Endodontic retreatment can be a challenging task that can result in many complications if not approached cautiously. Many of these difficulties revolve around regaining access to the pulp chamber through extensive coronal restorations and removing residual root filling material, the commonest being gutta-percha (GP), from within obturated canals. This can often be an untidy, time consuming process that places teeth at a greater risk of iatrogenic injury and inhibits the operator achieving the necessary chemical disinfection required to eliminate the persistent apical disease.

The use of dental implants is an accepted and predictable way of replacing missing or lost teeth. However, implants can and will fail and there are a variety of reasons why this occurs, which the practitioner should understand. In some instances failed implants may require removal and, therefore, practitioners should be aware of techniques that can be used to remove failed implants to potentially enable future rehabilitation of an edentulous region.

There is evidence of a correlation between special needs, unequal access to dental care, poor dental health and insufficient improvement in dental services, particularly for children with special healthcare needs. Factors contributing to this situation include the characteristics of the individual's disability, parental education and socioeconomic status, lack of appropriately trained dentists and a lack of research in the UK to guide progress.

The occurrence of odontogenic keratocysts in the maxilla is relatively rare, and invasion of the maxillary sinus unusual. Such cysts are normally small and unilocular. Two large keratocysts with different clinical features and radiological appearances, which had invaded the sinus, are discussed in the case reports presented here.

For the patient described in this report a nonprecious metal alloy that contains neither beryllium nor nickel was used. The retainers were sandblasted with 50 microns alumina and cemented by using a resin that bonds chemically to metal. With good oral hygiene and regular review the resin-bonded split-post prosthesis should provide a long period of service.
