Subscapularis pathology is difficult to diagnose, in part because of decreased sensitivity and accuracy in identifying tears with magnetic resonance imaging (MRI) when compared to other cuff tendons. Artificial intelligence evaluation of patient physical examination and MRI data using a machine learning model shows that arthroscopically confirmed partial- or full-thickness subscapularis tears are highly associated with abnormal subscapularis tendon length, long head of the biceps tears, and subscapularis fatty atrophy, and on physical examination, with weakness with internal rotation and positive lift-off, belly press, and bear hug tests. Today, physicians may use machine learning as a tool, but this model may not currently be sufficient to drastically change practice.

Although the superior portion of the shoulder joint capsule is a clearly distinct structure, its ability to prevent vertical humeral head translation in irreparable rotator cuff tears remains controversial. Distal biceps tendon (DBT) is a relatively rare injury mainly occurring in middle-aged men while in eccentric biceps muscle contraction. Clinical appearance with proximal avulsion of the muscle and specific clinical tests are most of the time sufficient for diagnosing DBT, but if needed ultrasonography and MRI, most often in FABS view, can be used to ensure diagnosis of DBT and partial DBT. Surgical anatomical reinsertion has shown to be a successful method of treatment, although conservative treatment can be initiated in older patients.

Background: We report two cases of biceps brachii and brachialis paralysis due to musculocutaneous nerve injury in which elbow joint flexion was reconstructed using rotational transfer of the latissimus dorsi muscle with sutures to the radial and ulnar tuberosities, thereby enabling flexion by simultaneous activation of the humeroradial and humeroulnar joints. In cases of associated brachialis paralysis, weaker flexion strength can be expected when the forearm is in a pronated position than when it is in a supinated state. Background: It has been suggested that hypertrophy of the radial tuberosity may result in impingement leading to either a lesion of the distal biceps tendon or rotational impairment. Two previous studies on hypertrophy of the radial tuberosity had contradictory results and did not examine the distance between the radius and ulna: the radioulnar window.
