The majority of Aspergillus fumigatus reproduction occurs asexually, with large numbers of conidiophores producing small hydrophobic conidia dispersed aerially. When healthy hosts inhale conidia, the mucosal cilia and phagocytosis by the innate immune system can remove them. However, in immunocompromised hosts, the conidia are not removed, which allows them to germinate, forming mycelium that invades host tissues and causes disease.

Objective: To determine the safety, tolerance, and adherence to self-administered intravaginal 5% fluorouracil (5FU) cream as adjuvant therapy following cervical intraepithelial neoplasia grade 2 or 3 (CIN2/3) treatment among women living with HIV (WLWH) in Kenya.
Methods: A phase I pilot trial was performed among 12 WLWH in Kenya, aged 18-49 years between March 2023 and February 2024 (ClinicalTrial.gov NCT05362955).

Currently there is no detailed, internationally agreed protocol defined to evaluate antimicrobial susceptibility testing (AST) for Legionella pneumophila (required to establish epidemiological cut-off value or "ECOFF" boundaries); therefore, antimicrobial resistance in these isolates cannot be defined. AST methods utilising media containing activated charcoal as an ingredient, to enable Legionella growth, are unreliable as noted in an internationally authored opinion paper and a new gold standard is required. Here we define a detailed protocol for broth microdilution (BMD) using defined cell culture collection-deposited control reference strains (Philadelphia-1 and Knoxville-1) as well as two accessible reference strains with moderately (lpeAB-carrying) and markedly (23S rRNA mutation-carrying) elevated azithromycin minimum inhibitory concentration (MIC).

There is growing interest in the potential exploitation of the gut microbiome as a diagnostic tool in medicine, but evidence supporting its clinical usefulness is scarce. An increasing number of commercial providers offer direct-to-consumer microbiome diagnostic tests without any consensus on their regulation or any proven value in clinical practice, which could result in considerable waste of individual and health-care resources and potential drawbacks in the clinical management of patients. We convened an international multidisciplinary expert panel to standardise best practices of microbiome testing for clinical implementation, including recommendations on general principles and minimum requirements for their provision, indications, pre-testing protocols, method of analyses, reporting of results, and potential clinical value.

Mycelial growth as biofilm structures and the activation of secondary metabolism leading to the release of low-molecular-weight molecules (known as secondary metabolites), are among the previously described strategies used by the filamentous fungi to adapt and survive. Our study unveils that strains can activate mechanisms linked to the production of gliotoxin, a crucial metabolite for , in the established biofilm model. Gliotoxin production exhibits strain- and time-dependent patterns and is associated -in a coordinated manner-with the expression levels of several genes involved in its regulation and synthesis.
