Sedation Practices in Mechanically Ventilated Neurocritical Care Patients from 19 Countries: An International Cohort Study.

Neurocrit Care

January 2025

Division of Neuroscience Critical Care, Departments of Neurology, Neurosurgery, and Anesthesiology and Critical Care Medicine, Johns Hopkins University School of Medicine, Baltimore, MD, USA.

Background: Our objective was to characterize the impact of common initial sedation practices on invasive mechanical ventilation (IMV) duration and in-hospital outcomes in patients with acute brain injury (ABI) and to elucidate variations in practices between high-income and middle-income countries.

Methods: This was a post hoc analysis of a prospective observational data registry of neurocritically ill patients requiring IMV. The setting included 73 intensive care units (ICUs) in 18 countries, with a total of 1,450 patients with ABI requiring IMV.

Background: Invasive mechanical ventilation can present complex challenges for patients with acute brain injury (ABI) in middle-income countries (MICs). We characterized the impact of country income level on weaning strategies and outcomes in patients with ABI.

Methods: A secondary analysis was performed on a registry of critically ill patients with ABI admitted to 73 intensive care units (ICUs) in 18 countries from 2018 to 2020.

Introduction: Novel approaches to improve long-term outcomes in kidney transplant recipients are required. Here, we present the 5-year data from a multicenter, prospective, Phase 3b trial evaluating treatment outcomes with standard (STD) or low (LOW) dose prolonged-release tacrolimus (TAC) combined with ACEi/ARB or other antihypertensive therapy (OAHT) in Canadian kidney transplant recipients.

Methods: Adult de novo kidney transplant recipients were randomized 2 × 2 to STD or LOW dose TAC and ACEi/ARB or OAHT.

Background: Spinal muscular atrophy (SMA) patients benefit from pre-mRNA splicing modifiers targeting the SMN2 gene, which aims to increase functional SMN production. The animal toxicity affecting spermatogenesis associated with one such treatment raised questions about male SMA patients' spermatogenesis.

Methods: This descriptive, cross-sectional study was conducted from June 2022 to July 2023.

Purpose: Pseudomonas aeruginosa clinical strains isolated harbored sometimes an atypical phenotype using the automated Vitek2: ciprofloxacin-susceptibility but levofloxacin-resistance according to 2019 CA-SFM criteria. The aims of this study are to investigate the resistance mechanism(s) involved and to identify the consequences on fluoroquinolone treatment.

Methods: Strain resistance profile, patient's data were recovered and reviewed from the database.

Objective: To evaluate DOAE and DFR outcomes after switching to tralokinumab or Janus kinase inhibitor (JAKi).

Methods: This retrospective study included 106 patients discontinuing dupilumab because of DOAEs and/or DFR.

Severe Allergic Contact Dermatitis From Octylisothiazolinone in Over-Ear Headphones: A Case Series.

Contact Dermatitis

December 2024

Department of Occupational and Environmental Diseases, Hotel-Dieu Hospital, AP-HP Centre - Université Paris Cité, AP-HP, Paris, France.

Article Synopsis
  • Octylisothiazolinone (OIT) is a biocide used in leather products, raising concerns about allergic reactions, particularly in headphones.
  • A study reported cases of allergic contact dermatitis (ACD) in patients linked to headphones containing OIT, confirming its strong allergic potential through patch testing and chemical analysis.
  • The findings suggest that OIT is a significant sensitizer and highlight the need for safer use guidelines and improved product labeling in the leather industry to protect consumers.

Background: Effectiveness of disease-modifying treatment (DMT) in people affected by primary progressive multiple sclerosis (PPMS) is limited. Whether specific subgroups may benefit more from DMT in a real-world setting remains unclear. Our aim was to investigate the potential effect of DMT on disability worsening among patients with PPMS stratified by different disability trajectories.

Background: Nutritional stress is a mechanism that allows tumor cells to evade the immune system. Arginine (ARG), an amino acid involved in immunomodulation, aids in regulating T-lymphocyte cell activity and the antitumor response. ARG deficiency in the tumor microenvironment can impair T-cell response while ARG supplementation may promote antitumor immune activity.

Unlabelled: Breast cancer is a public health issue worldwide. One of the main causes of death due to this disease is metastasis, which is understudied in our context. Thus, the objectives of this work were to: (1) estimate the frequency of synchronous metastatic breast cancer; (2) determine the overall survival rate; and (3) identify the main factors associated with metastatic breast cancer death in Malian women.

Background: The PEXIVAS (Plasma exchange and glucocorticoids in severe antineutrophil cytoplasmic antibody (ANCA)-associated vasculitis) trial showed that a reduced-dose glucocorticoid regimen (redGC) was non-inferior to a standard-dose regimen (standGC) with respect to death or end-stage kidney disease (ESKD) in patients with ANCA-associated vasculitis (AAV). However, the primary endpoint did not include disease progression or relapse, cyclophosphamide was the main induction therapy and rituximab (RTX)-treated patients tended to have a higher risk of death or ESKD with redGC. We aimed to evaluate the real-world use of redGC.
