A 52-year-old gentleman presented with symptoms of breathlessness and type 1 respiratory failure. His CT pulmonary angiogram showed a heterogenous, oval-shaped lesion between the heart and diaphragm with a right atrial (RA) filling defect, pericardial thickening and pulmonary metastasis. An RA debulking salvage operation confirmed this to be a pericardial tumour and further cytology and immunohistochemistry testing confirmed a primary synovial sarcoma.

Purpose: Transoral surgery for suspected or proven oropharyngeal malignancies has increased significantly with the practice of transoral laser microsurgery (TLM) and transoral robotic surgery (TORS). An accepted alternative technique is endoscopic video-assisted transoral (EVAT) surgery. Our aim is to review the clinical, oncological and functional outcomes of this technique at our institution.

A microfluidic chip based model for the study of full thickness human intestinal tissue using dual flow.

Biomicrofluidics

November 2016

Faculty of Life Sciences, University of Hull, Cottingham Road, Hull HU6 7RX, United Kingdom.

The study of inflammatory bowel disease, including Ulcerative Colitis and Crohn's Disease, has relied largely upon the use of animal or cell culture models; neither of which can represent all aspects of the human pathophysiology. Presented herein is a dual flow microfluidic device which holds full thickness human intestinal tissue in a known orientation. The luminal and serosal sides are independently perfused with nutrients with simultaneous waste removal for up to 72 h.

Aim: The medical management of inflammatory bowel disease (IBD) in pregnancy and the puerperium is well defined. Data on surgical management of complicated IBD in this setting are lacking. This study aimed to determine the optimal surgical strategy for medically refractory IBD during pregnancy and the puerperium.

Objective: To determine the nature and frequency of distorted presentation or "spin" (ie, specific reporting strategies which highlight that the experimental treatment is beneficial, despite a statistically nonsignificant difference for the primary outcome, or distract the reader from statistically nonsignificant results) in published reports of randomized controlled trials (RCTs) with statistically nonsignificant results for primary outcomes in surgical journals.

Background: Multiple reports have suggested that interpretation of RCT results in medical journals can be distorted by authors of published reports.

Methods: Using a defined search strategy, RCTs with clearly nonsignificant results for the primary outcome (P > 0.

A Systematic Review of Outcomes After Transanal Mesorectal Resection for Rectal Cancer.

Dis Colon Rectum

April 2016

Department of Colorectal Surgery, Castlehill Hospital, Cotttingham, East Yorkshire, United Kingdom.

Background: Transanal mesorectal resection has been developed to facilitate minimally invasive proctectomy for rectal cancer.

Objective: The purpose of this study was to evaluate the evidence regarding technical parameters, oncological outcomes, morbidity, and mortality after transanal mesorectal resection.

Data Sources: The Cochrane Library, PubMed, and MEDLINE databases were reviewed.

We report a case of a previously healthy 23-year-old Somalian care assistant. She presented with a 4 month history of persistent occipital headaches associated with intermittent nausea and vomiting. Computed tomography and magnetic resonance imaging of the brain showed a large enhancing lesion in the right cerebellar hemisphere with surrounding ring lesions, suggestive of an intracranial neoplasm with metastases.

COPD in young patients: a pre-specified analysis of the four-year trial of tiotropium (UPLIFT).

Respir Med

November 2010

Cardiovascular and Respiratory Studies, HullYork Medicine School, University of Hull, CastleHill Hospital, Cottingham, East Yorkshire, UK.

Whilst recent large-scale studies have provided much evidence on the natural history and therapeutic response in patients with chronic obstructive pulmonary disease (COPD), relatively little is known about the effect in younger patients. We report a pre-specified post-hoc analysis of 356 patients with COPD ≤ 50 years old from the four year randomised, double blind placebo controlled Understanding Potential Long Term Impact on Function with Tiotropium (UPLIFT) trial. Inclusion criteria included a post-bronchodilator forced expiratory volume in 1 s (FEV(1)) of ≤70%, FEV(1)/FVC < 0.

Giant colonic diverticulum.

Ir J Med Sci

April 2005

Academic Surgical Unit, Castlehill Hospital, Hull, East Yorkshire, UK.

Background: Over 100 giant colonic diverticuli have been recorded in the literature to date. The magnetic resonance imaging (MRI) and pathophysiology of their aetiology have rarely been described.

Aim: To report a giant colonic diverticulum along with the MRI of this clinical entity.

Background: Heart failure is commonly associated with vascular disease and a high rate of athero-thrombotic events, but the risks and benefits of antithrombotic therapy are unknown.

Methods: The current study was an open-label, randomized, controlled trial comparing no antithrombotic therapy, aspirin (300 mg/day), and warfarin (target international normalized ratio 2.5) in patients with heart failure and left ventricular systolic dysfunction requiring diuretic therapy.

Does rectal washout during anterior resection prevent local tumor recurrence?

Dis Colon Rectum

March 2004

Academic Surgical Unit, Castlehill Hospital, Cottingham-Hull, United Kingdom.

Purpose: Colorectal cancer is one of the leading causes of cancer deaths in the industrialized nations. Left-sided tumors, especially rectal, rectosigmoid, and sigmoid, account for more than half of these tumors. Among many colorectal surgeons, the practice of rectal washout with cytocidal agents before anastomosis is common.

Chronic pain due to talc pleurodesis for spontaneous pneumothorax.

Ann Thorac Surg

November 2003

East Yorkshire Heart Center, Castlehill Hospital, Cottingham, Hull, United Kingdom.

Spontaneous pneumothorax is a common condition that usually affects young adults. Talc insufflation, either as a poudrage or by using slurry, is recognized as an effective treatment. We report a case of talc that resulted in chronic debilitating pain and necessitated further surgery.

Background: Extrarenal retroperitoneal angiomyolipomas are benign tumours of mesenchymal tissue origin. The diagnosis of these tumours is usually based on the perioperative findings and confirmed by the resultant specimen's histology. The main differential diagnosis is liposarcoma, and accurate preoperative imaging combined with fine-needle aspiration cytology may permit a non-operative approach to these tumours.
