Introduction: Sleep is crucial for memory consolidation and the clearance of toxic proteins associated with Alzheimer's disease (AD). We examined the association between sleep characteristics and imaging biomarkers of early amyloid beta (Aβ) and tau pathology as well as neurodegeneration in brain regions known to be affected in the incipient stages of AD.

Methods: Thirty-nine cognitively unimpaired (CU) participants of the Harvard Aging Brain Study underwent at-home polysomnography as well as tau positron emission tomography (flortaucipir-PET), amyloid PET (Pittsburgh compound B [PiB]-PET), and magnetic resonance imaging-derived assessment of cortical thickness (CT).

Peripheral neuropathy is a well-described side effect of certain chemotherapeutic agents, including taxanes, and often improves in the weeks following treatment. The recurrence of motor and sensory neuropathies after anaesthesia has not yet been described to our knowledge. We present a case of transient recurrence of chemotherapy-induced peripheral neuropathy following general anaesthesia.

Article Synopsis
  • This study explores a new method for accurately calculating fractional flow reserve (FFR) using coronary angiography to better identify patients who need intervention.
  • The research involved a comparison of this new FFR framework against traditional wire-based FFR in 160 patients, revealing strong diagnostic performance metrics, such as a sensitivity of 79.3% and specificity of 95.4%.
  • A key finding was the identification of longitudinal vorticity as a significant hemodynamic marker indicating the risk of major adverse cardiac events in patients who haven't undergone revascularization.

Myeloproliferative neoplasms (MPNs) are associated with immune dysregulation and increased susceptibility to infection, emphasizing the importance of vaccination for patients. This pilot study evaluated immune responses to influenza vaccination in MPN patients compared with healthy donors using mass cytometry and serology. We observed diminished CXCR5+ B-cell, CXCR3+ T-cell, activated CD127+ memory T-cell subsets, and a trend toward lower hemagglutinin inhibition titer in MPN patients.

Psychological interventions delivered by non-specialist providers have shown mixed results for treating maternal depression. mHealth solutions hold the possibility for unobtrusive behavioural data collection to identify challenges and reinforce change in psychological interventions. We conducted a proof-of-concept study using passive sensing integrated into a depression intervention delivered by non-specialists to twenty-four adolescents and young mothers (30% 15-17 years old; 70% 18-25 years old) with infants (< 12 months old) in rural Nepal.

Article Synopsis
  • The study investigates the link between COPD exacerbations and the risk of cardiovascular disease (CVD), using data from the long-term COPDGene study, which followed patients for up to 15 years.
  • It found that patients who frequently experience COPD exacerbations have a higher risk of developing CVD, regardless of their CVD status at the start of the study.
  • The results suggest that frequent exacerbators may require closer monitoring and management to address potential cardiovascular risks.

Article Synopsis
  • MR imaging is essential for assessing tumor burden and changes in pediatric neuro-oncology, but accurately identifying tumor components can be complex and subjective.
  • Current response assessment guidelines do not fully address challenges such as differentiating tumor types and understanding the impact of imaging inconsistencies, leading to variability in evaluations by different observers.
  • The paper emphasizes the need for standardized criteria to improve the identification of tumor subregions and suggests that automated segmentation using AI could enhance objective assessments, aiming for more reliable metrics in clinical studies.

Background: Global emergency medicine (GEM) is situated at the intersection of global health and emergency medicine (EM), which is built upon a history of colonial systems and institutions that continue to reinforce inequities between high-income countries (HICs) and low- and middle-income countries (LMICs) today. These power imbalances yield disparities in GEM practice, research, and education.

Approach: The Global Emergency Medicine Academy (GEMA) of the Society for Academic Emergency Medicine formed the Decolonizing GEM Working Group in 2020, which now includes over 100 worldwide members.

Article Synopsis
  • Accurate quantification of sodium intake via self-reported surveys has been problematic, prompting researchers to use machine-learning (ML) algorithms to better predict urinary sodium excretion based on questionnaire data.
  • The study involved 3,454 participants from major health studies and found ML predictions of sodium excretion were more reliable than traditional food frequency questionnaires, showing stronger correlations and better calibration.
  • However, while ML improved predictions overall, it was still heavily influenced by body size and did not significantly reduce measurement errors related to disease outcomes.

Article Synopsis
  • - This study compares self-reported cardiovascular conditions (like hypertension and stroke) among individuals with moderate to severe traumatic brain injury (TBI) to a matched control group without TBI.
  • - Researchers found that those with TBI were more likely to report high blood pressure and stroke, but less likely to report heart failure and heart attacks, particularly in individuals over 50 years old.
  • - The results suggest that TBI survivors have varying rates of cardiovascular issues compared to uninjured adults, possibly influenced by factors such as age and survival bias post-injury.

Despite the growing use of point of care ultrasound (POCUS) in contemporary medical practice and the existence of clinical guidelines addressing its specific applications, there remains a lack of standardization and agreement on optimal practices for several areas of POCUS use. The Society of Point of Care Ultrasound (SPOCUS) formed a working group in 2022 to establish a set of recommended best practices for POCUS, applicable to clinicians regardless of their training, specialty, resource setting, or scope of practice. Using a three-round modified Delphi process, a multi-disciplinary panel of 22 POCUS experts based in the United States reached consensus on 57 statements in domains including: (1) The definition and clinical role of POCUS; (2) Training pathways; (3) Credentialing; (4) Cleaning and maintenance of POCUS devices; (5) Consent and education; (6) Security, storage, and sharing of POCUS studies; (7) Uploading, archiving, and reviewing POCUS studies; and (8) Documenting POCUS studies.
