Borderline personality features in relationship to childhood trauma in unipolar depressive and bipolar disorders.

J Affect Disord

October 2024

Leiden University Medical Center (LUMC), Department of Psychiatry, Leiden, the Netherlands; Health Campus The Hague, Department of Public Health and Primary Care, Leiden University Medical Centre, The Hague, the Netherlands.

Background: Childhood trauma, including emotional neglect, emotional abuse, physical abuse, and sexual abuse, may contribute to borderline personality features like affective instability, identity problems, negative relationships, and self-harm. This study aims to explore how different types of childhood trauma affect these features in bipolar versus unipolar depressive disorders.

Methods: We included 839 participants of the Netherlands Study of Depression and Anxiety (NESDA) with a lifetime diagnosis of major depressive disorder single episode (MDDS; N = 443), recurrent major depressive disorder (MDD-R; N = 331), or bipolar disorder (BD; N = 65).

Background: Mindfulness-based cognitive therapy (MBCT) seems a promising intervention for bipolar disorder (BD), but there is a lack of randomised controlled trials (RCT) investigating this. The purpose of this multicentre, evaluator blinded RCT was to investigate the added value of MBCT to treatment as usual (TAU) in BD up to 15 months follow-up (NCT03507647).

Methods: A total of 144 participants with BD type I and II were randomised to MBCT + TAU ( = 72) and TAU ( = 72).

Objective: This article describes the development and psychometric evaluation of the Manic Thought Inventory (MTI), a patient-driven self-report inventory to assess the presence of typical (hypo)manic cognitions.

Methods: The initial item pool was generated by patients with bipolar disorder (BD) type I and assessed for suitability by five psychiatrists specialized in treating BD. Study 1 describes the item analysis and exploratory factor structure of the MTI in a sample of 251 patients with BD type I.

Pharmacotherapy is a cornerstone in bipolar disorder (BD) treatment whereas borderline personality disorder (BPD) is treated primarily with psychotherapy. Given the overlap in symptomatology, patients with BD may benefit from psychotherapy designed for BPD. This paper reports the findings of a non-controlled open feasibility study of STEPPS training in patients with BD and borderline personality features (BPF).

Article Synopsis
  • The study aims to examine how pregnancy affects the course of bipolar disorder (BD) by comparing daily mood scores using a method called Lifechart Method (LCM) in pregnant and non-pregnant women.
  • The research analyzed two groups: 34 pregnant women and 52 non-pregnant women, focusing on aspects like the frequency of symptomatic days, severity of symptoms, and patterns in mood fluctuations.
  • Results suggested there were no significant differences in overall BD course between the two groups, but more pregnant women were classified as moderately ill, and their daily mood variation was less compared to non-pregnant women.

Purpose: To explore the experiences of ambulance nurses in emergency care of patients with acute manic and/or psychotic symptoms.

Methods: In this qualitative study, 14 interviews were conducted and analyzed using thematic analysis according to Braun and Clarke (2006).

Findings: Psychiatric emergency care causes stress and uncomfortable feelings for ambulance nurses due to a lack of information on the patients, being alone with the patient in a small place and the unpredictability of the situation.

Background: When patients diagnosed with bipolar disorder are suffering from acute mania (involuntary), hospitalization is often necessary. Patients are often quite disruptive, which makes it difficult to regulate their behavior and manage them in their current condition. The nursing team must also ensure the safety of the other patients on the ward.

Background: Despite multiple pharmacological interventions, many people with bipolar disorder (BD) experience substantial residual mood symptoms, even in the absence of severe mood episodes, which have a negative impact on the course of illness and quality of life. Limited data are available on how to optimize treatment for BD, especially for those who suffer from persistent and residual depressive symptoms. Preliminary evidence suggests Mindfulness-Based Cognitive Therapy (MBCT) as a psychological treatment option for BD.

Measuring personal recovery in people with bipolar disorder and exploring its relationship with well-being and social role participation.

Clin Psychol Psychother

September 2019

Centre for eHealth and Well-being Research, Department of Psychology, Health, and Technology, University of Twente, Enschede, Netherlands.

Article Synopsis
  • - The study focused on evaluating a Dutch translation of the Questionnaire about the Process of Recovery (QPR) for measuring personal recovery in individuals with bipolar disorder (BD) and assessed its psychometric properties.
  • - Results confirmed that the QPR has a strong, unidimensional factor structure and excellent internal consistency, with significant correlations to measures of well-being, social role participation, and psychopathology.
  • - Findings suggest that personal recovery is a critical aspect of BD treatment, as it explains additional variance in symptoms of depression and anxiety, highlighting its overlap with overall well-being.

Article Synopsis
  • Mood disorders during pregnancy are complex to treat, with limited evidence on how pregnancy affects their progression and the benefits of continuing medication despite potential risks to the fetus.* -
  • A systematic review analyzed 22 studies from a pool of 1387, revealing varied recurrence rates of bipolar disorder (19% average) and major depressive disorder (8% average) during pregnancy, and showing that maintenance therapy significantly reduced recurrence risks.* -
  • The research highlights gaps in understanding the effects of pregnancy on mood disorders, pointing to the need for more robust studies to clarify these issues and confirm the benefits of continued pharmacotherapy for affected women.*

The characteristics of psychotic features in bipolar disorder.

Psychol Med

September 2019

Department of Psychiatry, University Medical Center Utrecht, Brain Center Rudolf Magnus, Utrecht, The Netherlands.

Background: In a large and comprehensively assessed sample of patients with bipolar disorder type I (BDI), we investigated the prevalence of psychotic features and their relationship with life course, demographic, clinical, and cognitive characteristics. We hypothesized that groups of psychotic symptoms (Schneiderian, mood incongruent, thought disorder, delusions, and hallucinations) have distinct relations to risk factors.

Methods: In a cross-sectional study of 1342 BDI patients, comprehensive demographical and clinical characteristics were assessed using the Structured Clinical Interview for DSM-IV (SCID-I) interview.

Background: Although bipolar disorder (BD) and borderline personality disorder (BPD) share clinical characteristics and frequently co-occur, their interrelationship is controversial. Especially, the differentiation of rapid cycling BD and BPD can be troublesome. This study investigates the relationship between borderline personality features (BPF) and prospective illness course in patients with BD, and explores the effects of current mood state on self-reported BPF profiles.

Memantine augmentation in clozapine-refractory schizophrenia: a randomized, double-blind, placebo-controlled crossover study.

Psychol Med

July 2016

Early Psychosis Department,Academic Medical Center, University of Amsterdam, Academic Psychiatric Center,Arkin,Amsterdam,The Netherlands.

Background: Dysfunction of neuroplasticity due to N-methyl-d-aspartate (NMDA) receptor hypofunction may be a causal factor for memory and executive dysfunctioning in schizophrenia. Deregulation of NMDA transmission in the prefrontal cortex may also explain negative and positive symptoms. Clozapine augmentation with memantine targets altered NMDA receptor-mediated neurotransmission in schizophrenia and showed substantial beneficial effects on several symptom domains in a small proof-of-concept study.
