Article Synopsis
  • Pyoderma gangrenosum (PG) is a rare skin condition linked to immune system issues, but its exact causes are still unclear, prompting a systematic review to explore its connections with inborn errors of immunity.
  • A comprehensive literature search revealed 74 cases of PG associated with various genetic mutations, suggesting that PG can occur with a range of immunological deficiencies not typically linked to the disorder.
  • Certain genetic mutations, such as those in PSMB8 and NLRP3, are associated with severe cases of PG, while others, like mutations in RAG1 and complement deficiencies, tend to lead to milder symptoms.

OHSU Center for Health and Healing, Oregon Health & Science University, 3485 S Bond Ave, Mail Code OC14HO, Portland, OR, 97239, USA.

Purpose Of Review: Acute lymphoblastic leukemia (ALL) is a widely heterogeneous disease in terms of genomic alterations, treatment options, and prognosis. While ALL is considered largely curable in children, adults tend to have higher risk disease subtypes and do not respond as favorably to conventional chemotherapy. Identifying genomic drivers of leukemogenesis and applying targeted therapies in an effort to improve disease outcomes is an exciting focus of current ALL research.

Background: Exercise can rapidly drop glucose in people with type 1 diabetes. Ubiquitous wearable fitness sensors are not integrated into automated insulin delivery (AID) systems. We hypothesised that an AID can automate insulin adjustments using real-time wearable fitness data to reduce hypoglycaemia during exercise and free-living conditions compared with an AID not automating use of fitness data.

Article Synopsis
  • A study was conducted to assess whether a dermatology urgent care model could decrease healthcare and emergency service utilization among patients with psychiatric dermatoses, such as Morgellons disease and neurotic excoriations.
  • The results showed a significant reduction: an 88% decrease in overall healthcare visits and a 77% decrease in emergency room visits after patients engaged with the dermatology department.
  • The findings suggest that adopting urgent care models in dermatology can help manage healthcare overuse in this patient population, although limitations exist in tracking all healthcare usage.

Division of Facial Plastic and Reconstructive Surgery, Department of Otolaryngology-Head and Neck Surgery, Oregon Health & Science University, Center for Health and Healing, CH5E, 3303 Southwest Bond Avenue, Portland, OR 97201, USA.

Most surgeons who are not routinely treating gender dysphoric patients are more likely to see an isolated rhinoplasty consultation rather than a request for full facial gender confirmation surgery (FGCS). Different from other aspects of FGCS, the surgical basis of rhinoplasty is almost the same as for the cisgender population. Despite technical overlap, the care for patients seeking rhinoplasty for the indication of gender dysphoria vastly differs from that for the cisgender population.

Patients with skin-predominant lupus erythematosus, dermatomyositis, and morphea should be evaluated, treated, and followed by dermatologists who can take primary responsibility for their care. Many academic centers have specialized centers with dermatologists who care for these patients. Patients with skin-predominant lupus erythematosus should be followed regularly with laboratory tests to detect significant systemic disease.

Febrile ulceronecrotic Mucha-Habermann disease (FUMHD), a severe form of pityriasis lichenoides et varioliformis acuta (PLEVA), featuring large, ulcerative, necrotic skin plaques, high fever, and other systemic symptoms, is a rare disorder of unknown etiology. No randomized controlled trials have established treatment guidelines and multiple modalities are often employed, making it difficult to assess the efficacy of any single agent. We report two cases of this condition in which treatment with methotrexate plus antibiotic treatment for superinfection led to rapid improvement.
