Although physical activity (PA) is recommended for individuals with serious mental illness (ISMI), most of them are inactive or insufficiently active. The transtheoretical model (TTM) has been proposed to explain the motivational factors associated with PA however, there is little evidence of its applicability to ISMI. So, the objective was to analyse whether TTM components are related to PA in overweight ISMI.

Electronic address:

The blood-brain barrier (BBB) constitutes an elaborate structure formed by specialized capillary endothelial cells, which together with pericytes and perivascular glial cells regulates the exchanges between the central nervous system (CNS) and the periphery. Intricate interactions between the different cellular constituents of the BBB are crucial in establishing a functional BBB and maintaining the delicate homeostasis of the CNS microenvironment. In this review, we discuss the role of astrocytes and microglia in inducing and maintaining barrier properties under physiological conditions as well as their involvement during neuroinflammatory pathologies.

Article Synopsis
  • The study investigates the role of melanoma cell adhesion molecule (MCAM) in the recruitment of pathogenic CD8(+) T lymphocytes to the central nervous system (CNS) in multiple sclerosis (MS) and experimental autoimmune encephalomyelitis (EAE).
  • Researchers utilized various methods to analyze MCAM(+) CD8(+) T lymphocytes in both humans and mice, observing that these cells are particularly active during MS relapses and possess enhanced inflammatory capabilities.
  • Findings suggest that MCAM may serve as both a marker for disease activity in MS and a potential therapeutic target for treating neuroinflammatory disorders.

This study identifies patient risk factors present prior to an acute hospitalization that are associated with occupying acute care beds for non-acute reasons on the 30 day of a hospitalization. Data from 952 adult patients were obtained, among which 333 (35%) were evaluated as non-acute on their 30 day. Inability to move in and out of the bed, cognitive impairment, receiving home or community healthcare services prior to hospitalization, unavailable family resources, a secondary diagnosis within the mental and behavioural category, and age ≥75 years were found to increase the risk of occupying acute care beds for non-acute reasons, while patients with a feeding tube were less likely to be non-acute at day 30.

Does aneurysmal wall enhancement on vessel wall MRI help to distinguish stable from unstable intracranial aneurysms?

Stroke

December 2014

From the Departments of Radiology (M.E., C.R.-R., D.T., W.B.H., S.L., J.F.M., C.O., O.N.) and Neurosurgery (F.N.), Université Paris Descartes Sorbonne Paris Cité, INSERM S894, Centre Hospitalier Sainte-Anne, Paris, France; Department of Interventional Neuroradiology, University of Montreal, CHUM Notre-Dame Hospital, Montreal, Quebec, Canada (J.-C.G., J.R.); and the Departments of Medical Physics and Radiology (O.W., P.T.), University of Wisconsin, Madison.

Background And Purpose: Arterial wall enhancement on vessel wall MRI was described in intracranial inflammatory arterial disease. We hypothesized that circumferential aneurysmal wall enhancement (CAWE) could be an indirect marker of aneurysmal wall inflammation and, therefore, would be more frequent in unstable (ruptured, symptomatic, or undergoing morphological modification) than in stable (incidental and nonevolving) intracranial aneurysms.

Methods: We prospectively performed vessel wall MRI in patients with stable or unstable intracranial aneurysms.

Y-stent placement to treat bifurcation aneurysms requires the second device to cross the confines of the first stent, with concerns regarding the formation of stenosis of the second device at the site of crossing. Various braided stents and flow diverters (FDs) were deployed to cross through a high porosity braided stent, in a Y configuration, with the ends of the devices inserted in plastic tubes of various diameters, leaving the mid-portion free to expand. The ensuing constructs were photographed, paying attention to the degree of stenosis, if any, created where the second device crosses the first stent.

While the risk of haemorrhage related to aneurysm recurrence after endovascular treatment is not well established, concerns about it lead to imaging follow-up and further treatments that have a real impact on patients' lives. These concerns are also the motor of technological developments aiming at reducing the recurrence rate but also having the potential to increase the complication rate of initial treatment. This article will address some issues related to aneurysm recurrence in view of the actual literature.

Analysis by categorizing or dichotomizing continuous variables is inadvisable: an example from the natural history of unruptured aneurysms.

March 2011

Department of Radiology, Interventional Neuroradiology Research Unit, International Consortium of Neuroendovascular Centres, University of Montreal, CHUM Notre-Dame Hospital, Quebec, Canada.

In medical research analyses, continuous variables are often converted into categoric variables by grouping values into ≥2 categories. The simplicity achieved by creating ≥2 artificial groups has a cost: Grouping may create rather than avoid problems. In particular, dichotomization leads to a considerable loss of power and incomplete correction for confounding factors.

Autosomal dominant sensory ataxia is a rare genetic condition that results in a progressive ataxia that is caused by degeneration of the posterior columns of the spinal cord. To date only two families have been clinically ascertained with this condition, both from Maritime Canada. We previously mapped both families to chromosome 8p12-8q12 and have now screened the majority of annotated protein-coding genes in the shared haplotype region by direct DNA sequencing.
