EUS-guided hepaticogastrostomy (EUS-HGS) is one of the preferred methods in biliary drainage where ERCP fails or is contraindicated. The clinical outcomes of EUS-HGS are not well studied because of variability in procedure technique. We conducted a search of multiple electronic databases and conference proceedings from inception through January 2023.

Early and Late Readmissions of Radiation Proctitis in the United States: Are We Getting Better?

J Clin Med

January 2024

Interventional Oncology & Surgical Endoscopy (IOSE) Division, GI Oncology Tumor Site Team, Parkview Cancer Institute, Parkview Health, Fort Wayne, IN 46845, USA.

Cold Versus Hot Endoscopic Mucosal Resection for Sessile Serrated Colorectal Polyps ≥10 mm: A Systematic Review and Meta-analysis.

J Clin Gastroenterol

October 2024

Department of Gastroenterology, Center for Advanced Therapeutic Endoscopy (CATE), Centura Health, Porter Adventist Hospital, Denver, CO.

Introduction: We performed a systematic review and meta-analysis studying the efficacy and safety of cold versus hot endoscopic mucosal resection (EMR) for resection of sessile serrated polyps (SSPs) ≥10 mm.

Methods: Multiple databases were searched until January 2023 for studies reporting outcomes of cold versus hot EMR for SSPs ≥10 mm. The primary outcome was the residual SSP rate.

Background And Objectives: Gastric varices (GVs) are associated with a higher risk of uncontrolled bleeding and death when compared with esophageal varices. While endoscopic glue injection therapy has been traditionally used for secondary prophylaxis in GV, data regarding primary prophylaxis continue to emerge. Recently, EUS-guided therapies have been used in GV bleeding.

Background/objectives: We aimed to assess 30-day readmissions of endoscopic retrograde cholangiopancreatography (ERCP) in the United States.

Methods: The National Readmission Database was utilized from 2016 to 2020 to identify 30-day readmissions of ERCP. Hospitalization characteristics and outcomes were compared between index hospitalizations and readmissions.

Article Synopsis
  • The study aimed to determine how body mass index (BMI) affects hospitalizations for Acute Pancreatitis (AP) in the US from 2016-2019.
  • Data showed that the majority of AP hospitalizations were among obese patients, who had younger ages but fewer complications compared to those of normal weight.
  • Normal weight patients had higher rates of mortality and complications, such as pancreatic necrosis and pseudocysts, despite a longer hospital stay and higher healthcare costs compared to obese patients.

Background: A common cause of mild thrombocytopenia is chronic liver disease, the most common etiology being metabolic dysfunction-associated steatotic liver disease (MASLD). Mild thrombocytopenia is a well-defined, independent marker of hepatic fibrosis in patients with chronic liver disease. Currently, there is a paucity of information available to characterize perioperative risk in patients with MASLD; therefore, the characterization of perioperative morbidity is paramount.

Background: The management of patients with chronic hepatitis B (CHB) is complex and spans multiple medical specialties. As a result of this complexity, patients with CHB often do not receive adequate monitoring including hepatocellular carcinoma (HCC) surveillance with abdominal ultrasonography. Previous studies have identified multiple factors associated with decreased HCC surveillance.

Background: There is limited evidence on the comparative diagnostic performance of endoscopic tissue sampling techniques for subepithelial lesions. We performed a systematic review with network meta-analysis to compare these techniques.

Methods: A systematic literature review was conducted for randomized controlled trials (RCTs) comparing the sample adequacy and diagnostic accuracy of bite-on-bite biopsy, mucosal incision-assisted biopsy (MIAB), endoscopic ultrasound-guided fine-needle aspiration (EUS-FNA), and EUS-guided fine-needle biopsy (FNB).

Article Synopsis
  • - The study examines trends and outcomes related to acute pancreatitis (AP) in liver transplant (LT) patients in the US from 2007 to 2019, noting an increase in LT hospitalizations with AP from 305 to 610.
  • - It highlights demographic changes, showing an increase in Hispanic and Asian LT patients with AP while the percentage of Black patients decreased, alongside a growing comorbidity burden among these patients.
  • - Despite the rise in hospitalizations and complications like sepsis and acute kidney failure, there were no significant changes in inpatient mortality, length of stay, or total healthcare charges during the timeframe studied.

Article Synopsis
  • The study conducts a systematic review and meta-analysis to evaluate the safety and effectiveness of a new procedure called endoscopic ultrasound-directed transgastric endoscopic retrograde cholangiopancreatography (EDGE) in patients with Roux-en-Y gastric bypass, comparing it to traditional methods (LA-ERCP and E-ERCP).
  • Results from 16 studies involving 470 patients show that EDGE has a high technical success rate of 96% and clinical success rate of 91%, but a 17% rate of adverse events was also reported.
  • The effectiveness of EDGE was found to be comparable to LA-ERCP, and potentially superior to E-ERCP, raising questions about its

We describe the risk factors for the development, timing, and severity of doxycycline induced acute pancreatitis (DIAP) in two case reports and a review of prior published cases, to better understand DIAP. Clinicians must maintain a high level of suspicion for DIAP in patients presenting with acute pancreatitis, while on doxycycline therapy. The latency and severity of DIAP are variable, making diagnosis challenging.

Barrett's esophagus (BE) is a precursor condition to esophageal adenocarcinoma (EAC), resulting in transformation of the squamous epithelium of distal esophagus to columnar-lined epithelium with intestinal metaplasia (IM). Liquid nitrogen spray cryotherapy (LNSC) is a non-contact method of BE eradication and has been used both as primary and salvage therapy. We conducted a systematic review and meta-analysis to assess the safety and efficacy of LNSC.

Background: Spontaneous bacterial empyema (SBE) occurs when a hepatic hydrothorax becomes infected and runs a course similar to spontaneous bacterial peritonitis (SBP). It remains underdiagnosed as patients with cirrhosis do not routinely undergo diagnostic thoracentesis. Current understanding is limited by small cohorts, while studies reporting its association with ascites/SBP are conflicting.
