Article Synopsis
  • A study called ACTION-Galactosemia Kids evaluated the effects of govorestat, a drug for Classic Galactosemia, on children aged 2-17 by comparing it to a placebo over 18 months.
  • Govorestat showed a significant and lasting reduction in plasma galactitol levels, leading to stabilization or improvement in various clinical measures like behavior and daily living skills, while the placebo group showed decline.
  • Despite its benefits, govorestat did not show improvements in speech or gross motor skills, and both the drug and placebo groups experienced similar rates of adverse effects, indicating it is safe and well tolerated.

Runt-related transcription factor 1 translocated to 1 (RUNX1T1; also known as eight-twenty-one [ETO]) encodes a transcription regulator for hematopoietic genes and is well known for its involvement in hematologic malignancies, particularly acute myeloid leukemia (AML). However, its role in congenital disease is less understood. This study provides detailed clinical and molecular information on three cases exhibiting neurodevelopmental and congenital anomalies with germline de novo alterations in RUNX1T1.

  • The study analyzed trends in the economic and utilization of sliding hip screws (SHS) and intramedullary nails (IMN) for treating intertrochanteric femur fractures among Medicare patients over 21 years.
  • It found that the use of IMN significantly increased by 695%, while SHS use dropped by 96% during the same period.
  • Furthermore, physician reimbursements for both methods decreased, with SHS averaging $943.36 and IMN $999.88, indicating a possible cost-effectiveness in choosing SHS when suitable.

The Role of Ultrasound in Bedside Procedures.

Med Clin North Am

January 2025

Department of Medicine, University of Texas Health Science Center San Antonio, 7703 Floyd Curl Drive, MC 7982, San Antonio, TX 78229, USA.

Ultrasound guidance for bedside procedures improves rates of success while reducing complications. It is the standard of care for most bedside procedures and has ever-increasing utility for peri-procedural assessment. Herein, we provide a concise description with associated images and videos detailing the proper technique for ultrasound image acquisition and interpretation, along with common pitfalls to be avoided.

Article Synopsis
  • A balance between dopamine and acetylcholine (ACh) is crucial for motor function, and imbalances can lead to issues like Parkinson's disease (PD) and related motor disorders.
  • Contrary to traditional beliefs that ACh signaling is overly active in PD, recent findings show that cholinergic transmission at specific synapses is actually decreased due to reduced M4 receptor function in dopamine-depleted mice.
  • Restoring M4 receptor activity helped alleviate some motor deficits and dyskinesia, suggesting a new therapeutic target for treating Parkinson's and its associated complications.

We describe our experience implementing an intensive quality improvement cohort pilot focused on managing asymptomatic bacteriuria in 19 critical access hospitals. Participation in the pilot was high, and almost all sites identified an improvement goal and collected clinical data. Barriers to implementation included staffing shortages, turnover, and lack of bandwidth.

Ethical issues in implementation science: perspectives from a National Heart, Lung, and Blood Institute workshop.

Implement Sci

November 2024

Center for Translation Research and Implementation Science, National Heart, Lung, and Blood Institute, National Institutes of Health, Bethesda, MD, USA.

Ethical issues arise in the context of implementation science that may differ from those encountered in other research settings. This report, developed out of a workshop convened by the Center for Translation Research and Implementation Science within the United States National Heart, Lung, and Blood Institute, identifies six key themes that are important to the assessment of ethical dimensions of implementation science. First, addressing ethical challenges in implementation science does not require new ethical principles, commitments, or regulations.

This narrative review seeks to summarize chemotherapeutic regimens commonly used for patients with newly diagnosed Philadelphia (Ph) chromosome-negative B-cell precursor acute lymphoblastic leukemia (BCP-ALL) in the frontline setting and to describe the latest clinical research using the bispecific T-cell-engaging immunotherapy blinatumomab in the first-line treatment setting. Current standard-of-care chemotherapeutic backbones for newly diagnosed Ph-negative BCP-ALL are based on the same overarching treatment principle: to reduce disease burden to undetectable levels and maintain lasting remission. The adult treatment landscape has progressively evolved following the adoption of pediatric-inspired regimens.

Imaging of primary periphyseal finger stress injuries in climbers.

Skeletal Radiol

November 2024

Department of Orthopedic and Trauma Surgery, Klinikum Bamberg, Bamberg, FRG, Germany.

Periphyseal stress injuries (PPSIs) in young rock climbers are gaining recognition due to the surge in popularity of the sport. These injuries, occurring near the growth plates of fingers, pose diagnostic challenges due to their subtle presentation and potential long-term complications if left untreated. This review emphasizes the importance of radiologists' awareness of PPSIs, covering their anatomy, biomechanics, and imaging findings.

Prostaglandin E regulates senescence and post-senescence neoplastic escape in primary human keratinocytes.

Aging (Albany NY)

November 2024

CNRS, Inserm, CHU Lille, Institut Pasteur de Lille, UMR9020 – U1277 – CANTHER - Cancer Heterogeneity, Plasticity and Resistance to Therapies, University of Lille, Lille F-59000, France.

Aging of the epidermis partially occurs as a consequence of epidermal cell senescence, a non-proliferative state in which cells remain metabolically active and acquire changes in their secretome. We previously reported that senescent normal human epidermal keratinocytes (NHEKs) have two opposite outcomes: either cell death by excess of autophagic activity or escape from senescence to give rise to post-senescence neoplastic emerging (PSNE) cells. In this study, we investigated the role of PTGS2, the inducible enzyme of the prostaglandin biosynthesis pathway, in the onset of NHEK senescence and in the switch from senescence to pre-transformation.

Ann Neurol

November 2024

Neuroinfectious Diseases Group, Department of Neurology, and Department of Medicine and Immunology-Microbiology, University of Colorado School of Medicine, Aurora, CO.

Article Synopsis
  • - Oropouche virus (OROV) is an emerging arbovirus found in parts of South and Central America, the Caribbean, and is now being reported in Cuba, the US, and Europe, primarily transmitted between certain animals and mosquitoes to humans.
  • - It causes flu-like symptoms in about 60% of infections, can have a biphasic course leading to symptom recurrence, and poses risks of vertical transmission that might affect pregnancies and fetal development.
  • - Diagnosis involves detecting the virus or antibodies in blood or cerebrospinal fluid, with no antiviral treatments or vaccines currently available, making mosquito bite prevention essential for control.

Background: Higher level of care (HLOC) treatment for eating disorders (EDs) is sometimes necessary, but research is lacking on whether HLOCs are actually more effective than less structured, lower levels of care. The purpose of the current study was to compare outcomes for patients with EDs at low weights who entered 24/7 care (inpatient and residential) to those entering non-24/7 care (partial hospitalization programming and intensive outpatient programming).

Methods: Participants were 1104 adults with body mass indices (BMI) between 14 and 17 receiving treatment for an ED at a large multisite treatment facility offering HLOCs between August 2019 and February 2024.

Our aim is to determine the rate of unexpected malrotation identified on routine preoperative upper gastrointestinal (UGI) contrast study in infants with congenital heart disease (CHD) prior to gastrostomy tube (GT) placement and quantify any associated delay in care. We performed a retrospective review of infants with CHD who underwent GT placement following initial cardiac surgery at a single center between 2016 and 2021. Patients were identified in the electronic medical record.
