Sex and the Risk of Atheromatous and Nonatheromatous Cardiovascular Disease in CKD: Findings From the CKD-REIN Cohort Study.

Am J Kidney Dis

November 2024

Centre for Research in Epidemiology and Population Health, Paris-Saclay University, Institut National de la Santé et de la Recherche Médicale (INSERM) U1018, Clinical Epidemiology Team, Versailles Saint-Quentin University, Villejuif.

Article Synopsis
  • The study investigates how chronic kidney disease (CKD) affects sex differences in cardiovascular disease (CVD) risk, specifically distinguishing between atheromatous CVD (ACVD) and nonatheromatous CVD (NACVD).
  • Utilizing data from a cohort of nearly 3,000 patients with moderate to severe CKD across France, the results indicate that women have a significantly lower rate of ACVD compared to men, but no significant difference in NACVD rates was found between the sexes.
  • The findings highlight that as kidney function declines (measured by eGFR), the sex differences in ACVD risk diminish, whereas NACVD risk remains consistent across both sexes, suggesting gender impacts risk differently based on CVD

Article Synopsis
  • The study aims to enhance the consistency of interpreting anticardiolipin (aCL) and anti-β2-glycoprotein I (aβ2GPI) antibody tests for diagnosing antiphospholipid syndrome (APS) by establishing moderate and high thresholds using various testing methods.
  • Research involved analyzing samples from 381 APS patients and 727 controls across four different immunoassay systems to calculate likelihood ratios (LRs) that could distinguish between disease states.
  • The findings show that the defined thresholds lead to better harmonization in interpreting antibody levels, particularly for IgG aCL and aβ2GPI, suggesting a more reliable method for diagnosing APS that requires further validation.

Electronic address:

Genetic cerebellar ataxias are still a diagnostic challenge, and yet not all of them have been identified. Very recently, in early 2023, a new cause of late-onset cerebellar ataxia (LOCA) was identified, spinocerebellar ataxia 27B (SCA27B). This is an autosomal dominant ataxia due to a GAA expansion in intron 1 of the FGF14 gene.

Aims: Insulin-like growth factor binding protein-7 (IGFBP7) is a biomarker of tissue senescence with a role in cardio-renal pathophysiology. The role of IGFBP7 as a prognostic biomarker across the full ejection fraction (EF) spectrum of heart failure (HF) remains less well understood. We examined associations between IGFBP7 and risk of cardio-renal outcomes regardless of EF and the effect of empagliflozin treatment on IGFBP7 concentrations among individuals with HF.

Sudden cardiac death (SCD) and ventricular fibrillation are rare but severe complications of many cardiovascular diseases and represent a major health issue worldwide. Although the primary causes are often acute or chronic coronary diseases, genetic conditions, such as inherited channelopathies or non-ischemic cardiomyopathies are leading causes of SCD among the young. However, relevant experimental models to study the underlying mechanisms of arrhythmias and develop new therapies are still needed.

Use of the Win Ratio Analysis in Critical Care Trials.

Am J Respir Crit Care Med

April 2024

Université de Lorraine, Centre d'Investigations Cliniques Plurithématique, Institut National de la Santé et de la Recherche Médicale U1116, Nancy, France.

Article Synopsis
  • Composite outcomes in critical care trials often treat all events equally, which can misrepresent their clinical importance; for example, death is more significant than organ failure.
  • The win ratio (WR) method offers an alternative way to evaluate these outcomes by prioritizing events based on their clinical relevance, making it useful for trials with smaller sample sizes common in critical care.
  • While WR has potential benefits for more accurately assessing treatment effects, it also faces challenges like ignoring tied outcomes and difficulties in interpreting clinical significance, which are discussed alongside its application to recent trials like HYPO-ECMO.
