Article Synopsis
  • Loneliness is a significant social issue in Japan, prompting a study to assess how family physicians and nurses perceive patient loneliness in older adults (50+ years) visiting clinics.
  • The study involved 470 patients and revealed that both family physicians and nurses often misjudge or overlook loneliness, with misjudging rates at 20.2% for physicians and 9.6% for nurses, while overlooking rates were 20.9% and 29.8%, respectively.
  • Key factors influencing these misperceptions included marital status and community involvement, with unmarried, divorced, or bereaved individuals and those not involved in community activities being more likely to have their loneliness overlooked or misjudged.

Background: Skills Training in Affective and Interpersonal Regulation (STAIR) Narrative Therapy (SNT) has shown efficacy in alleviating symptoms of posttraumatic stress disorder (PTSD) and improving emotion regulation and interpersonal skills among individuals with complex trauma, such as childhood abuse. Although this therapy is expected to be effective for patients with complex PTSD (CPTSD), no study has directly assessed diagnostic and symptom outcomes. Moreover, the potential of therapy to achieve good outcomes in non-Western countries remains unclear.

Purpose: The role of erectile dysfunction (ED) has recently shown an association with the risk of stroke and coronary heart disease (CHD) via the atherosclerotic pathway. Cardiovascular disease (CVD)/stroke risk has been widely understood with the help of carotid artery disease (CTAD), a surrogate biomarker for CHD. The proposed study emphasizes artificial intelligence-based frameworks such as machine learning (ML) and deep learning (DL) that can accurately predict the severity of CVD/stroke risk using carotid wall arterial imaging in ED patients.

Benefits and drawbacks of statins and non-statin lipid lowering agents in carotid artery disease.

Prog Cardiovasc Dis

September 2022

Department of Clinical Biochemistry, Royal Free Hospital Campus, University College London Medical School, University College London (UCL), London, UK.

Article Synopsis
  • International guidelines recommend using statins, alone or with other medications, to lower LDL cholesterol in patients with asymptomatic or symptomatic carotid stenosis, as it significantly reduces risks of stroke and cardiovascular events.
  • The overview evaluates the effectiveness of various lipid-lowering agents, emphasizing that statins and PCSK9 inhibitors offer substantial benefits like stabilizing carotid plaques and lowering stroke rates, while non-statin options like ezetimibe and fibrates also help but to a lesser extent.
  • Despite potential side effects, the advantages of lowering LDL cholesterol for these patients outweigh the risks, leading to a recommendation for high-dose statin therapy for all individuals, regardless of their baseline LDL levels.

Anxiety, Depression and Quality of Life in Pulmonary Hypertension: A Comparison of Incident and Prevalent Cases.

Respiration

August 2022

Department of Internal Medicine, Respiratory Medicine and Ventilatory Support, Medical Mission Hospital, Central Clinic Würzburg, Academic Teaching Hospital of the Julius Maximilian University of Würzburg, Würzburg, Germany.

Introduction: Anxiety and depression are common in pulmonary hypertension (PH) and health-related quality of life (HRQoL) is reduced. Sufficient analyses in incident and prevalent patients are lacking, so we provide a comparative analysis of these groups with focus on anxiety, depression and HRQoL.

Methods: Depression, anxiety and HRQoL were retrospectively analyzed by Hospital Anxiety and Depression Scale (HADS) and Short Form 36 questionnaire in 91 prevalent and 21 incident PH outpatients from a German tertiary care center specialized in PH.

Purpose: To analyze the mid-term (12 months) effects of carotid endarterectomy (CEA) on cognition and on the microstructural properties of the whole brain white matter in terms of derived diffusion Tensor imaging (DTI) metrics.

Methods: We analyzed a population of 19 asymptomatic patients with extra-cranial internal carotid artery stenosis (eICA) eligible for CEA. All patients underwent cognitive evaluation with the Italian version of the Mini-Mental State Examination corrected for age and schooling, and with a Magnetic Resonance Imaging (MRI) investigation on a 1.

Article Synopsis
  • Parkinson's disease (PD) is linked to an increased risk of cardiovascular disease (CVD) and stroke, but this connection is under-researched, leading to confusion in prognosis and treatment.
  • The study aims to solidify the relationship between PD and CVD/stroke while leveraging artificial intelligence (AI) to accurately stratify risks associated with these conditions in PD patients.
  • It highlights the main cause of cardiovascular issues in PD as cardiac autonomic dysfunction and proposes AI-driven solutions to enhance risk prediction and eliminate biases in studies related to PD and its cardiovascular implications.

Background: The prognostic value of improvement endpoints that have been used in clinical trials of treatments for pulmonary arterial hypertension (PAH) needs to be further investigated.

Methods: Using the COMPERA database, we evaluated the prognostic value of improvements in functional class (FC) and absolute or relative improvements in 6-min walking distance (6MWD) and N-terminal fragment of pro-brain natriuretic peptide (NT-proBNP). In addition, we investigated multicomponent endpoints based on prespecified improvements in FC, 6MWD and NT-proBNP that have been used in recent PAH trials.

Despite the publication of several national/international guidelines, the optimal management of patients with asymptomatic carotid stenosis (AsxCS) remains controversial. This article compares 3 recently released guidelines (the 2020 German-Austrian, the 2021 European Stroke Organization [ESO], and the 2021 Society for Vascular Surgery [SVS] guidelines) vs the 2017 European Society for Vascular Surgery (ESVS) guidelines regarding the optimal management of AsxCS patients. The 2017 ESVS guidelines defined specific imaging/clinical parameters that may identify patient subgroups at high future stroke risk and recommended that carotid endarterectomy (CEA) should or carotid artery stenting (CAS) may be considered for these individuals.

Preclinical atherosclerosis and cardiovascular events: Do we have a consensus about the role of preclinical atherosclerosis in the prediction of cardiovascular events?

Atherosclerosis

May 2022

Department of Vascular Disease, University Medical Center Ljubljana, Zaloska cesta 2, 1000, Ljubljana, Slovenia; Faculty of Medicine, University of Ljubljana, Vrazov trg 2, 1000, Ljubljana, Slovenia. Electronic address:

Article Synopsis
  • * Key indicators like endothelial dysfunction, measured by flow-mediated dilation (FMD), and the presence of asymptomatic plaques in arteries can help assess cardiovascular risk but have varying predictive powers depending on existing health conditions.
  • * Other measures like coronary calcium scoring and ankle-brachial index (ABI) can aid in risk stratification, with ABI highlighting lower limb atherosclerosis, but new blood biomarkers currently have low effectiveness in improving risk assessment.
