Article Synopsis
  • Variants of uncertain significance (VUS) present challenges in diagnosing rare diseases, and episignatures have emerged as potential biomarkers to help classify these variants.
  • A study analyzed DNA methylation data from different groups, including carriers of pathogenic variants and healthy controls, using a k-nearest-neighbour classifier to assess the predictive abilities of various episignatures.
  • Results revealed that while some signatures (ATRX, DNMT3A, KMT2D, NSD1) achieved 100% sensitivity, others (CREBBP-RSTS, CHD8) showed lower performance, indicating that not all episignatures are equally reliable for diagnostic use and highlighting the need for further validation with larger sample sizes.

[Doctors' feelings when facing chronic pain patients: Countertransference between symptom and treatment].

Rev Med Suisse

June 2023

Psychologue clinicien. Hôpital Reine Hortense, Centre fédératif d'évaluation et de traitement de la douleur, Centre hospitalier Métropole Savoie, 1 Boulevard Berthollet, 73100 Aix-les-Bains, France.

Doctors, when facing patients suffering from chronic pain, are driven to the limits of their clinical practice, which essentially is based on a diagnostic interpretation of the patient's signs and symptoms, leading to the appropriate therapeutic intervention. A doctor's awareness of his own experience of helpless distress facing these patients is bound to occur and to make him address the issues of transference between him and his patient. Listening to the patient's narrative is essential.

A new method for estimating the first ventilatory threshold in patients with chronic respiratory diseases: A feasibility study.

Respir Med Res

June 2023

Service Hospitalier Universitaire Pneumologie Physiologie, Pôle Thorax et Vaisseaux, Centre Hospitalier Universitaire Grenoble Alpes, Grenoble, France; Laboratoire HP2, INSERM U1300, Université Grenoble Alpes, Grenoble, France.

Background: The identification of the first ventilatory threshold (VT1) on an incremental cardiopulmonary exercise test (CPET) is useful to guide exercise reconditioning. However, determination of the VT1 is sometimes difficult in patients with chronic respiratory disease. Our hypothesis was that it would be possible to identify a "clinical threshold" based on patients' perceptions at which they subjectively consider that they can perform endurance training during a rehabilitation programme.

J Midwifery Womens Health

November 2022

Univ Rennes, CHU Rennes, Inserm, EHESP, Irset (Institut de recherche en santé, environnement et travail) - UMR_S 1085, Rennes, F-35000, France.

These clinical practice guidelines from the French National College of Midwives (CNSF) are intended to define the messages and the preventive interventions to be provided to women and co-parents by the different professionals providing care to women or their children during the perinatal period. These guidelines are divided into 10 sections, corresponding to 4 themes: 1/ the adaptation of maternal behaviors (physical activity, psychoactive agents); 2/ dietary behaviors; 3/ household exposure to toxic substances (household uses, cosmetics); 4/ promotion of child health (breastfeeding, attachment and bonding, screen use, sudden unexplained infant death, and shaken baby syndrome). We suggest a ranking to prioritize the different preventive messages for each period, to take into account professionals' time constraints.
