotinib treatment was assessed using real-life data that had been collected in a prospective, national, multicenter, non-interventional cohort study.

Results: Data from 274 patients were analysed, 185 (68%) treated with erlotinib and 89 (32%) treated with supportive care only.

Alpha-1 antitrypsin deficiency in a French General Hospital: fortuitous detection rather than efficient screening.

Introduction: We studied the characteristics of the screening procedure for alpha-1 antitrypsin at Nevers Hospital (France), together with the performance of serum protein gel electrophoresis for the fortuitous detection of patients with deficiency.

Material And Methods: We carried out a retrospective study of requests for alpha-1 antitrypsin determination referred to the laboratory during 3 years. We compared these requests with the numbers of patients seen at the hospital and requiring screening according to international recommendations.

[Thrombosis during thrombopoietin receptor agonist treatment for immune thrombocytopenia. A French multicentric observational study].

Introduction: Thrombopoietin-receptor agonists (TPO-RA) are marketed for immune thrombocytopenia (ITP). They have been associated to thrombosis occurrence in randomized controlled trials. However, the characteristics of these thromboses in the real-life practice as well as their management are poorly known.

Aspergillus species are filamentous fungi of Ascomycete class. They are opportunistic pathogens frequently responsible for many severe infections. Among these species Aspergillus fumigatus is the most important human pathogen because of his thermophilic feature.

[Fulminant coma: think hyperammonemia and urea cycle disorders].

The authors report the case of 14-year-old boy admitted for acute coma without neurological focal symptom. The only relevant finding was the death of one uncle after a coma in the year 1992. This coma was associated with an ammonia blood level of 344 mumol l-1 and it rapidly lead to cerebral death despite a symptomatic treatment.

[Bilateral choroidal metastasis revealing disseminated cancer: a case report].

We report the case of a 46-year-old woman, with no prior medical history, who developed a metamorphopsia of the right eye, followed by visual impairment. Ophthalmological examination revealed bilateral choroidal metastasis. Further investigations led to the conclusion of multifocal metastasis of bronchial adenocarcinoma.

[Aggregation of erythrocyte suspensions of diabetic patients in dextran 70 solutions].

Our previous studies have shown an erythrocyte hyperaggregation during diabetes. This hyperaggregation phenomenon seems usually be promoted by quantitative plasma protein's changes mainly fibrinogen. The aim of the present work is to appreciate the possible effect of only diabetic red cells on aggregation induced by dextran 70 (Dx 70).

Transcutaneous oxygen pressure and hemorheology in diabetes mellitus.

Diabetes is associated with altered blood viscosity and abnormal tissue oxygenation. Transcutaneous oxygen tension is measured in 119 diabetic and 20 normal subjects. Measurements of transcutaneous oxygen pressure (TcPO2) are made by Oxymonitor SM 361 at 45 degrees C at the dorsum of the foot.

[Hemorheology of venous insufficiency].

Material And Methods: Measurements have been performed for 20 healthy control subjects and 20 patients presenting a venous insufficiency with varicose veins of lower limbs. Blood has been withdrawn from the veins of the lower limbs at rest and after 10 minutes of venous stasis (standing position and 100 mmHg tourniquet).

Red blood cell aggregation in diabetes mellitus.

The aim of this work is to study the red blood cell aggregation in diabetic patients. Investigations have been performed via rheometric technics on four various types of diabetics (diabetics with good metabolic control, diabetics with poor metabolic control, diabetics with a distal angiopathy, without clinical signs, diabetics with a distal angiopathy characterized by cutaneous trophic disorders). Results, compared with those of healthy subjects, show that the reversible phenomenon of erythrocyte aggregation is significantly modified in diabetic.

Pharmacoclinical study of the action of vinburnin versus placebo: study of the hemodynamic, microcirculatory and rheological effects in angiopathy of the lower limbs in diabetes mellitus.

The diabetic macroangiopathy of lower limbs in characterized by chronic hypoxia with several abnormalities of hemodynamic, microcirculatory, rheological and metabolic red cell parameters. This study, in a double blind test with placebo was carried out on 40 subjects: 20 normal subjects and 20 diabetic patients suffering from microangiopathy of lower limbs. Ten are treated by 45 mg iv per day of Vinburnin, the 10 others received a placebo.
