Article Synopsis
  • A study examined the effectiveness of repeated Dix-Hallpike tests (DHt) versus the Epley maneuver (EM) in treating patients with posterior canal benign paroxysmal positional vertigo (pc-BPPV), focusing on "BPPV fatigue," where nystagmus decreases with repeated testing.
  • In a multicenter, randomized controlled trial involving 180 patients, both groups showed similar success rates in eliminating positional nystagmus after one week, with 50% for EM and 47.8% for repeated DHt.
  • The results indicated that repeated DHt is as effective as EM in treating pc-BPPV, suggesting that disintegrated otoconial debris might provide a therapeutic effect

Article Synopsis
  • - The study investigates whether combining two PD-L1 assessment methods (SP142 and 22C3) can better predict responses to immunotherapy in patients with advanced non-small cell lung cancer (NSCLC).
  • - Researchers analyzed tissue samples from 288 patients treated with immune-checkpoint inhibitors to compare results from the two PD-L1 tests, focusing on their impact on treatment outcomes like objective response rate (ORR) and overall survival (OS).
  • - Results indicated that patients with higher PD-L1 levels (22C3 ≥50%) displayed significantly better treatment responses with SP142 assays compared to those with lower levels, suggesting that SP142 could be a valuable prognostic factor.

Prognostic value of PD-L1 expression in recurrent renal cell carcinoma after nephrectomy: a secondary analysis of the ARCHERY study.

Int J Clin Oncol

February 2023

Department of Urology, Graduate School of Medical Sciences, Kyushu University, 3-1-1 Maidashi, Higashi-Ku, Fukuoka, 812-8582, Japan.

Background: Nephrectomy is a curative treatment for localized renal cell carcinoma (RCC), but patients with poor prognostic features may experience relapse. Understanding the prognostic impact of programmed death-ligand 1 (PD-L1) expression in patients who underwent nephrectomy for RCC may aid in future development of adjuvant therapy.

Methods: Of 770 surgical specimens collected from Japanese patients enrolled in the ARCHERY study, only samples obtained from patients with recurrent RCC after nephrectomy were examined for this secondary analysis.

Programmed cell death protein-1 (PD1), PD1 ligand 1 (PD-L1), and human leukocyte antigen (HLA) class I molecule play pivotal roles in T cell-induced anti-tumor immunity; however, the clinical impact of these parameters in resected malignant pleural mesothelioma (MPM) cases is unknown. We immunohistochemically evaluated the tumor infiltrated lymphocytes (TILs), PD1/PD-L1 axis, and expression of HLA class I in resected specimens from 58 patients with MPM who underwent extra-pleural pneumonectomy (EPP). Higher infiltration of CD3-TIL, CD8-TIL, and PD1-TIL, loss of HLA class I, and overexpression of PD-L1 by tumor cells (PD-L1 TC) or immune cells (PD-L1 IC) were observed in 34 (58.
