Novel protective role of endogenous cardiac myocyte P2X4 receptors in heart failure.

Circ Heart Fail

May 2014

From Pat and Jim Calhoun Cardiology Center, University of Connecticut Medical Center, Farmington, CT (T.Y., J.S., R.Y., J.R., K.D.-K., J.G., B.T.L.); and Laboratory of Bioorganic Chemistry, NIDDK, National Institutes of Health, Bethesda, MD (K.A.J.).

Background: Heart failure (HF), despite continuing progress, remains a leading cause of mortality and morbidity. P2X4 receptors (P2X4R) have emerged as potentially important molecules in regulating cardiac function and as potential targets for HF therapy. Transgenic P2X4R overexpression can protect against HF, but this does not explain the role of native cardiac P2X4R.

Novel Approaches for the Treatment of the Patient with Resistant Hypertension: Renal Nerve Ablation.

Curr Cardiovasc Risk Rep

October 2013

Division of Hypertension and Clinical Pharmacology, Calhoun Cardiology Center, University of Connecticut School of Medicine, Farmington, Connecticut.

Sympathetic innervation of the kidneys plays a major role in the pathogenesis of hypertension through modulation of renin secretion, glomerular filtration rate and renal absorption of sodium. Targeted interventions for renal nerve ablation are being developed for treatment of drug resistant hypertension in the USA and rest of the world. Early studies with the use of radiofrequency based renal denervation systems have shown encouraging results with significant reduction of blood pressure in patients inadequately controlled despite nearly maximal drug therapy regimens.

Alogliptin after acute coronary syndrome in patients with type 2 diabetes.

N Engl J Med

October 2013

Calhoun Cardiology Center, Department of Medicine, University of Connecticut School of Medicine, 263 Farmington Ave., Farmington, CT 06030-3940, USA.

Background: To assess potentially elevated cardiovascular risk related to new antihyperglycemic drugs in patients with type 2 diabetes, regulatory agencies require a comprehensive evaluation of the cardiovascular safety profile of new antidiabetic therapies. We assessed cardiovascular outcomes with alogliptin, a new inhibitor of dipeptidyl peptidase 4 (DPP-4), as compared with placebo in patients with type 2 diabetes who had had a recent acute coronary syndrome.

Methods: We randomly assigned patients with type 2 diabetes and either an acute myocardial infarction or unstable angina requiring hospitalization within the previous 15 to 90 days to receive alogliptin or placebo in addition to existing antihyperglycemic and cardiovascular drug therapy.

Review of the state of renal nerve ablation for patients with severe and resistant hypertension.

J Am Soc Hypertens

September 2014

Division of Hypertension and Clinical Pharmacology, Calhoun Cardiology Center, University of Connecticut School of Medicine, Farmington, CT. Electronic address:

Through modulation of renin secretion, glomerular filtration rate, and renal absorption of sodium, the sympathetic innervation of the kidneys plays an important role in the pathogenesis of hypertension. Renal nerve ablation technology is being developed for treatment of drug-treatment-resistant hypertension worldwide. Preliminary research with the use of radiofrequency-based renal denervation systems have demonstrated encouraging results with significant reduction of blood pressure in patients inadequately controlled despite nearly maximal drug therapy regimens.

Is there still a role for ultrafiltration in the management of acute heart failure? CARRESS and beyond.

Curr Heart Fail Rep

September 2013

Pat and Jim Calhoun Cardiology Center, UCONN Health Center, 263 Farmington Avenue, Farmington, CT 06030, USA.

The Cardiorenal Rescue Study in Acute Decompensated Heart Failure (CARRESS-HF) trial was a prospective, randomized study comparing ultrafiltration versus pharmacological therapy in the treatment of acute decompensated heart failure (ADHF) complicated by cardiorenal syndrome Bart et al. (N Eng J Med 367:2296-2304, 1). The study found that ultrafiltration was inferior to pharmacological therapy, resulting in a significant increase in serum creatinine and serious adverse events while producing no significant difference in weight loss.

Income received during treatment does not affect response to contingency management treatments in cocaine-dependent outpatients.

Drug Alcohol Depend

October 2013

Calhoun Cardiology Center-Behavioral Health, University of Connecticut Health Center MC-3944, 263 Farmington Avenue, Farmington, CT 06030-3944, United States. Electronic address:

Background: Prior studies find no effect of baseline income on response to contingency management (CM) interventions. However, income among substance disordered patients is variable, particularly at treatment entry. This study investigated the impact of during-treatment income, a more proximal estimate of economic resources at the time that CM is in effect, on response to standard treatment or the standard treatment plus CM.

Background And Purpose: Transient receptor potential melastatin 7 (TRPM7) is a unique channel kinase which is crucial for various physiological functions. However, the mechanism by which TRPM7 is gated and modulated is not fully understood. To better understand how modulation of TRPM7 may impact biological processes, we investigated if TRPM7 can be regulated by the phospholipids sphingosine (SPH) and sphingosine-1-phosphate (S1P), two potent bioactive sphingolipids that mediate a variety of physiological functions.
