Objective: Blunt traumatic aortic injury (BTAI) in severe trauma patients is rare but potentially lethal. The aim of this work was to perform a current epidemiological analysis of the clinical and surgical management of these patients in a European country.

Methods: This was a multicentre, retrospective study using prospectively collected data from the French National Trauma Registry and the National Uniform Hospital Discharge Database from 10 trauma centres in France.

Department of Pediatric and Adult Congenital Heart Diseases, Marie Lannelongue Hospital, Groupe Hospitalier Paris Saint-Joseph, centre de reference cardiopathies congénitales complexes M3C, université Paris-Sud, Le Plessis-Robinson, France.

Unlabelled: The long-term prospective multi-centre nationwide (French) observational study FRANCISCO will provide new information on perimembranous ventricular septal defect with left ventricular overload but no pulmonary hypertension in children older than 1 year. Outcomes will be compared according to treatment strategy (watchful waiting, surgical closure, or percutaneous closure) and anatomic features of the defect. The results are expected to provide additional guidance about the optimal treatment of this specific population, which is unclear at present.

In this article, we studied geographic variation in the use of personalized genetic testing for advanced non-small cell lung cancer (NSCLC) and we evaluated the relationship between genetic testing rates and local socioeconomic and ecological variables. We used data on all advanced NSCLC patients who had a genetic test between April 2012 and April 2013 in France in the frame of the IFCT Biomarqueurs-France study (n = 15814). We computed four established measures of geographic variation of the sex-adjusted rates of genetic testing utilization at the "départment" (the French territory is divided into 94 administrative units called 'départements') level.

Background: Postoperative nausea and vomiting (PONV) occurs commonly in surgical patients despite widespread prophylactic antiemetic use. Rescue options are currently limited. 5HT3 antagonists are most frequently used for prophylaxis, but if they fail, additional doses are not effective as rescue medication.

rearrangement and / mutations constitute the primary biomarkers tested to provide targeted or nontargeted therapies in advanced nonsmall cell lung cancer (NSCLC) patients. Our objective was to assess the cost-effectiveness of biomarker testing for NSCLC.Between 2013 and 2014, 843 treatment-naive patients were prospectively recruited at 19 French hospitals into a longitudinal observational cohort study.

Article Synopsis
  • This study examines the effects of aspirin on patients who have had prior percutaneous coronary intervention (PCI) and are undergoing noncardiac surgery, as there is uncertainty regarding aspirin's role in this context.
  • A subgroup analysis of a multicenter trial involving nearly 5000 patients found that aspirin significantly reduced the risk of death or nonfatal heart attacks in those with prior PCI, with an absolute risk reduction of 5.5%.
  • However, the impact of aspirin on major bleeding events in this group was unclear, suggesting that while aspirin may provide cardiovascular protection, it could also carry bleeding risks that need further investigation.

STARD3 mediates endoplasmic reticulum-to-endosome cholesterol transport at membrane contact sites.

StAR-related lipid transfer domain-3 (STARD3) is a sterol-binding protein that creates endoplasmic reticulum (ER)-endosome contact sites. How this protein, at the crossroad between sterol uptake and synthesis pathways, impacts the intracellular distribution of this lipid was ill-defined. Here, by using cholesterol labeling and quantification, we demonstrated that STARD3 induces cholesterol accumulation in endosomes at the expense of the plasma membrane.

Background: The molecular profiling of patients with advanced non-small-cell lung cancer (NSCLC) for known oncogenic drivers is recommended during routine care. Nationally, however, the feasibility and effects on outcomes of this policy are unknown. We aimed to assess the characteristics, molecular profiles, and clinical outcomes of patients who were screened during a 1-year period by a nationwide programme funded by the French National Cancer Institute.

Spasticity (muscle rigidity and spasms) is a frequent and disabling feature of multiple sclerosis (MS), and it can have a marked negative impact on the patient's overall wellbeing and quality of life through a range of symptoms including impaired mobility, bladder dysfunction, stiffness, spasms and sleep disorders. Numerous antispastic agents such as baclofen and tizanidine, as well as others, are available for the management of MS spasticity but, overall, they offer limited clinical benefit. The current questionnaire survey assessed the epidemiology and management of MS spasticity globally and across the EU, among 157 healthcare professionals (>95% of all respondents were neurologists) attending a large, international MS congress (European Committee for Treatment and Research in Multiple Sclerosis, Lyon, France, 10-13 October 2012).

[Tetralogy of Fallot in monozygotic twins].

April 2001

Service de pédiatrie 2, centre hospitalier universitaire de Hautepierre, avenue Molière, 67098 Strasbourg, France.

Unlabelled: The causative mechanisms of congenital heart defects remain unclear and little is known about the respective implication of chance, genetics and environment, though recent findings in molecular biology may provide further insight into understanding the pathophysiologic basis of congenital heart diseases.

Case Report: We report the exceptional but significant case of monozygotic twins both affected by tetralogy of Fallot, for whom prenatal diagnosis ruled out 22q11 microdeletion.

Conclusion: We discuss how far this observation is consistent with the latest hypothesis, which emphasizes the leading role of genetic factors.

A prospective study has been conducted in 35 cases of pharyngolaryngeal's carcinoma, between december 1990 and june 1992 in the department of Head and Neck Surgery of Strasbourg. The authors compared, in a statistic analysis, Computed Tomography and histopathologic's correlations. The operative specimen were sectioned in the horizontal plane to correspond with the CT sections.

About one case of septic sigmoid sinus thrombosis, the authors present various clinical and paraclinical elements of this rare pathology, whose entity are generally subtle for benign but also for serious forms. Although uncomplicated dural venous thrombosis carries an excellent prognosis with proper treatment, delay in diagnosis and institution of appropriate therapy may lead to serious, or even fatal, consequences. The modern literature is reviewed and the pathology, presentation and management of lateral sinus pathology is discussed.

A retrospective study have been conducted in 117 cases of sudden sensori-neural hearing loss, between 1988 and 1991, int he department of Head and Neck Surgery (Strasbourg). After a brief review of actually treatments, they show, in a statistic and comparative analysis, that efficacy of therapeutics are never significant. This results are compared with international bibliography, and show that treatments of sudden loss remains doubtful.

Neuroendocrine tumors are rare neoplasms of the larynx and hypopharynx. This tumors are divided in paragangliomas (usually benign, although malignant case have been reported), large cell neuroendocrine carcinoma, described in this case as a primary pharyngo-laryngeal locate, and small cell neuroendocrine carcinoma (characterized by early and diffuse metastatic disease, and best treated by radiotherapy and chemotherapy). Through the international literature, the authors reviewed the characteristics of the large cell neuroendocrine tumors and their management.

Results of an inquiry in 40 French hospitals (28 universities and 12 private clinics). With 2 exceptions, all questioned surgeons use stapler instruments in oesophagus, stomach and colon rectal surgery. 50% of the surgeons noticed the same percentage of fistulae by stapler as by hand suturing and the severity of these leakages was more or less identical in both series.
