C-reactive protein thresholds for discriminating active disease in psoriatic arthritis may be different in early versus established disease.

Clin Exp Rheumatol

December 2024

Rheumatology Division, Central University Hospital of Asturias, Oviedo; Department of Medicine, Oviedo University School of Medicine, Oviedo; and Translational Immunology Division, Biohealth Research Institute of the Principality of Asturias (ISPA), Oviedo, Spain.

Objectives: Inflammatory biomarkers such as C-reactive protein (CRP) lack discriminatory capacity to detect active disease in psoriatic arthritis (PsA). Our aim was to find CRP thresholds capable of discriminating active disease in both early and established PsA.

Methods: We included a total of 345 PsA patients (215 early-onset not exposed to high-impact therapies and 130 with established disease under biologics and oral targeted therapies).

Protective Effect of High Adherence to Mediterranean Diet on the Risk of Incident Type-2 Diabetes in Subjects with MAFLD: The Di@bet.es Study.

Nutrients

November 2024

Centro de Investigaciónn Biomedica en Red de Diabetes y Enfermedades Metabolicas Asociadas (CIBERDEM), Instituto de Salud Carlos III, 28029 Madrid, Spain.

: Metabolic Dysfunction-Associated Fatty Liver Disease (MAFLD) increases the risk of Type-2 Diabetes (T2DM). The Mediterranean diet (MD) has shown advantages in the management of MAFLD and preventing co-morbidities; however, its relationship with T2DM development in MAFLD has been less investigated. We aimed to evaluate the association of MD adherence with the risk of incident T2DM in the Spanish adult population with MAFLD and according to their weight gain at 7.

Article Synopsis
  • A study investigated how fiber and probiotic supplements can help protect against colon damage and gut microbiota changes caused by harmful substances (PhIP and DSS) in male Fischer-344 rats.* -
  • The results showed that while PhIP and DSS caused significant inflammation and damage to the colon, fiber reduced this damage and inflammation, and probiotics increased colon length.* -
  • Additionally, fiber shifted the gut microbiota toward less inflammation, contrasting with the negative changes seen after exposure to PhIP and DSS without supplements.*

Background/objectives: In recent years, a possible connection between HLA-Cw6 and a distinctive cardiometabolic (CM) profile in patients with psoriatic disease (PsD) has been proposed, although there is still little support for this. Our aim was to further investigate this possible association by studying a large population of PsD patients.

Methods: For this study, three different cohorts of patients with PsD were analyzed: two with a majority of cutaneous psoriasis, pooled n: 600, and a third with only psoriatic arthritis-PsA-cases, n: 340.

Article Synopsis
  • Myxedema coma often involves reduced consciousness and low body temperature, potentially linked to thyroid issues, especially in patients with head trauma and central hormone deficiencies.
  • A 52-year-old man with head trauma exhibited symptoms like drowsiness and hypotension, with initial tests showing normal TSH levels but low ACTH and FT4, and his condition worsened before improving with medication.
  • The case highlights the importance of assessing both TSH and FT4 levels for diagnosing central hypothyroidism, as relying solely on TSH might lead to overlooked conditions in patients showing hormone deficiency symptoms post-trauma.

The gut microbiota is a key and primary stimulus for the development of a host's immune system. The early establishment of the gut microbiota is affected by several perinatal factors but little is known about their influence on shaping normal immune development and, consequently, on the programming of future health. The analysis of different immune compounds is well-documented in serum samples; however, their presence in faecal samples has not been studied, and this information could be valuable in early life.

The ability of F-FDG positron emission tomography (PET) to track disease activity and treatment response in patients with Ankylosing Spondylitis (AS) or Psoriatic Arthritis (PsA) remains unclear. Here, we assessed whether F-FDG uptake is a marker of disease activity and treatment response in AS or PsA, and explored the ability of F-FDG to predict treatment response. Patients with AS (n = 16) or PsA (n = 8) who were scheduled to initiate treatment with biologics were recruited.

During the last years, there has been an increasing research interest in the analysis of biological fluids requiring non-invasive sampling for biomedical and clinical applications. In this work, we have focused on the nasal exudate with the aim of investigating the potential use of this fluid to know the role of iron in stroke and also for diagnosis. Potential differences in the nasal exudate, collected in swabs, from diagnosed hemorrhagic stroke, ischemic stroke, and control groups were investigated with regard to total iron by inductively coupled plasma-mass spectrometry, iron fractionation studies by size exclusion chromatography together with post-column isotope dilution analysis, and four proteins containing iron (ferritin, transferrin, lactoferrin, and ferroportin) with ELISA kits.

The correct initial colonization and establishment of the gut microbiota during the early stages of life is a key step, with long-lasting consequences throughout the entire lifespan of the individual. This process is affected by several perinatal factors; among them, feeding mode is known to have a critical role. Breastfeeding is the optimal nutrition for neonates; however, it is not always possible, especially in cases of prematurity or early pathology.
