Primary intraosseous squamous cell carcinoma (PIOSCC) is a rare malignant tumor originating from remnants of odontogenic epithelium within the central jaw bone. The majority of the PIOSCC cases are diagnosed incidentally on histological findings while a potential malignancy wasn't necessarily determined at first on initial investigations. The diagnosis of PIOSCC is challenging and relies on ruling out other types of carcinomas, including metastatic tumors from other primary sites.

Background: The convergence of the economic crisis, COVID-19 pandemic, and Beirut Blast has precipitated unprecedented challenges for the healthcare system in Lebanon, particularly for cancer patients. Amidst these crises, our study evaluates its contribution to a concerning trend of operating on more late-stage and complex colorectal cancer (CRC) cases.

Methods: We included 155 patients operated for CRC between 2017 and 2023.

Lyme borreliosis and medical wandering: what do patients think about multidisciplinary management? A qualitative study in the context of scientific and social controversy.

BMC Infect Dis

March 2024

Department of Infectious Diseases, Tick-Borne Diseases Reference Center of Paris and the Northern Region, Centre Hospitalier Intercommunal de Villeneuve-Saint-Georges, 40 Allée de La Source, 94190, Villeneuve-Saint-Georges, France.

Introduction: To answer to patients' medical wandering, often due to "unexplained symptoms" of "unexplained diseases" and to misinformation, multidisciplinary care centers for suspected Lyme borreliosis (LB), such as the 5 Tick-Borne Diseases (TBDs) Reference Centers (TBD-RC), were created a few years ago in France, the Netherlands and Denmark. Our study consisted of a comprehensive analysis of the satisfaction of the patients managed at a TBD-RC for suspected LB in the context of scientific and social controversy.

Methods: We included all adults who were admitted to one of the TBD-RC from 2017 to 2020.

Background: Laparoscopy is widely adopted across nearly all surgical subspecialties in the elective setting. Initially finding indication in minor abdominal emergencies, it has gradually become the standard approach in the majority of elective general surgery procedures. Despite many technological advances and increasing acceptance, the laparoscopic approach remains underutilized in emergency general surgery and in abdominal trauma.

Until now, the early approach of palliative care patients for corneal harvesting has been unheard of in France. At the Centre Hospitalier Intercommunal de Villeneuve-Saint-Georges Lucie-et-Raymond-Aubrac, in the Val-de-Marne region of France, we offer a rigorous and respectful procedure and organization for patients who have been carefully selected for an early approach to donation.

Laparotomy incisions provide easy and rapid access to the peritoneal cavity in case of emergency surgery. Incisional hernia (IH) is a late manifestation of the failure of abdominal wall closure and represents frequent complication of any abdominal incision: IHs can cause pain and discomfort to the patients but also clinical serious sequelae like bowel obstruction, incarceration, strangulation, and necessity of reoperation. Previous guidelines and indications in the literature consider elective settings and evidence about laparotomy closure in emergency settings is lacking.

Source control in emergency general surgery: WSES, GAIS, SIS-E, SIS-A guidelines.

General, Acute Care, Abdominal Wall Reconstruction, and Trauma Surgery, Foothills Medical Centre, Calgary, AB, Canada.

Intra-abdominal infections (IAI) are among the most common global healthcare challenges and they are usually precipitated by disruption to the gastrointestinal (GI) tract. Their successful management typically requires intensive resource utilization, and despite the best therapies, morbidity and mortality remain high. One of the main issues required to appropriately treat IAI that differs from the other etiologies of sepsis is the frequent requirement to provide physical source control.

Many countries are facing an aging population. As people live longer, surgeons face the prospect of operating on increasingly older patients. Traditional teaching is that with older age, these patients face an increased risk of mortality and morbidity, even to a level deemed too prohibitive for surgery.

Article Synopsis
  • - The MELODY system enables remote ultrasonography and was tested for effectiveness in children aged 1 to 10 during the COVID-19 pandemic, with a focus on assessing its feasibility.
  • - A study involving 38 children showed substantial agreement between telerobotic and traditional ultrasound results, although telerobotic sessions took longer; both methods effectively visualized abdominal and lung conditions.
  • - Parents of the children reported that 45% felt their kids experienced less stress during the telerobotic ultrasound, indicating it may be a more comfortable option for pediatric patients.

The first wave of the COVID-19 pandemic in France severely tested the healthcare system and its teams from the first weeks of 2020. With two years of hindsight, healthcare professionals have learned a great deal from this health crisis which, despite vaccination, maintains pressure on social activities and on the daily lives of caregivers. Looking back to the early days of this crisis, a nurse and her former trainer report on the spontaneous collaborations deployed to reinforce the resuscitation teams.

Factors Associated With Severe SARS-CoV-2 Infection.

Pediatrics

March 2021

French Pediatric Infectious Disease Group, Paris, France;

Background: Initial reports on severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) infections in children suggested that very young age and comorbidities may increase risk of severe evolution, but these findings remained to be confirmed. We aimed to analyze the clinical spectrum of hospitalized pediatric SARS-CoV-2 infection and predictors of severe disease evolution.

Methods: We conducted a French national prospective surveillance of children hospitalized with SARS-CoV-2 infection.

Purpose: Isolating colonies and obtaining accurate colony counts from bacterial cultures are critical steps for the optimal management of infected patients. The uncertainties in the colony count results from the bacterial cultures were evaluated by verifying the performance of the WASP inoculation system according to the International Organization for Standardization (ISO) 15189 standard.

Methodology: We first (i) evaluated the cross-contamination and precision of the WASP instrument (Copan Diagnostics, Italy) and (ii) established enumeration reading grids for urine, swab, bronchopulmonary specimens (BPSs) and catheter tip cultures.

GREFON recommendations for assessment of instrumental activities of daily living in memory clinics.

Rev Neurol (Paris)

April 2017

Centre de gériatrie, groupe hospitalier Pitié-Salpêtrière-Charles-Foix, Pierre et Marie University, Sorbonne-Paris-Cité Universities, UMR 8256 CNRS B2A, AP-HP, DHU FAST, 47-83, boulevard de l'Hôpital, 75013 Paris, France.

Following a review of the available assessment scales and current practices of evaluation of instrumental activities of daily living (IADL) in French memory centres by GREFON (Groupe de réflexion sur l'évaluation fonctionnelle; Working Group on Functional Assessment), the main aim of this position paper was to provide good clinical practice (GCP) guidelines for the assessment of IADL. Another aim was to highlight the need for innovative tools adapted to the present and future evolution of such activities in real life, including the use of new technologies, the need for earlier detection of IADL impairment during the diagnostic process of mild neurocognitive disorders, and greater sensitivity to IADL changes during follow-up to allow adaptation of clinical management and evaluation of the impact of therapeutic interventions.
