Background: Cognitive impairment is common in patients with heart failure and preserved ejection fraction but its clinical correlates and prognostic associations are poorly understood.

Methods: We analyzed cognitive function, using the Mini-Mental State Examination (MMSE), in patients with heart failure and preserved ejection fraction enrolled in a prespecified substudy of the PARAGON-HF trial (Prospective Comparison of Angiotensin Receptor Neprilysin Inhibitor With Angiotensin Receptor Blocker Global Outcomes in Heart Failure With Preserved Ejection Fraction). Logistic regression analyses were performed to determine the variables associated with lower MMSE scores at baseline and postbaseline decline in MMSE scores at 48 weeks.

Article Synopsis
  • This study compares the prognostic value of different body measurement indices, like BMI and waist-to-height ratio, in predicting mortality in patients with non-ischaemic heart failure with reduced ejection fraction (HFrEF).
  • It finds that higher BMI is linked to increased risks of all-cause and cardiovascular death, particularly in patients with a BMI of 35 or more.
  • The research suggests that greater waist-to-height ratios also indicate higher mortality risk, highlighting the importance of fat location and distribution beyond just body weight.

Article Synopsis
  • - The study investigated whether the heart failure medication sacubitril/valsartan could lead to cognitive impairment, as it affects enzymes that degrade amyloid-β peptides, which are linked to Alzheimer's disease.
  • - Researchers used the Mini-Mental State Examination (MMSE) to assess cognitive function changes over 96 weeks in a subset of heart failure patients from the PARAGON-HF trial, comparing those on sacubitril/valsartan to those on valsartan alone.
  • - Results showed no significant difference in cognitive decline between the two groups, with both medications resulting in nearly identical changes in MMSE scores, suggesting sacubitril/valsartan does not have a negative effect on cognitive function in these patients. *

Aims: Although trials have proven the group-level effectiveness of various therapies for heart failure with reduced ejection fraction (HFrEF), important differences in absolute effectiveness exist between individuals. We developed and validated the LIFEtime-perspective for Heart Failure (LIFE-HF) model for the prediction of individual (lifetime) risk and treatment benefit in patients with HFrEF.

Methods And Results: Cox proportional hazards functions with age as the time scale were developed in the PARADIGM-HF and ATMOSPHERE trials (n = 15 415).

Background: Non-alcoholic Steatohepatitis (NASH) cirrhosis is the second most common indication for liver transplantation (LT) in the US and often is associated with significant co-morbidities. We validated a model and risk prediction score that reflects the benefit derived from LT for NASH cirrhosis by predicting 5-year survival post-LT.

Methods: We developed a prediction score utilizing 6515 NASH deceased donor LT (DDLT) recipients from 2002 to 2019 from the Scientific Registry of Transplant Recipients (SRTR) database to identify a parsimonious set of independent predictors of survival.

Role of Cardiac Contractility Modulation in Heart Failure With a Higher Ejection Fraction.

J Card Fail

December 2022

Department of Medicine, University of Mississippi Medical Centre, Jackson, Mississippi, USA; Baylor Heart and Vascular Institute, Baylor University Medical Centre, Dallas, Texas, USA. Electronic address:

Cardiac contractility modulation (also known as CCM) is a novel device therapy that delivers nonexcitatory electric stimulation to cardiac myocytes during the absolute refractory period, and it has been shown to improve functional status and clinical outcomes in patients with heart failure (HF) with reduced ejection fraction (HFrEF). CCM therapy is currently recommended for a subset of patients with advanced HFrEF who are not candidates for cardiac resynchronization therapy. A growing body of evidence demonstrates the benefit of CCM therapy in patients with HFrEF and with ejection fraction at the upper end of the spectrum and in patients with HF and with mildly reduced ejection fraction (HFmrEF).

Uterine transplantation: legal and regulatory implications in England.

BJOG

March 2022

Department of Surgery and Cancer, Hammersmith Hospital, Imperial College London, London, UK.

Uterus transplantation (UTx) is fast evolving from an experimental to a clinical procedure, combining solid organ transplantation with assisted reproductive technology. The commencement of the first human uterus transplant trial in the United Kingdom leads us to examine and reflect upon the legal and regulatory aspects closely intertwined with UTx from the process of donation to potential implications for fertility treatment and the birth of the resultant child. As the world's first ephemeral transplant, the possibility of organ restitution requires consideration and is discussed herein.

Absolute uterine factor infertility (AUFI) is defined as the absence of a uterus or the presence of a non-functional uterus. Before the first live birth from a uterus transplant in 2014, the only fertility options for women with AUFI were surrogacy and adoption. In November 2019, our team was granted approval for the first uterus transplant trial in Australia using known living donors.

Uterine transplantation (UTx) is a fertility restoring treatment for women with absolute uterine factor infertility. At a time when there is no question of the procedure's feasibility, and as the number of livebirths begins to increase exponentially, various important reproductive, fetal, and maternal medicine implications have emerged. Detailed outcomes from 17 livebirths following UTx are now available, which are reviewed herein, along with contextualized extrapolation from pregnancy outcomes in other solid organ transplants.

Several blood pressure guidelines recommend low sodium intake (<2.3 g/day, 100 mmol, 5.8 g/day of salt) for the entire population, on the premise that reductions in sodium intake, irrespective of the levels, will lower blood pressure, and, in turn, reduce cardiovascular disease occurrence.

J Muscle Res Cell Motil

February 1992

Baylor Research Institute, Baylor University Medical Centre, Dallas, TX 75226.

We looked for evidence that the diffusion of heavy meromyosin is modified by its interaction with actin. To be able to observe diffusion in one dimension, we electrophoresed the complex of F-actin and heavy meromyosin in agarose gels in thin capillaries. The intensity profile of the electrophoretic band of the complex showed a sharp peak, which in 1% agarose in the electric field of 17.
