Lower risk (LR) myelodysplastic syndromes (MDS) are heterogeneous hematopoietic stem and progenitor disorders caused by the accumulation of somatic mutations in various genes including epigenetic regulators that may produce convergent DNA methylation patterns driving specific gene expression profiles. The integration of genomic, epigenomic, and transcriptomic profiling has the potential to spotlight distinct LR-MDS categories on the basis of pathophysiological mechanisms. We performed a comprehensive study of somatic mutations and DNA methylation in a large and clinically well-annotated cohort of treatment-naive patients with LR-MDS at diagnosis from the EUMDS registry (ClinicalTrials.

Article Synopsis
  • The announcement of Parkinson's disease (PD) diagnosis often leads to negative emotions for patients, impacting their coping strategies and overall adjustment to the condition.
  • A national French survey involving 397 recent PwPD, along with their caregivers and healthcare professionals, found that 60% of patients were not expecting their diagnosis and 82% experienced negative feelings, particularly influenced by male gender and older age.
  • There is a significant need for improved communication and support during the diagnosis process, as many PwPD and caregivers felt they lacked sufficient information and expressed a desire for multidisciplinary follow-up.

Service d'oncologie-radiothérapie, Institut du cancer de Montpellier (ICM), université de Montpellier, Institut de recherche sur le cancer de Montpellier (IRCM), unit 1194, Inserm, Montpellier, France.

Radiation-induced acute and late toxicity depends on several parameters. The type, severity and duration of morbidity are mainly related to irradiated volume, total dose and its fractionation and the intrinsic radiosensitivity of the patients. The follow-up of these toxicities is essential.

Article Synopsis
  • Patients with initially unresectable advanced non-small cell lung cancer (NSCLC) may achieve positive treatment outcomes after receiving immune checkpoint inhibitors (ICIs) and undergoing later surgery, although this is rare.
  • A study of 21 patients over four years found that 43% had early post-operative complications, with one patient dying from complications.
  • Despite these risks, the study showed a 100% rate of complete tumor resection and a 43% major pathologic response, with only diffusing capacity for carbon monoxide linked to complications.

Background: Increasing evidence links obstructive sleep apnea (OSA) to cognitive decline. Autonomic dysfunction assessed by heart rate variability is a promising early biomarker of cognitive impairment in populations without major neurocognitive disorder (MND). We aimed to determine whether nocturnal pulse rate variability (PRV) extracted from oximetry signal and OSA severity could predict MND onset among older OSA patients.

Background: Several studies report an increased susceptibility to SARS-CoV-2 infection in cancer patients. However, data in the intensive care unit (ICU) are scarce.

Research Question: We aimed to investigate the association between active cancer and mortality among patients requiring organ support in the ICU.

Backgrounds: Malignant pleural mesothelioma (MPM) is a cancer with poor prognosis. Second-line and onward therapy has many options, including immune-checkpoint inhibitors with demonstrated efficacy: 10−25% objective response rate (ORR) and 40−70% disease-control rate (DCR) in clinical trials on selected patients. This study evaluated real-life 2L+ nivolumab efficacy in MPM patients and looked for factors predictive of response.

Background: Benefit from maintenance in advanced non-squamous non-small cell lung cancer (NS-NSCLC) might favor switch maintenance after disease stabilization (SD) and continuation after objective response (OR). This trial assessed a maintenance strategy conditioned by response to cisplatin-gemcitabine (CG) with G continuation for patients with OR or switch to pemetrexed (P) for patients with SD as compared with a control arm based on the Paramount regimen.

Methods: Eligibility criteria: age 18-70 years, ECOG PS 0-1, untreated stage IV NS-NSCLC without EGFR or ALK alteration, ineligibility to bevacizumab.

Fractional Flow Reserve to Guide Treatment of Patients With Multivessel Coronary Artery Disease.

J Am Coll Cardiol

November 2021

Hôpital Cardiologique et Pneumologique Louis Pradel, Hospices Civils de Lyon, Bron, France.

Article Synopsis
  • The FUTURE trial investigated whether using fractional flow reserve (FFR) to guide treatment in multivessel coronary artery disease (CAD) is more effective than traditional approaches.
  • The study was a randomized trial involving 927 patients, comparing outcomes of an FFR-guided strategy with a standard strategy without FFR, focusing on major adverse cardiac events over one year.
  • Results showed no significant differences in major adverse events or mortality between the two groups, indicating that FFR guidance did not provide a clear benefit in managing multivessel CAD.

Objectives: Prone position is used in acute respiratory distress syndrome and in coronavirus disease 2019 acute respiratory distress syndrome. However, it is unclear how responders may be identified and whether an oxygenation response improves outcome. The objective of this study was to quantify the response to prone position, describe the differences between coronavirus disease 2019 acute respiratory distress syndrome and acute respiratory distress syndrome, and explore variables associated with survival.

  • Thrombocytopenia in MDS and CMML:
  • Despite being moderate in prevalence for low-risk myelodysplastic syndromes (MDS) and chronic myelomonocytic leukaemia (CMML), thrombocytopenia poses significant risks for severe bleeding, and current treatment options remain limited.
  • Efficacy of Eltrombopag (ELT):
  • In a retrospective study involving 61 patients (50 with MDS and 11 with CMML), ELT demonstrated a platelet response in 77% of patients, with a median duration of response lasting about 8 months, and none of the patients who stopped treatment relapsed within a follow-up period.

Oral therapy adherence and satisfaction in patients with multiple myeloma.

Ann Hematol

July 2021

Pharmacy Department, Centre Hospitalier Universitaire de Besançon, University Hospital of Besançon, Besançon, France.

The transition to oral therapies in patients with multiple myeloma (MM) offers potential benefits to patients; however, they must self-manage their medication and adherence plays an important role in patient care. It has been shown that patient satisfaction with their medication has a strong positive correlation with adherence in chronic diseases. The aim of this study was to estimate adherence rate of oral antimyeloma therapies and to identify risk factors for medication non-adherence.

Background: In the general population, metabolic syndrome (MetS) is predictive of major adverse cardiovascular events (MACE). Waist circumference (WC), a component of the MetS criteria, is linked to visceral obesity, which in turn is associated with MACE. However, in haemodialysis (HD) patients, the association between MetS, WC and MACE is unclear.

Since management and long-term outcomes remain poorly described, we retrospectively analyzed results of 27 adults from 21 departments of nephrology in France accrued over 19 years. Inclusion criteria were presence of glomerular Congo red-negative monotypic immunoglobulin deposits with ultrastructural microtubular organization, without evidence for cryoglobulinemic glomerulonephritis.
