[Risk of acquiring antibiotic-resistant bacteria and travel].

Rev Prat

October 2024

Service de bactériologie, hôpital Bichat-Claude-Bernard, AP-HP Nord-Université Paris Cité IAME Inserm, UMR 1137, Université Paris Cité, Paris, France.

Article Synopsis
  • International travel increases exposure to different bacteria, raising the risk of acquiring antibiotic-resistant infections for travelers and enabling the spread of these bacteria globally.
  • Travelers are especially vulnerable to multidrug-resistant Enterobacterales, particularly when visiting South Asia, often due to factors like diarrhea and antibiotic use.
  • Serious infections such as shigellosis, typhoid fever, and skin infections from MRSA show the urgent need for pre-travel consultations to help minimize the risk of antibiotic resistance acquisition.

Article Synopsis
  • Nocardiosis is a serious infection that can occur after hematopoietic cell transplantation (HCT), and this study aimed to identify its risk factors and the impact of certain preventive treatments.
  • A review of 64 cases of nocardiosis and 128 matched controls showed that factors like tacrolimus use, low lymphocyte counts, male sex, recent corticosteroid use, and previous CMV infections increased the risk of developing this infection.
  • Conversely, patients on trimethoprim-sulfamethoxazole prophylaxis had a significantly lower risk of nocardiosis; however, those who did develop it had poorer survival rates compared to controls.

Determinants of radiological patterns and severity in immunocompromised adults with Metapneumovirus infection.

Respir Med

June 2024

Université Paris Cité, Service de Maladies Infectieuses et Tropicales, Hôpital Necker-Enfants Malades, Centre d'Infectiologie Necker-Pasteur, Assistance Publique-Hôpitaux de Paris, Paris, France.

Background: Human Metapneumovirus (HMPV) belongs to the Pneumoviridae family and is responsible for respiratory infections. Mild infections are well-recognized in children, while its precise impact in various categories of immunocompromised adults has not been well addressed.

Research Question: We retrospectively studied HMPV infections in immunocompromised adults followed in two large French university medical centers.

Global guideline for the diagnosis and management of cryptococcosis: an initiative of the ECMM and ISHAM in cooperation with the ASM.

Lancet Infect Dis

August 2024

Division of Infectious Diseases, Department of Medicine, Duke University Medical Center, Durham, NC, USA; Department of Molecular Genetics and Microbiology, Duke University Medical Center, Durham, NC, USA. Electronic address:

Cryptococcosis is a major worldwide disseminated invasive fungal infection. Cryptococcosis, particularly in its most lethal manifestation of cryptococcal meningitis, accounts for substantial mortality and morbidity. The breadth of the clinical cryptococcosis syndromes, the different patient types at-risk and affected, and the vastly disparate resource settings where clinicians practice pose a complex array of challenges.

Performance of clinical metagenomics in France: a prospective observational study.

Lancet Microbe

January 2024

Microbiology Department, AP-HP, Hôpital Necker, Paris, France; Université Paris Cité, CNRS, INSERM, Institut Necker-Enfants Malades, Team Pathogenesis of Systemic Infection, Paris, France. Electronic address:

Background: Metagenomic next-generation sequencing (mNGS) allows untargeted identification of a broad range of pathogens, including rare or novel microorganisms. Despite the recognition of mNGS as a valuable diagnostic tool for infections, the most relevant indications for this innovative strategy remain poorly defined. We aimed to assess the determinants of positivity and clinical utility of mNGS.

Electronic address:

Article Synopsis
  • - The study investigates the relationship between genetic factors and the severity of adult hemophagocytic lymphohistiocytosis (HLHa), focusing on gene variants related to HLH.
  • - Clinical data from 130 HLHa patients, including their genotypes for 8 specific HLH-related genes, were analyzed to assess disease outcomes, with findings showing a significant association between the number of genetic variants and severity of HLHa.
  • - Results indicate that HLH gene variants can impact the severity and treatment resistance in HLHa patients, highlighting their potential role in understanding the condition better.

Article Synopsis
  • Researchers tested a cream containing paromomycin (with or without gentamicin) on travelers with cutaneous leishmaniasis (CL), achieving cure rates of 75-82%, comparable to clinical trials.
  • Two groups were studied: one treated under supervision (Group 1) and another using a locally produced paromomycin-only cream (Group 2), both showing similar medical outcomes despite some differences in cure rates.
  • Adverse effects were mostly mild, with common issues being itching and pain, and no serious complications or mucosal involvement occurred throughout the study.

Eculizumab, a recombinant humanized monoclonal antibody (mAb), is used for the treatment of patients (both adults and children) with paroxysmal nocturnal hemoglobinuria and atypical hemolytic uremic syndrome. This mAb binds to complement protein 5 (C5), thereby inhibiting its cleavage. On the other hand, one of the C5 cleavage products, C5a, is a potent anaphylatoxin with proinflammatory properties, involved in antimicrobial surveillance.

A Functional Polymorphism in IL-1B Is Associated With Immune Reconstitution Inflammatory Syndrome of Chronic Disseminated Candidiasis.

Open Forum Infect Dis

March 2023

Université de Paris Cité, APHP, Service des Maladies Infectieuses et Tropicales, Hôpital Necker-Enfants Malades, Centre d'Infectiologie Necker-Pasteur, Institut Imagine, Paris, France.

We investigated single nucleotide polymorphisms (SNPs) possibly involved in immune reconstitution inflammatory syndrome of chronic disseminated candidiasis (IRIS-CDC) through a candidate gene approach and a prospective matched-control study. We found that an SNP located in interleukin-1B at rs1143627 was significantly associated with the risk of developing IRIS-CDC.

Background: Many studies have reported weight gain in ART-naive people living with HIV (PWH) initiating an integrase strand-transfer inhibitor-based regimen. We studied the impact of early or advanced presentation and that of individual drugs in PWH initiating combined ART (cART) between 2012 and 2018.

Methods: From the French Hospital Database HIV cohort, we assessed factors associated with a weight gain  ≥10%, weight change after cART initiation or BMI increase  ≥5 kg/m2 up to 30 months.

Depressive symptoms after hepatitis C cure and socio-behavioral correlates in aging people living with HIV (ANRS CO13 HEPAVIH).

JHEP Rep

January 2023

Aix Marseille Univ, Inserm, IRD, SESSTIM, Sciences Economiques & Sociales de la Santé & Traitement de l'Information Médicale, ISSPAM, Marseille, France.

Background & Aims: A growing literature shows an improvement of chronic hepatitis C virus (HCV)-related depression after successful treatment with direct-acting antivirals. However, depression after HCV cure remains insufficiently documented in people living with HIV (PLWH) and HCV, a population with specific mental health challenges. This study aimed to (i) document the prevalence of moderate-to-severe depression (PHQ-9 score ≥10) across different age classes in HCV-cured PLWH; (ii) identify associated socio-behavioral correlates.
