A woman in her early 90s presented to the acute surgical take with a 3-day history of worsening reflux, vomiting, epigastric pain and constipation. Subsequent imaging demonstrated two large, impacted gallstones in the pylorus and proximal jejunum secondary to a cholecystoduodenal fistula. A diagnosis of Bouveret syndrome was made, and endoscopic attempts to break down and remove the stones were unsuccessful.

Introduction: Platelet-rich fibrin (PRF) is being increasingly utilized in surgical procedures due to various improvements in clinical outcomes. More recently, a heating process to denature albumin in the platelet poor plasma (PPP) layer has been shown to extend the resorption time of PRF from a typical 2-week period to 4-6 months. Because of its > 4 month resorption properties, extended PRF (e-PRF) membranes have been used in dentistry as an alternative to collagen membranes in alveolar ridge preservations, ridge augmentations, soft tissue grafting, and as a barrier membrane in lateral sinus grafting procedures.

Here we present 4 patient cases describing common challenges in anticoagulation management: inappropriate dosing of direct oral anticoagulants (DOACs), the diagnosis and management of heparin-induced thrombocytopenia (HIT), periprocedural anticoagulation management, and heavy menstrual bleeding on anticoagulation.

Study Objective: The primary objective of this study was to describe vaginal hysterectomy practice patterns as well as facilitators and inhibitors to performing vaginal hysterectomy among gynecologic surgeons. Secondary objectives were to describe facilitators and inhibitors to the teaching and training of vaginal hysterectomy.
Design: Quantitative analysis of an online survey and qualitative analyses of the one-on-one interviews on gynecologic surgeons was conducted.
