Purpose: Acute mesenteric ischemia (AMI) is a rare, but life-threatening condition occurring among critically ill patients. Several factors have been associated with AMI, but the causal link is debated, most studies being retrospective. Among these factors, enteral nutrition (EN) could be associated with AMI, in particular among patients with shock.

Background: Patients with solid cancers and hematopoietic malignancy can experience systemic symptoms compatible with adult-onset Still's disease (AOSD). The newly described VEXAS, associated with somatic UBA1 mutations, exhibits an overlap of clinical and/or biological pictures with auto inflammatory signs and myelodysplastic syndrome (MDS).

Objectives: To describe a cohort of patients with signs of undifferentiated systemic autoinflammatory disorder (USAID) concordant with AOSD and MDS/chronic myelomonocytic leukemia (CMML) and the prevalence of VEXAS proposed management and outcome.

Liver Iron Load Influences Hepatic Fat Fraction in End-Stage Renal Disease Patients on Dialysis: A Proof of Concept Study.

EBioMedicine

January 2019

Division of Radiology, Ramsay Générale de Santé, Hôpital Privé Claude Galien, Quincy-Sous-Sénart, France.

Background: Nonalcoholic fatty liver disease (NAFLD) is a spectrum of diseases including steatosis, nonalcoholic steatohepatitis (NASH), cirrhosis, and end-stage liver failure. Hepatic iron accumulation has been linked to hepatic fibrosis severity in NASH and NAFLD. Iron overload induced by parenteral (IV) iron therapy is a potential clinical problem in dialysis patients.

Article Synopsis
  • - A strict anaerobic microorganism, which is normally found in the human mouth and gut, can cause serious diseases and is especially known for causing abscesses in the body.
  • - A case was reported of a healthy 64-year-old woman who developed multiple abscesses in her liver, pleura, and brain, with a diagnosis confirmed through genetic testing rather than traditional cultures.
  • - It's important to consider this microorganism in patients with organ abscesses, particularly if there are multiple sites of infection, and molecular techniques can help diagnose cases that are culture-negative.

Septic shock with no diagnosis at 24 hours: a pragmatic multicenter prospective cohort study.

Crit Care

November 2016

Service de réanimation Médicale, Groupe de Recherche CARMAS, Centre Hospitalier Universitaire Henri Mondor, Assistance Publique-Hôpitaux de Paris, 51, avenue du Maréchal de Lattre de Tassigny, Créteil, 94010, France.

Background: The lack of a patent source of infection after 24 hours of management of shock considered septic is a common and disturbing scenario. We aimed to determine the prevalence and the causes of shock with no diagnosis 24 hours after its onset, and to compare the outcomes of patients with early-confirmed septic shock to those of others.

Methods: We conducted a pragmatic, prospective, multicenter observational cohort study in ten intensive care units (ICU) in France.

Encephale

September 2014

Unité les Tilleuls, centre hospitalier Valvert, boulevard des Libérateurs, 13391 Marseille cedex 11, France.

Among the hepatic forms, three of them are clinically characterised by acute abdominal pain and neuropathy, sometimes associated with psychiatric disorders.

Corynebacterium diphtheriae septicaemia is rarely encountered, usually in very particular situations: children with severe congenital heart disease or after heart surgery. Rare cases have been reported in immunodepressed adults or drug addicts. We observed a case in a formerly healthy 41-year-old woman who was hospitalized for fever unresponsive to bacampicillin.

The authors describes a case of posterior dislocation of the elbow in the extreme stage, associating total destruction of the capsule and ligaments, a total arterial lesion, elongation of the median nerve and the opening of the focus at Tscherne's stage III. This case allows considering the biomechanical aspects of dislocation of the elbow and updating the principles of its management.
