Recent advances in the treatment of early stage breast cancer give women a choice of treatments. Prospective and retrospective studies demonstrate the comparable efficacy of treatment with a modified radical mastectomy and treatment with conservative surgery and primary radiation. While the number of women with early stage breast cancer choosing the latter treatment continues to increase, there is a paucity of information in the nursing literature assessing the informational and psychosocial needs of this group.

Predictions made more than a quarter of a century ago regarding excessive use of cesarean delivery for cases of dystocia have been realized. Breech presentation, correctable dystocia, and twin gestation are increasingly being delivered by the abdominal route. Recent studies have shown that neonatal morbidity is not inherently improved by cesarean birth, and in fact have reported comparable results with vaginal operative procedures in properly selected and managed cases.

Community physicians may play an increasing role in treating patients with acquired immunodeficiency syndrome (AIDS) because of the shift away from inpatient care. At a community hospital in New York, NY, we surveyed 230 attending physicians in a department of medicine to determine their attitudes toward the care of patients with AIDS. Factor analysis produced three clusters of attitudes termed antipathy, liability, and isolation.

The presence of melanin pigment in the cytoplasm of breast carcinoma cells has been reported. Fine-needle aspiration of a solitary lung lesion in a woman who had undergone mastectomy for ductal carcinoma revealed malignant cells consistent with the primary mammary tumor; many of these tumor cells contained melanin pigment.

Benign fibrous histiocytomas of bone are unusual neoplasms that often are confused with metaphyseal fibrous defects. Although the two lesions have overlapping microscopic characteristics, they differ in their clinicopathologic presentations. This report describes the clinicopathologic features of the first reported case of benign fibrous histiocytoma involving the maxilla and the fourth description of this tumor in the jawbones.

A total of 3970 cases of Kaposi's sarcoma (KS) associated with the acquired immunodeficiency syndrome had been reported to the Centers for Disease Control by the end of 1986. The prevalence of oral KS in patients with KS of the skin varies, reaching a maximum of 44% in one published study. We present a retrospective clinicopathologic analysis of 23 previously unreported cases of oral KS in male homosexual and bisexual patients from the New York metropolitan area.

This case report describes a 45-year-old Oriental male with spontaneous oral hemorrhage and submucosal bullae of 4 days' duration. Oral pigmentation due to minocycline hydrochloride (Minocin) therapy is an unusual and infrequently reported side effect of the drug. This report describes a patient on long-term minocycline therapy who presented with gingival pigmentation. Upon surgical exploration, the gingiva was noted to be normal in color, but the underlying bone was deeply pigmented.

Ring butterfly fragment was described after it was noted to pose problems during insertion of flexible intramedullary nails. This fragment contains the entire circumference of the medullary canal or a substantial part of it. Once recognized preoperatively the fragment is reduced, providing a stable fracture configuration for fixation with flexible intramedullary nails.

Fungal intravascular graft infections are rare. In addition to our case, which forms the basis of this article, only 13 documented instances could be found in the literature in the 20-year period from 1966 to 1986. Three of these cases (21%) had both fungus and bacteria grown in culture.
