Digoxin is associated with worse outcomes in patients with heart failure with reduced ejection fraction.

ESC Heart Fail

February 2020

Department of Cardiology, Zhongshan Hospital, Fudan University, Shanghai Institute of Cardiovascular Diseases, 180 Fenglin Road, Shanghai, 200032, China.

Article Synopsis
  • The study investigates how digoxin affects patients with heart failure with reduced ejection fraction (HFrEF) in China, focusing on its interaction with atrial fibrillation and guideline-recommended treatments.
  • A total of 882 patients were analyzed, with findings showing that those using digoxin had significantly higher all-cause mortality and re-hospitalization rates compared to those not using it, despite similar heart failure-specific outcomes.
  • The conclusions indicate that digoxin is linked to an increased risk of overall mortality and re-hospitalization in HFrEF patients, regardless of other treatments or heart rhythms.

Background: Damage-associated molecular patterns (DAMPs) generated by major surgery can induce global inflammation response and may degrade the vascular endothelial glycocalyx layer (EGL); in turn, the resulting EGL fragments can act as DAMPs, in a destructive positive feedback loop, to promote exacerbation of inflammation. Ulinastatin (UTI) may attenuate EGL shedding by inhibiting serine proteases and hyaluronidase.

Objective: This trail evaluates whether EGL shedding elicited by Traditional Whipple Procedure (TWP) could be decreased by using UTI.

Atherosclerosis (AS) is a major pathogenic factor in patients with cardiovascular diseases, and endothelial dysfunction (ED) plays a primary role in the occurrence and development of AS. In our study, we attempted to evaluate the role of phosphatase and tensin homolog (PTEN) in endothelial cell apoptosis under oxidized low-density lipoprotein (ox-LDL) stimulation and identify the associated mechanisms. The results of our study demonstrated that ox-LDL induced human umbilical vein endothelial cell (HUVEC) death via mitochondrial apoptosis, as evidenced by reduced mitochondrial potential, increased mitochondria permeability transition pore opening, cellular calcium overload, and caspase-9/-3 activation.
