Improving access to care for underserved communities through telemedicine.

JAAPA

September 2023

Jennifer D. Guthrie practices at Grace Health in Battle Creek, Mich. Jennifer A. Snyder is an associate dean and professor in the PA program at Butler University in Indianapolis, Ind. The authors have disclosed no potential conflicts of interest, financial or otherwise.

Telemedicine use expanded rapidly during the COVID-19 pandemic, giving patients access to quality care while reducing the spread of infection. Although beneficial changes were made to reimbursement and privacy guidelines to increase the ease of telemedicine for clinicians, the lack of digital devices, Internet, digital knowledge, and trust in this method of delivery are potential barriers to telemedicine for healthcare visits, and may have widened the care gap for underserved patients. To ensure that patients of all socioeconomic levels have access to telemedicine requires education, expanding broadband Internet access across the United States, and offering free or reduced Internet services to patients in need.

Article Synopsis
  • Human-driven environmental changes significantly shape wildlife diversity in urban areas, influenced by local factors like landscape patterns and species traits.
  • Research across 20 North American cities revealed that urbanization, particularly in warmer and less vegetated regions, negatively impacts mammal species occupancy and community composition.
  • Larger-bodied mammal species faced the most severe declines due to urbanization, indicating that climate change could exacerbate these effects, and highlighting the need for effective conservation strategies.

Background: This study explores factors related to physician assistant (PA) education program directors' (PD) consideration to leave their leadership role. This is important to better understand, with the need for additional PA education PDs as the number of PA programs grows in addition to current PA program leaders considering leaving their PD role.

Methods: Data from the 2019 Physician Assistant Education Association (PAEA) Faculty and Directors Survey were used to analyze factors related to consideration for leaving the PD position.

Phloem is a critical tissue for transport of photosynthates and extracellular signals in vascular plants. However, it also represents an ideal environment for pathogens seeking access to valuable host nutrients. Although many vascular pathogens induce economically relevant crop damage, there is still little known about the mechanisms by which immune signaling operates through the phloem.

Females spend a third to half of their life in menopause, and the number of US females in menopause is growing. A high percentage of postmenopausal females experience bothersome, sometimes debilitating genitourinary symptoms, which can affect quality of life. The genitourinary syndrome of menopause (GSM) describes the condition previously referred to as vulvovaginal atrophy, atrophic vaginitis, or urogenital atrophy.

Chronic obstructive pulmonary disease therapy management can be complex given the various types of inhaler devices available, even within a therapeutic class. Appropriateness of an inhaler relies on many patient-specific factors. Senior care pharmacists can positively impact patient care by providing appropriate inhaler assessment as well as education on inhaler technique to ensure maximal benefit from therapeutic treatment choices.

Despite efforts in recent years, including in policy and research, to address health disparities in the United States, many of those disparities continue to fester in marginalized racial/ethnic populations. Understanding sleep health disparities is critical in understanding the health and wellness of these groups. Using obstructive sleep apnea (OSA) in Black populations as a focus, this paper presents the role of race and ethnicity in the clinical understanding of sleep health-related issues by medical practitioners and the implications of the lack of clear policies or best practices to guide medical practitioners' attempts to meet sleep-related needs of marginalized racial/ethnic populations.

Introduction: Pharmacists must be knowledgeable to care for all patients, including transgender and gender diverse individuals. Some institutions may have gaps in their pharmacy school curriculum specific to transgender contraceptive care. The current study evaluated and offered recommendations regarding the current state of transgender contraceptive care education within pharmacy curricula.

Neonatal brachial plexus injury (NBPI), a leading cause of pediatric upper limb paralysis, results in disabling and incurable muscle contractures that are driven by impaired longitudinal growth of denervated muscles. A rare form of NBPI, which maintains both afferent and sympathetic muscle innervation despite motor denervation, protects against contractures. We have previously ruled out a role for NRG/ErbB signaling, the predominant pathway governing antegrade afferent neuromuscular transmission, in modulating the formation of contractures.

Emergence, Tropism, Disease, and Treatment of Australian Bat Lyssavirus Infections in Humans.

Vector Borne Zoonotic Dis

September 2023

Department of Biological Sciences, Butler University, Indianapolis, Indiana, USA.

Australian bat lyssavirus (ABLV) is a negative-sense, single-stranded RNA rhabdovirus capable of causing fatal acute encephalitis in humans with similar pathogenesis to its closest serologic relative, rabies virus (RABV). In this review, we describe emergence and classification of ABLV, its known virology, reservoirs, and hosts, as well as both the pathogenesis and treatment approaches currently employed for presumed infections. ABLV was first identified in New South Wales, Australia in 1996 and emerged in humans months later in Queensland, Australia.

We performed four experiments to investigate whether people can perceive the length of a target object (a "fish") that is attached to a freely wielded object (the "fishing pole") by a length of string, and if so, whether this ability is grounded in the sensitivity of the touch system to invariant mechanical parameters that describe the forces and torques required to move the target object. In particular, we investigated sensitivity to mass, static moment, and rotational inertia-the forces required to keep an object from falling due to gravity, the torque required to keep an object from rotating due to gravity, and the torques required to actively rotate an object in different directions, respectively. We manipulated the length of the target object (Experiment 1), the mass of the target object (Experiment 2), and the mass distribution of the target object (Experiments 3 and 4).

Background: Pharmaceutical drug recall is a relentless issue that is composed of multidimensional criteria. The distinct criteria that contributed to drug recalls have been identified in previous literature; however, there is limited information regarding the causal relationships between each criterion. Highlighting key influential aspects and criteria of pharmaceutical drug recall is critical in addressing this ongoing issue and promoting patient safety.

J Physician Assist Educ

September 2023

Stephanie Neary, MPA, MMS, PA-C, is an assistant professor adjunct and director of didactic education, Department of General Internal Medicine, Yale University, New Haven, Connecticut.

Introduction: Mental illness and decreased well-being are pervasive throughout physician assistant (PA) training, and the accreditation standards require a wellness curriculum. The purpose of this study was to determine if a mandatory, multifaceted virtual wellness curriculum can mitigate the decline in well-being experienced by first-year PA students.

Methods: First-year PA students from 5 programs (n = 259) participated in a 16-week asynchronous virtual course.

LipN (Rv2970c) is a conserved mycobacterial serine hydrolase with regulated catalytic activity at the interface between active and dormant growth conditions. LipN also catalyzes the xenobiotic degradation of a tertiary ester substrate and contains multiple conserved motifs connected with the ability to catalyze the hydrolysis of difficult tertiary ester substrates.
