Background: Diabetic foot ulcers (DFUs) are a real public health problem which carry a high risk of amputation. The treatment of DFUs is based on general management such as the treatment of infection, arterial disease, and offloading, but recent studies have shown that the quality of the local covering can impact the healing rate.

Case Summary: We report the case of a 39-year-old man, living with diabetes since the age of 15, who developed DFU on the dorsum of his left foot, with muscle and tendon involvement.

Ethanol Chemical Gallbladder Ablation for Cholecystitis in Inoperable Elderly Patients.

Cardiovasc Intervent Radiol

January 2025

Service de Radiologie Interventionnelle, Centre Hospitalier Sud Francilien, 40 Avenue Serge Dassault, 91100, Corbeil-Essonnes, France.

Purpose: Cholecystitis presents significant treatment challenges, especially in elderly patients with high surgical or anesthetic risks. While cholecystectomy remains the standard intervention, its feasibility is sometimes limited, leading to reliance on cholecystostomy, which has a high recurrence rate and does not address the underlying cause. The aim was to evaluate the efficacy and safety of chemical gallbladder ablation as a minimally invasive treatment option for acute cholecystitis in elderly and comorbid patients.

Genetic Landscape of a Cohort of 120 Patients with Diminished Ovarian Reserve: Correlation with Infertility.

Int J Mol Sci

November 2024

Unité de Génétique Moléculaire des Maladies Métaboliques et de la Reproduction, Hôpital Bicêtre, Faculté de Médecine Paris Saclay, INSERM U1193, 94275 Le Kremlin-Bicêtre, France.

Article Synopsis
  • Diminished ovarian reserve (DOR) and primary ovarian insufficiency (POI) significantly contribute to female infertility, with a recent study finding a genetic cause in 29.3% of POI cases.
  • A prospective study of 120 patients with unexplained DOR revealed a 24.2% diagnostic yield using advanced sequencing technology to identify genes related to various biological pathways.
  • Findings indicated that impaired oocyte quality is linked to meiosis/DNA repair genes, and the study supports the need for genetic testing as a routine part of infertility diagnosis and a potential predictor for progression to POI.

Percutaneous Anterograde Varicocele Embolisation: Technique and Clinical Outcomes.

J Vasc Interv Radiol

November 2024

Service de radiologie interventionnelle, American Hospital of Paris, 55 Boulevard du Château, 92200, Neuilly-sur-Seine, France.

Transcatheter retrograde testicular vein embolisation for symptomatic varicoceles is well-established but has a documented failure rate. Percutaneous anterograde varicocele embolisation may be a suitable alternative. A retrospective observational, descriptive study of consecutive patients who underwent percutaneous anterograde varicocele embolisation at a single centre was performed.

This article evaluates the criteria for diatom testing in forensic investigations, focusing on drowning cases. Diatoms, unicellular algae found in aquatic environments, are critical to the determination of drowning because water containing diatoms is inhaled during submersion. The primary objectives include defining the exact amount and type of tissue to be analyzed, expressed in terms of diatom concentration relative to tissue weight, and detailing the conditions under which water samples are collected to study the diatom flora at the site.

Background: Tirofiban has been shown to be superior to aspirin in achieving functional independence at 3 months for acute ischemic stroke of atheromatous or microatheromatous origin. As intravenous thrombolysis (IVT) has previously been shown to be nonsuperior to aspirin in achieving functional independence at 3 months for anterior choroidal or paramedian pontine infarction (ACI/PPI), we aimed to compare the outcomes of Caucasian patients receiving IVT plus tirofiban (IVT + T) with those receiving tirofiban alone for acute ACI/PPI.

Methods: A retrospective study was conducted in patients aged ≥ 18 years with ACI/PPI treated in our stroke unit between December 1, 2020, and April 30, 2023, who received therapeutic intervention within 9 hours of symptom onset or after awakening with stroke symptoms.

Article Synopsis
  • - Sickle cell disease (SCD) is caused by mutations in the hemoglobin beta chain gene, leading to the production of abnormal hemoglobin (HbS), which results in symptoms like anemia and painful vaso-occlusive events.
  • - Gene therapy shows promise for treating SCD, with two specific therapies already on the market, but there is a need to enhance their effectiveness, particularly with strategies that involve transferring a corrective beta-globin gene.
  • - Research demonstrates that using cyclosporin H (CsH) during the gene therapy process can improve transgene expression and the number of gene copies in stem cells from cord blood, making it a potential effective combination for treating SCD.

  • Mutations in a protein called calreticulin (mutCALR) cause blood disorders known as myeloproliferative neoplasms (MPNs), but there aren’t any treatments specifically targeting this mutation.
  • A new antibody called INCA033989 can specifically attack cells with the mutCALR mutation, stopping them from growing and causing problems.
  • In tests on mice, INCA033989 not only prevented issues like high blood cell counts but also showed it could help treat the disease without harming normal blood cell production.

Arterial embolization for massive bleeding from a locally advanced breast tumor.

Radiol Case Rep

November 2024

Department of Radiology, Hôpital Ambroise Paré (AP-HP), 9 Av. Charles de Gaulle, Boulogne-Billancourt 92100, France.

Advances in breast cancer treatment have markedly reduced the incidence of massive bleeding, yet severe hemorrhage remains a critical issue in locally advanced or metastatic cases. Traditional management strategies often prove inadequate for significant bleeding, highlighting the need for alternative interventions. We detail the management of a 64-year-old patient with a neglected locally advanced breast tumor, leading to life-threatening hemorrhage.

Functional impairment is of major importance in the evaluation of assault victims. French law assesses the seriousness of the violence in terms of the functional impairment experienced by the victims, quantified by the duration of the victims' inability to fulfill their usual daily activities, measured in days of "total incapacity to work" (TIW). Significant variations in the duration of TIW have been reported depending on the examination centers or physician.
