Left Vertebral Artery Arising Directly From the Arch of the Aorta With a Prevailing Vascular Problem: A Case Report.

Aberrant origin of the vertebral artery is a rare case. Due to its important clinical significance during operations in the superior mediastinum and the root of the neck, the variations of this artery should be clarified, and any cadaveric case should be explored specifically if accompanied by any vascular problem. In this cadaveric case, the embalmed male cadaver was found to have a pacemaker wire inserted in the heart through the superior vena cava, denoting a vascular incompetence due to sinus arrhythmia.

Proposal of a modified tip apex distance for prediction of lag screw cut-out in trochanteric hip fractures.

Introduction: Lag screw cut-out is a serious complication of dynamic hip screw fixation of trochanteric hip fractures. The lag screw position has been acknowledged as one of the important factors affecting the lag screw cut-out. We propose a modification of the Tip Apex Distance (TAD) and hypothesise that it could improve the reliability of predicting lag screws cut-out in these injuries.

Successful prioritisation of inguinal herniotomies in children during the COVID-19 pandemic to minimise emergency presentations.

Background: The coronavirus disease 2019 (COVID-19) disrupted the delivery of elective surgery in children. We introduced guidance to mitigate this impact. By reviewing the outcomes for inguinal herniotomies, we aimed to determine if this guidance has enabled us to prevent an increase in the elective surgery wait time and therefore the need for emergency surgery for incarcerated hernias.
