Anterior transolecranon fracture-dislocations of the elbow are rare in children and the literature is poor in recommendations for the management of these lesions. We report a new case with a type of lesion that has not been described previously and discuss this pathology. Based on a literature review, we propose a classification into four types of anterior transolecranon fracture-dislocations of the elbow in children, thereby guiding surgical indications.

Purpose: The respective roles of the dorsoradial (DRL) and anterior oblique (AOL) ligaments in stability of the highly mobile trapeziometacarpal (TMC) joint remain disputed. Earlier publications have pointed to the AOL as the key stabilizing structure; yet, more recent publications have challenged the stabilizing role of the AOL, favoring the DRL as the main TMC joint stabilizer. We executed an anatomical study of the ligaments, including detailed dissection to quantify the length, width, and thickness of the AOL and DRL and tested the material properties of these ligaments.

[The modified Lamberty and Cormack's flap for neuropathies of the ulnar nerve at the elbow].

Chir Main

December 2013

Clinique du Parc, 155, boulevard Stalingrad, 69006 Lyon, France.

Surgical treatment of ulnar nerve syndrome at the elbow gives good results in the majority of cases but there are sometimes incomplete results or recurrences that can lead to iterative procedures. This iterative surgery is responsible for painful neurological sequelae often resistant to pharmacological treatments. In these selected resistant cases, we made the choice of a surgical treatment associating a neurolysis of the ulnar nerve and wrapping with a vascularized flap modified from Lamberty and Cormack.

A spontaneous rupture of the extensor pollicis longus (EPL) tendon occurred in a patient with trapeziometacarpal arthritis. We hypothesize that a mechanical attrition by protrusion at the trapeziometacarpal joint alone was responsible of a spontaneous rupture of the extensor pollicis longus tendon.

Could this unusual scaphoid fracture occurring in a badminton player be a stress fracture?

Chir Main

February 2004

Centre de Chirurgie de la Main et de Microchirurgie, Clinique du Parc Léopold, SOS Main Bruxelles, 38 rue Froissart, 1040 Brussels, Belgium.

An unusual fracture of the scaphoid occurred in an otherwise healthy young badminton player, caused by a violent movement of extension/flexion of the wrist while performing a smash. There was no direct blow or fall on the wrist, nor history of wrist pain prior to the fracture. No underlying pathology was identified.

Some authors attribute recurrences of giant cell tumours to biological factors which are only expressed in some tumours. Grover et al. (1998) suggested that the risk for recurrence is associated with the down-regulation of the nm23-H1 gene.

Centre de Chirurgie de la Main et de Microchirurgie, Clinique du Parc Léopold, 38 rue Froissart, 1040 Brussels, Belgium; ; Department of Plastic and Reconstructive Surgery, University of Montreal, Quebec.

Background: Regional anesthesia of a single finger is commonly achieved by the traditional ring block, which requires at least two painful injections in the digit. Single injection digital block techniques have been described to avoid this problem. Among these, the subcutaneous technique described by Harbison appears to be safe and to allow most procedures to be carried out with good tolerance.

Although based on their long-term clinical evolution there appeared to be no doubt that fractures of the scaphoid modify the mechanical behavior of the carpus, the mechanisms of these modifications have not yet been investigated. This study based on finite element analysis provides insight into the sequence behind the onset of arthritis of the wrist, highlighting the existence of pressure peaks at the nonunion and at the midcarpal interface (scaphoid-capitate and lunate-capitate). This evidence explains the clinical evolution of nonunions of the scaphoid.

Unlabelled: A new surgical procedure has been conceived to avoid persistent pain and to restore the functionality and pinch strength of the basal joint of the thumb destroyed by degenerative osteoarthritis. Since January 1998 a double total joint transposition was performed on eight patients, switching the trapezoid-metacarpal joint and the trapezio-metacarpal joint. A 180 degrees rotation of the bi-articular complex (trapezoid-trapezio-metacarpal joints) in its entirety was achieved, while preserving the intrinsic ligaments and vascular supply insured by the dorsal branch of the radial artery; the Cardan conformation in Physics of the trapezoid-metacarpal joint provided complete thumb motion.

Scaphoid fractures are the most common carpal fractures. Conservative treatment is long, and non-union is frequent after immobilization in a cast. The Herbert procedure is an improvement, but access to the scaphoid is through an extensive open exposure that damages the blood vessels and the anterior radiocarpal ligaments.
