BackgroundIn most countries, including France, data on clinical indications for outpatient antibiotic prescriptions are not available, making it impossible to assess appropriateness of antibiotic use at prescription level.AimOur objectives were to: (i) propose proxy indicators (PIs) to estimate appropriateness of antibiotic use at general practitioner (GP) level based on routine reimbursement data; and (ii) assess PIs' performance scores and their clinimetric properties using a large regional reimbursement database.MethodsA recent systematic literature review on quality indicators was the starting point for defining a set of PIs, taking French national guidelines into account.

Observatoire Français de la Sclérose en Plaques, Centre de Recherche en Neurosciences de Lyon, INSERM 1028 et CNRS UMR 5292, 69003 Lyon, France; Department of Radiology, Centre Hospitalier Lyon-Sud, Hospices Civils de Lyon, 69310 Pierre-Bénite, France; CREATIS, CNRS UMR 5220, INSERM U1044, 69622 Villeurbanne, France.

Purpose: The purpose of this study was to create an algorithm that combines multiple machine-learning techniques to predict the expanded disability status scale (EDSS) score of patients with multiple sclerosis at two years solely based on age, sex and fluid attenuated inversion recovery (FLAIR) MRI data.

Materials And Methods: Our algorithm combined several complementary predictors: a pure deep learning predictor based on a convolutional neural network (CNN) that learns from the images, as well as classical machine-learning predictors based on random forest regressors and manifold learning trained using the location of lesion load with respect to white matter tracts. The aggregation of the predictors was done through a weighted average taking into account prediction errors for different EDSS ranges.

Transient hypothyroxinaemia of prematurity (THOP) presents as decreased free thyroxine without an increase in thyroid stimulating hormone. Thyroxine availability is important in case of premature birth, and THOP could be associated with impaired adaptation to extra-uterine life but the association of thyroxine level and clinical status has not yet been clearly defined. To defined a free thyroxine threshold likely associated with neonatal clinical impairment and outcomes at age three years.

Article Synopsis
  • Endothelial cells are crucial for cardiovascular health but their function declines with risk factors and improves when these risks are managed.
  • This paper emphasizes the need for more research on the different types of endothelial cells and their impact on cardiovascular disease, highlighting the absence of a perfect method for assessing endothelial function.
  • It suggests adopting standardized protocols for existing techniques like flow-mediated dilatation, exploring newer and simpler methods, and integrating these measurements with genetic and biomarker data for better patient diagnosis and treatment.
