Article Synopsis
  • * The E846D variant has been found in patients but lacks clear evidence of being pathogenic; it appears in some individuals without correlating to a definitive blood disorder.
  • * Analysis of a large UK Biobank cohort shows that while the E846D variant can slightly increase hemoglobin levels, it does not alone cause absolute polycythemia, indicating additional factors are necessary for the condition.

Oral Cavity Squamous Cell Carcinoma Risk Factors: State of the Art.

J Clin Med

May 2023

Department of Otolaryngology-Head and Neck Surgery, CHU Rouen, 76000 Rouen, France.

Head and neck (HN) squamous cell carcinomas (SCCs) originate from the epithelial cells of the mucosal linings of the upper aerodigestive tract, which includes the oral cavity, the pharynx, the larynx, and the sinonasal cavities. There are many associated risk factors, including alcohol drinking coupled with tobacco use, which accounts for 70% to 80% of HNSCCs. Human papilloma virus (HPV) is another independent risk factor for oropharyngeal SCC, but it is only a minor contributor to oral cavity SCC (OSCC).

Background: Prophylactic central neck dissection in clinically low-risk cT1bT2N0 papillary thyroid carcinoma is controversial, due to a large number of conflicting retrospective studies, some showing an advantage in terms of locoregional recurrence, others showing no advantage. These previous studies all show high rates of excellent response. We aim to demonstrate the non-inferiority of thyroidectomy alone as compared to total thyroidectomy with prophylactic central neck dissection in conjunction with adjuvant RAI 30 mCi with rTSH stimulation in terms of excellent response at 1 year.

Background: Retrospective studies in hematological unit have suggested that single red blood cell (1-RBC) unit transfusion policy may reduce the number of RBC used without negative clinical impact.

Method: Acute leukemia patients requiring intensive chemotherapy or patients receiving autologous or allogeneic transplantation were randomly assigned to receive either single RBC (1-RBC arm) or double RBC (2-RBC arm) per transfusion with a hemoglobin trigger of 8 g/dL. The primary composite endpoint was the percentage of patients experiencing serious complications, such as a non-hematological adverse event grade ≥ 3 or intensive care admission or death.

Disequilibrium between and Circular and Messenger RNAs Plays a Role in Breast Cancer.

Cancers (Basel)

April 2023

Univ Rouen Normandie, INSERM U1245, FHU-G4 Génomique and CHU Rouen, Department of Genetics, 76000 Rouen, France.

Article Synopsis
  • Breast cancer detection and prognosis are difficult due to the need for effective biomarkers, and the role of circRNAs in this process is being explored.
  • Researchers created a new method called SEALigHTS to study the interplay between circular RNAs and mRNAs in breast tissue samples.
  • Their findings indicate that the balance between circRNAs and mRNAs is disrupted in tumor tissues, supporting the idea that this imbalance contributes to the development of breast cancer.

In previously untreated, medically fit patients with chronic lymphocytic leukemia (CLL), research is focused on developing fixed-duration strategies to improve long-term outcomes while sparing patients from serious toxicities. The ICLL-07 trial evaluated a fixed-duration (15-month) immunochemotherapy approach in which after obinutuzumab-ibrutinib induction for 9 months, patients (n = 10) in complete remission (CR) with bone marrow (BM) measurable residual disease (MRD) <0.01% continued only ibrutinib 420 mg/day for 6 additional months (I arm), whereas the majority (n = 115) received up to 4 cycles of fludarabine/cyclophosphamide-obinutuzumab 1000 mg alongside the ibrutinib (I-FCG arm).

Case report: CAR-T cell therapy-induced cardiac tamponade.

Front Cardiovasc Med

March 2023

Medical Intensive Care Unit, Normandie Univ, UNIROUEN, UR 3830, CHU Rouen, Medical Intensive Care Unit, Rouen, France.

Unlabelled: CD19-specific chimeric antigen receptor T (CAR-T) cell therapy has recently been shown to improve the prognosis of refractory diffuse large B-cell lymphoma (DLBCL). However, CAR-T cells may induce numerous adverse events, in particular cytokine release syndrome (CRS) which is frequently associated with cardiovascular manifestations. Among the latter, acute pericardial effusion represents less than 1% of cases and cardiac tamponade has only been reported once.

Impact of central nervous system involvement in adult patients with Philadelphia-negative acute lymphoblastic leukemia: a GRAALL-2005 study.

Haematologica

December 2023

Maladies du Sang, CHU d'Angers, Angers, France; Federation Hospitalo-Universitaire Grand-Ouest Acute Leukemia, FHU-GOAL; Universite d'Angers, Inserm UMR 1307, CNRS UMR 6075, Nantes Universite, CRCI2NA, F-49000 Angers.

Whereas the prognosis of adult patients with Philadelphia-negative acute lymphoblastic leukemia (ALL) has greatly improved since the advent of pediatric-inspired regimens, the impact of initial central nervous system (CNS) involvement has not been formerly re-evaluated. We report here the outcome of patients with initial CNS involvement included in the pediatric-inspired prospective randomized GRAALL-2005 study. Between 2006 and 2014, 784 adult patients (aged 18-59 years) with newly diagnosed Philadelphia-negative ALL were included, of whom 55 (7%) had CNS involvement.
