Article Synopsis
  • The ECOST-CRT study assessed whether integrating a comprehensive remote monitoring (RM) scheme into heart failure (HF) care could improve patient outcomes compared to standard RM methods.
  • 652 patients with cardiac resynchronization therapy (CRT) devices were split into two groups: one receiving only standard monitoring and the other receiving additional symptom tracking and notifications.
  • Despite finding better quality of life improvements in the comprehensive RM group, the study did not show a significant difference in all-cause mortality or hospitalizations for worsening heart failure, partly due to early termination caused by the COVID-19 pandemic.

Article Synopsis
  • Management of first episodes of primary spontaneous pneumothorax (air in the chest cavity) is debated, with this study comparing simple aspiration and chest tube drainage as treatment methods.
  • The trial involved 402 adults aged 18-50 and measured lung expansion 24 hours post-treatment, finding a higher failure rate in the aspiration group (29%) compared to the chest tube group (18%).
  • Despite higher failure rates, the aspiration method resulted in less overall pain and fewer complications, with recurrence rates of pneumothorax being 20% for aspiration and 27% for drainage.

CRT-D replacement strategy: results of the BioCONTINUE study.

J Interv Card Electrophysiol

August 2023

Clinique Ambroise Paré, Neuilly Sur Seine, France.

Background: In patients with cardiac resynchronization therapy defibrillators (CRT-Ds), the need for implantable cardioverter-defibrillator (ICD) back-up may be questionable at time of CRT-D replacement (REP) if ICD implant criteria are no longer met due to an improved left ventricular ejection fraction (LVEF) and if no major ventricular arrhythmic event (VAE) occurred during the CRT-D lifetime. The aim of our study was to assess the relevance of ICD back-up and predictors of VAE after REP in primary prevention CRT-D patients.

Methods: The prospective, observational, international BioCONTINUE study investigated the rate of patients with at least 1 sustained VAE (sVAE) post-REP and searched for predictive factors of sVAE.

Introduction: People who inject drugs represent an under-treated chronic hepatitis C virus (HCV)-infected patient population.

Methods: INTEGRATE was a prospective, observational study investigating the effectiveness, safety, and adherence in routine clinical practice to telaprevir in combination with peg-interferon and ribavirin (Peg-IFN/RBV) in patients with history of injecting drug use chronically infected with genotype 1 HCV.

Results: A total of 46 patients were enrolled and included in the intent-to-treat (ITT) population.

Background: There is insufficient research into the use of mechanical ventilation with increased inspiratory oxygen concentration (FiO) and fluid resuscitation with hypertonic saline solution in patients with septic shock. We tested whether these interventions are associated with reduced mortality.

Methods: This two-by-two factorial, multicentre, randomised, clinical trial (HYPERS2S) recruited patients aged 18 years and older with septic shock who were on mechanical ventilation from 22 centres in France.

Clinical balance tests, proprioceptive system and adolescent idiopathic scoliosis.

Eur Spine J

June 2017

Soins de Suite et de Réadaptation Pédiatrique Marc Sautelet de Villeneuve d'Ascq, 59650, Villeneuve d'Ascq, France.

Purpose: Adolescent idiopathic scoliosis (AIS) is a three-dimensional deformity of the spinal column of unknown etiology. Multiple factors could be involved, including neurosensory pathways and, potentially, an elective disorder of dynamic proprioception. The purpose of this study was to determine whether routine balance tests could be used to demonstrate an elective alteration of dynamic proprioception in AIS.

Objectives: We aimed to update the epidemiology of bacteremia and evaluate their management and short-term outcome.

Methods: We conducted a prospective multicenter survey from October to November 2011. Consecutive patients with at least one positive blood culture (BC) were included in the study.

Purpose: Adolescent idiopathic scoliosis (AIS) is a three-dimensional deformity of the spine associated with disturbed postural control. Cervical proprioception participates in controlling orthostatic posture via its influence on head stabilization. We hypothesized that patients with AIS exhibit altered cervical proprioception.

Unlabelled: Botulinum toxin is a proven and widely used treatment for numerous conditions characterized by excessive muscular contractions. Recent studies have assessed the analgesic effect of botulinum toxin in joint pain and started to unravel its mechanisms.

Literature-search-methodology: We searched the international literature via the Medline database using the term "intraarticular botulinum toxin injection" combined with any of the following terms: "knee", "ankle", "shoulder", "osteoarthritis", "adhesive capsulitis of the shoulder".

Purpose: Adolescent idiopathic scoliosis (AIS) is a three-dimensional deformity of the spine, with unknown origin. Some studies have noted impaired postural balance in AIS, in particular, difficulty to manage situations with sensory conflict. The motion sickness susceptibility can be secondary to a sensory conflict, for example, between visual and vestibular information.

Background: Much attention is being paid to the education of and provision of medical information to patients, to optimize their understanding and acceptance of their disease.

Aims: To ascertain the impact of educating recent recipients of an implantable cardioverter defibrillator (ICD) on their perception and acceptance of a home monitoring (HM) system.

Methods: Questionnaire 1, completed one month after ICD implantation, was designed to assess: the quality of patient preparation for HM; patient comprehension of HM; and patient anxiety experienced during its installation.

In order to evaluate the incidence and risk factors of infection by hepatitis C virus (HCV) among injecting drug users (IDUs), we conducted a prospective cohort study of HCV- and human immunodeficiency virus (HIV)-negative IDUs in the North and East of France. A total of 231 HCV and HIV IDUs who had injected drugs at least once in their lifetime were followed up every 3 months over a 12-month period. Serum anti-HCV and anti-HIV were tested at inclusion in the study and at the end of the follow-up.

The dynamic pattern of artery obstruction is detailed as a key element to the diagnosis of extrinsic coronary compression throughout the angiography. Treatment by direct stenting restored complete anterograde coronary flow and improved myocardial perfusion.
