Article Synopsis
  • Identification of cardiogenic shock severity is important for managing patients upon admission, and this study focuses on the role of peripheral tissue perfusion signs, specifically capillary refill time, in predicting outcomes.
  • The study involved 61 patients admitted to ICU with cardiogenic shock, measuring various hemodynamic parameters and capillary refill time at multiple intervals to assess their relationship with mortality and VA-ECMO support needs.
  • Findings showed that longer capillary refill times (greater than 3 seconds) at admission were strongly linked to 90-day mortality and the requirement for VA-ECMO, while capillary refill time was less correlated with macro-circulatory parameters but significantly related to microcirculatory measures.

Background: Diagnosis of antiphospholipid syndrome (APS) requires persistent presence of lupus anticoagulant (LAC), anticardiolipin (aCL) IgG/IgM, or anti-β2 glycoprotein I (aβ2GPI) IgG/IgM antibodies. Other antiphospholipid antibodies (aPL) such as antiphosphatidylserine/prothrombin antibodies (aPS/PT) are promising in assessment of thrombotic APS (TAPS).

Aim: To evaluate the added value of aPS/PT IgG and IgM in TAPS.

Prenatal exome sequencing could be complex because of limited phenotypical data compared to postnatal/portmortem phenotype in fetuses affected by multiple congenital abnormalities (MCA). Here, we investigated limits of prenatal phenotype for ES interpretation thanks to a blindly reanalysis of postmortem ES data using prenatal data only in fetuses affected by MCA and harboring a (likely)pathogenic variant or a variant of unknown significance (VUS). Prenatal ES identified all causative variant previously reported by postmortem ES (22/24 (92%) and 2/24 (8%) using solo-ES and trio-ES respectively).

We describe the management of post-traumatic stress disorder (PTSD) and obsessive-compulsive disorders (OCD), through Cognitive Behavioral Therapy (CBT) in a Huntington's disease (HD) patient.Even with a psychopharmacological treatment to manage PTSD and OCD, psychiatric disorders remained in the HD patient. We proposed CBT essentially based on progressive exposure.

Introduction: Expanded-criteria donors (ECDs) are used to reduce the shortage of kidneys for transplantation. However, kidneys from ECDs are associated with an increased risk of delayed graft function (DGF), a risk factor for allograft loss and mortality. HYPOREME will be a multicentre randomised controlled trial (RCT) comparing targeted hypothermia to normothermia in ECDs, in a country where the use of machine perfusion for organ storage is the standard of care.

Case Report: Atonic PNES Capture in fMRI.

Front Neurol

Pôle Hospitalo-Universitaire de Psychiatrie d'Adultes et d'Addictologie du Grand Nancy, Centre Psychothérapique de Nancy, Laxou, France.

Psychogenic Non-Epileptic Seizures (PNES) are a misunderstood and disabling pathology, characterized by a paroxysmal occurrence of clinical signs without the epileptic activity. Resting-state functional MRI (fMRI) studies in patients with PNES have shown abnormal functional connectivity of the resting-state networks, especially in the limbic and motor systems, and in the precuneus. However, the transient nature of PNES episodes prevents us from elucidating the underlying mechanisms of seizures.

Article Synopsis
  • The SERENE UC study aimed to assess the effectiveness of higher doses of adalimumab (a medication for ulcerative colitis) in patients with moderate to severe cases.
  • This phase 3 trial involved a main study plus a Japan substudy, with participants randomly assigned to either higher or standard dosing regimens for both induction and maintenance phases.
  • While the study did not meet its primary endpoints, it indicated a potential benefit with higher maintenance doses, suggesting improved rates of clinical remission while maintaining a similar safety profile to standard dosing.

Article Synopsis
  • About half of patients with heart failure (HF) have HF with preserved ejection fraction (HFpEF), which poses challenges in diagnosis and treatment due to its complex underlying mechanisms.
  • Advanced diagnostic techniques may be needed to distinguish HFpEF from other causes of breathing difficulties, as traditional reliance on left ventricular ejection fraction (LVEF) cut-points may not adequately capture the nuances of this condition.
  • Effective management of HFpEF includes lifestyle modifications, treatment of comorbidities, and the use of specific medications like sodium-glucose cotransporter 2 inhibitors, while a phenotypic classification approach may offer more tailored treatment strategies compared to the standard LVEF categories.
