Portal hypertension in children: High-risk varices, primary prophylaxis and consequences of bleeding.

J Hepatol

February 2017

Hépatologie Pédiatrique and Centre de Référence National de l'Atrésie des Voies Biliaires, Hôpital Bicêtre, AP-HP and Université Paris-Sud 11, Le Kremlin-Bicêtre 94275, France.

Background & Aims: Primary prophylaxis of bleeding is debated for children with portal hypertension because of the limited number of studies on its safety and efficacy, the lack of a known endoscopic pattern carrying a high-risk of bleeding for all causes, and the assumption that the mortality of a first bleed is low. We report our experience with these issues.

Methods: From 1989 to 2014, we managed 1300 children with portal hypertension.

Avoidable Emergency Visits for Acute Asthma in Children: Prevalence and Risk Factors.

Pediatr Allergy Immunol Pulmonol

September 2016

APHP, Hôpital Robert Debré, Service de Physiologie Pédiatrique, Paris, France.

Emergency department (ED) visits for asthma exacerbation have not become less frequent, essentially because the self-management of mild-to-moderate asthma exacerbations by children and their families remains sub-optimal. The objective of our study was to assess the proportion of visits to EDs for asthma exacerbation that were potentially avoidable and their risk factors [such as no Written Asthma Action Plan (WAAP)]. We conducted an 8-month multicenter study in 6 French pediatric EDs.

Smoking only partly explains the higher lung cancer incidence observed among socially deprived people. Occupational exposures may account for part of these inequalities, but this issue has been little investigated. We investigated the extent to which smoking and occupational exposures to asbestos, silica and diesel motor exhaust mediated the association between education and lung cancer incidence in men.

Urinary metabolomic profiling of asthmatics can be related to clinical characteristics.

Allergy

September 2016

QOPNA and Departamento de Química, Universidade de Aveiro, Aveiro, Portugal.

Metabolomics has been increasingly explored to achieve an improved understanding of asthma. In the current observational and exploratory study, the first to have examined the relationship between oxidative stress extension, eosinophilic inflammation, and disease severity in asthmatic patients, metabolomics (using target aliphatic aldehydes and alkanes) was carried out using solid-phase microextraction (SPME) followed by a comprehensive two-dimensional gas chromatography coupled to mass spectrometry with a high-resolution time-of-flight analyzer (GC×GC-ToFMS). We were able to demonstrate that metabolomics can give valuable insights into asthma mechanisms once lipidic peroxidation assessed by urinary metabolomics is related to the clinical characteristics of nonobese asthmatics, such as disease severity, lung function, and eosinophilic inflammation.

July 2016

Biostatistics Unit, Gustave Roussy, Villejuif, France; CESP Centre for Research in Epidemiology and Population Health, INSERM U1018, Team-2, Paris-Sud University, Villejuif, France. Electronic address:

Article Synopsis
  • The study evaluated the effectiveness of postoperative radiotherapy (PORT) in patients with localized Ewing sarcoma who had a good response to pre-surgery chemotherapy, focusing on its impact on preventing local relapse after surgery.
  • Out of 599 patients involved in the EE99-R1 trial, 142 received PORT, and results showed a significant reduction in local relapse rates for those treated with surgery plus PORT compared to surgery alone.
  • The findings suggest that PORT enhances local control of the disease, particularly in larger tumors or those with complete necrosis, leading to a recommendation for its use in cases of incomplete tumor removal, although further research is needed to evaluate the associated risks.

Objective: Observational studies show an association between ferritin and type 2 diabetes (T2D), suggesting a role of high iron stores in T2D development. However, ferritin is influenced by factors other than iron stores, which is less the case for other biomarkers of iron metabolism. We investigated associations of ferritin, transferrin saturation (TSAT), serum iron, and transferrin with T2D incidence to clarify the role of iron in the pathogenesis of T2D.
