Schwannomas commonly occur in the head and neck region but are rarely seen in the gastrointestinal tract; the stomach and small intestine are the most commonly involved sites. These tumors are usually misdiagnosed as gastrointestinal stromal tumors (GISTs) before histopathological confirmation due to radiological similarity. GI schwannomas show positivity for S100 protein and vimentin but are negative for CD 117 and CD 34, which helps in differentiating the tumor from GISTs.

Diffuse Type Pancreatic Ductal Adenocarcinoma: The Linitis Plastica Type of the Pancreas.

J Med Cases

December 2024

Department of General Surgery, Rambam Health Care Campus, Haifa, Israel.

Pancreatic malignant tumors are diverse and characterized by aggressive nature with high mortality rates. Pancreatic ductal adenocarcinoma (PDAC) is the most common type of pancreatic malignant tumors and accounts for approximately 90% of pancreatic malignancies. A subtype of PDAC, diffuse type PDAC (DTP), is a very rare and underreported tumor, known for its aggressive traits.

Article Synopsis
  • - Papillary thyroid carcinoma typically presents as a painless, irregular mass in the thyroid region, often seen in middle-aged individuals; however, it may show varied symptoms like hoarseness and difficulty swallowing.
  • - A case study of a 63-year-old man showed an unusual presentation of this cancer, initially mistaken for lymphoma due to the absence of thyroid swelling and presence of multiple swollen lymph nodes in the neck.
  • - The case emphasizes the importance of fine-needle aspiration over excision biopsies in diagnosing neck swellings, as misdiagnosis or improper biopsy can lead to complications and worsen the prognosis for conditions like papillary thyroid carcinoma.

Article Synopsis
  • The study investigates how body measurements and ultrasound features affect the choice between fine-needle aspiration biopsy (FNAB) and fine-needle non-aspiration biopsy (FNNAB) for diagnosing thyroid nodules, aiming for better management and cancer detection.
  • A total of 188 cases with 225 nodules were analyzed, comparing the diagnostic effectiveness of FNAB and FNNAB based on criteria like nodule shape, size, vascularity, and patient obesity.
  • Results showed that FNAB had a higher diagnostic success rate than FNNAB, especially in specific nodule characteristics and certain patient demographics, indicating its superiority in various clinical situations.

Article Synopsis
  • Papillary thyroid cancer (PTC) is the most common type of thyroid cancer, with this case report focusing on two instances of occult thyroid carcinoma (OTC), where cancer spreads to lymph nodes without a detectable primary tumor in the thyroid.
  • The first case involves a 67-year-old female who had swelling in her neck, leading to the discovery of PTC metastasis in lymph nodes after surgeries, while the second case features a 79-year-old male with PTC spreading to his parotid gland, both lacking any primary tumor in the thyroid on final analysis.
  • OTC is rare, and its diagnosis can be complicated, so the authors emphasize the need for more research to create standardized diagnostic and treatment protocols for this condition

Background Lateral neck masses have always been difficult to diagnose without proposing a differential diagnosis. Fine-needle aspiration (FNA) was proposed to be a cost-effective method and less invasive than a tru-cut biopsy and may provide a provisional diagnosis in relation to cytopathology. FNA has also been shown to improve the diagnosis of neck masses such as cervical lymphadenopathy, neck cysts, and parotid masses, whether malignant or benign.

Technetium-99m sestamibi single-photon emission computed tomography/computed tomography (Tc-sestamibi SPECT/CT) is a mainstay of the pre-operative localization of parathyroid lesions. We report here the case of a 30 year-old woman with a fortuitously discovered 2 cm cervical mass for which a parathyroid origin was originally suspected due to its retro-thyroidal localization and a personal history of nephrolithiasis. Normal serum calcium and parathyroid hormone (PTH) levels excluded primary hyperparathyroidism, raising suspicion of a non-functional parathyroid adenoma, and SPECT/CT imaging showed that the mass was Tc-sestamibi-avid.
