A number of laboratory studies have shown very low levels of archwire friction for self-ligating brackets when compared to conventional ligation methods. However, justifiable reservations have been expressed as to the in vivo relevance of these findings. This study was designed to compare treatment efficiency with conventional fully programmed brackets and Damon SL self-ligating brackets.

Periodontal disease result from mixed bacterial infections, in which both host resistance barriers and bacterial interactions are important. Approximately ten bacterial species are strongly implicated with various forms of periodontal disease, although species that cannot yet be cultivated are likely also to be relevant. New technologies have shown that pathogenic bacterial species are present in defined complexes within subgingival plaque, thus identifying specific targets for therapeutic intervention.

Threaded and cylindrical metallic endosseous implants are being used increasingly in cranio-facial, dental, and orthopedic surgery. There is a significant need for the development of quantitative noninvasive test methods to measure implant stability and success. Such success can be quantified by measurements of the stiffness and damping of an implant in the surrounding bone, and also the height of the marginal bone around an implant.

The impact of computers in dental education.

Prim Dent Care

April 1998

Division of Restorative Dentistry, Bristol Dental Hospital and School.

Computer technology is increasingly influencing many aspects of our lives and will continue to do so in the future. This paper discusses the impact of computers in dentistry; how their use is radically changing the way practices are run and the way under/postgraduate dental education is evolving, by incorporating computer-aided learning (CAL) material as an efficient and convenient educational tool. Greater computer literacy may result in a future where there is a shift away from traditional centres of learning to computer conferencing and distance learning.

This study evaluated the inter-relationship between five aesthetic factors: the form of the face, tooth and arch, maxillary anterior tooth arrangement and palatal contour. An assessment was made to determine if correlation was sufficient to recommend their use as reliable aesthetic factors for the selection of suitable tooth moulds for edentulous and partially edentulous patients. This would enable the clinician to select a tooth mould at the chairside, based on an evaluation of the individual's aesthetic factors where, for example, the maxillary incisors should have the same outline shape as the individual's face.

Missing teeth, whether, congenitally absent or lost through trauma or caries may lead to reduced friction, poor aesthetics or distress to the patient. A variety of methods have therefore been developed to replace the missing dentition with bridgework or dentures. The method chosen for replacement should be tailored towards the individual patient as far as possible.

Granulomatous disorders of the oral mucosa.

Semin Diagn Pathol

May 1996

Department of Oral and Dental Science, Bristol Dental Hospital and School, England.

A wide variety of granulomatous disorders can involve the orofacial tissues. As in other sites, appropriate clinicopathologic correlations are needed to reach a definitive diagnosis. The clinical and pathologic features of some of the more common oral granulomatous disorders are considered, including idiopathic conditions, foreign body lesions, and infectious lesions.

This study examined the effect of transforming growth factor beta-1 (TGF-beta 1) on c-myc, RB1, junB and p53 expression together with pRb phosphorylation, in carcinoma-derived and normal human oral keratinocytes with a range of inhibitory responses to this ligand. Amplification of c-myc was observed in eight of eight tumour-derived cell lines and resulted in corresponding mRNA expression. The down-regulation of c-myc expression by TGF-beta 1 predominantly reflected growth inhibition by TGF-beta 1, but in two of eight tumour-derived cell lines which were partially responsive to TGF-beta 1 c-myc expression was unaltered by this ligand.

While rising levels of dental caries were primarily responsible for the growth in dental schools in developed countries, the recent decline in caries prevalence has not resulted in a major decline in dental education. Indeed, the reduction in caries prevalence has had very little influence upon a group of major changes that arguably should have had a marked effect upon dental education in recent years, but which have failed so to do. These include a quantum leap in applying the scientific basis of caries management, focused down to the teaching of minimally invasive restorative care within a context of risk assessment and maximum non-invasive care where appropriate.

Oral melanotic macules that develop after radiation therapy.

Oral Surg Oral Med Oral Pathol

April 1994

Department of Oral Medicine, Pathology and Microbiology, Bristol Dental Hospital and School, U.K.

External beam radiotherapy treatment of malignant conditions in the head and neck can give rise to several adverse oral effects if the oral or salivary tissues are within the field of beam. We report on a patient with widespread oral melanotic hyperpigmentation that developed after a course of radiotherapy for metastatic carcinoma in the cervical lymph nodes. As no other local or systemic cause was evident it is possible this abnormal hyperpigmentation was a result of the radiotherapy.

It is known that high levels of residual monomer have a deleterious effect on the properties of denture base polymers and may cause irritation or hypersensitivity of the oral tissues. In the present study variations in determination methodology and their influence on residual monomer levels are evaluated. The results demonstrate that filtration of the sample, sealed storage and incorporation of an appropriate volume of internal standard enhanced the accuracy of the technique.

New patterns of oral mucosal disease and indeed new disorders have been emerging over the past decade. Although infection with human immunodeficiency viruses (HIV) is having the most profound impact, there are also major changes in oral health and the standard of health care required, because of other new disorders and medical care. The increase in tissue transplantation, with the concomitant use of immunosuppression is resulting in a range of oral problems.
