In periodontal treatment the importance of self-performed plaque control is frequently acknowledged but often underestimated. This article reviews the evidence relating to the effects of good oral hygiene on both the initiation and progression of gingivitis and periodontitis, and on treatment outcomes. Recent evidence suggests that an optimal level of supragingival plaque control can have profound effects on the subgingival microflora and this, together with evidence indicating that root surface microbial contaminants are superficially located within the biofilm, has significant therapeutic implications.

The tunnel or 'internal' restoration to restore an approximal caries lesion was first described in 1984. The technique allows the operator to conserve the important external anatomical features of the tooth, thereby saving considerable clinical time while leaving relatively little of the material externalized. The development of a new generation of glass-ionomer materials with higher flexural and compressive strengths appears to satisfy the main objective of carious dentine replacement facilitated by a 'keyhole' internal preparation.

The Transmandibular Implant is a transosteal implant developed to help improve masticatory function in patients with severe mandibular atrophy. It is a functional implant with a rigid box frame design which allows acceptance of masticatory loads in mandibles with minimal bone height. The implant is fabricated of a gold alloy and is placed between the mental foramina from a submental approach.
