Article Synopsis
  • The study examines the effects of low vs. high vancomycin trough levels on clinical outcomes in adults with sepsis or gram-positive infections, moving away from the previously recommended dosage guidelines.
  • A systematic review and meta-analysis of 14 studies involving over 5,200 patients showed no significant links between vancomycin levels and various clinical outcomes like microbial clearance and ICU stay.
  • However, lower trough levels appeared to trend towards reduced treatment failure and were significantly associated with lower all-cause mortality rates.

Background: Breast cancer screening with dynamic contrast-enhanced MRI (DCE-MRI) is recommended for high-risk women but has limitations, including variable specificity and difficulty in distinguishing cancerous (CL) and high-risk benign lesions (HRBL) from average-risk benign lesions (ARBL). Complementary non-invasive imaging techniques would be useful to improve specificity.

Purpose: To evaluate the performance of a previously-developed breast-specific diffusion-weighted MRI (DW-MRI) model (BS-RSI3C) to improve discrimination between CL, HRBL, and ARBL in an enriched screening population.

Article Synopsis
  • Incomplete revascularization (ICR) after percutaneous coronary intervention (PCI) can lead to higher risks of mortality and morbidity, and this study aimed to compare the effects of ICR in different arteries (LAD vs. RCA/LCX) and in patients with chronic total occlusion (CTO) versus those without.
  • The research included 2,651 patients from the RIVER-PCI trial and found that about 66.5% had ICR involving the left anterior descending artery, and follow-up showed similar rates of hospitalization for ischemia regardless of the artery affected.
  • However, patients with a CTO experienced increased hospitalizations for ischemia, heart failure, and myocardial infarction compared to those without CTO

Health profession educators readily identify with the goal of fostering healthcare providers who are critical thinkers focused on quality patient care. In the following paper, we aim to delve into critical thinking at the team level and help educators begin the process of creating a shared mental model focusing on cognition to identify gaps and opportunities for growth in their trainees. We will distinguish between microcognition (an individual's own critical thinking process in a controlled environment), macrocognition (critical thinking process in a real-world environment), and team cognition (the interaction and relationship among team members to augment macrocognition).

Background And Aims: There are a limited number of pharmacologic therapies for coronary artery disease, and few rodent models of occlusive coronary atherosclerosis and consequent myocardial infarction with which one can rapidly test new therapeutic approaches. Here, we characterize a novel, fertile, and easy-to-use HDL receptor (SR-B1)-based model of atherogenic diet-inducible, fatal coronary atherosclerosis, the SR-B1ΔCT/LDLR KO mouse. Additionally, we test intramyocardial injection of Stromal Cell-Derived Factor-1 alpha (SDF-1α), a potent angiogenic cytokine, as a possible therapy to rescue cardiac function in this mouse.

December 2023

Brookdale Department of Geriatrics and Palliative Medicine, Mount Sinai Beth Israel Hospital, New York, New York, USA.

Palliative care social workers are trained to identify how trauma manifests over the course of serious and chronic illness. This expertise can guide patients and team members to ensure a trauma-informed experience of care. This paper identifies the growth of outpatient palliative care clinics as an opportunity for palliative care social workers to emphasize this clinical skill set and assume a leadership role in the implementation of trauma-informed practices early in the trajectory of serious illness.

Coexisting serious illness and posttraumatic stress place hospitalized individuals at risk for complex pain, anxiety, and retraumatization. Hospital palliative care social workers increasingly recognize the value of trauma-informed care (TIC) for reducing harm in the inpatient setting. Despite this recognition, there is limited operationalization of TIC principles for inpatient interventions.
