The pathogenesis of schizophrenia is rarely attributed to dysfunction of the cerebellum. However, coordination of mental processes might involve the cerebellum and a cortico-cerebellar-thalamic-cortical circuit (CCTCC) that can mediate that process has been proposed. We present the case of a 31-year-old male patient with a family history of psychosis who developed schizophrenia in association with a slow-growing glioblastoma at the left posterior cerebellar pontine angle.

Role of inflammatory markers in the preoperative evaluation of patients undergoing salvage total hip arthroplasty.

Surgeons may use laboratory tests, including erythrocyte sedimentation rate, C-reactive protein (CRP), and white blood cell count, as well as joint aspirations to diagnose prosthetic joint infections. There is a paucity of literature correlating preoperative inflammatory markers with risk of infection in the setting of salvage total hip arthroplasty (THA). This retrospective case analysis included patients who underwent a THA salvage procedure a minimum of 3 months after a failed fixation of a proximal femur or acetabulum, with a goal of assessing the utility of inflammatory markers as a screening tool in preoperative evaluation of salvage THA.

Background: Cerebral venous sinus thrombosis (CVST) is a rare cause of stroke that mainly affects the young. Anticoagulation (AC) with heparin is the mainstay of treatment for CVST. Although highly anticipated, endovascular therapy (ET) including local thrombolysis and mechanical thrombectomy has been controversial.

Development and validation of a prediction model for estimating one-month mortality of adult COVID-19 patients presenting at emergency department with suspected pneumonia: a multicenter analysis.

There are only a few models developed for risk-stratifying COVID-19 patients with suspected pneumonia in the emergency department (ED). We aimed to develop and validate a model, the COVID-19 ED pneumonia mortality index (CoV-ED-PMI), for predicting mortality in this population. We retrospectively included adult COVID-19 patients who visited EDs of five study hospitals in Texas and who were diagnosed with suspected pneumonia between March and November 2020.

Vertebral artery dissection and high-intensity workouts.

This case describes a 59-year-old athletic man who developed a lateral medullary stroke from a vertebral artery dissection after a CrossFit-style overhead lifting workout. The patient presented for dysarthria and a right-sided facial droop upon wakening. A secondary evaluation of the computed tomography angiogram, in corroboration with exercise history obtained much later, led to the diagnosis of right vertebral artery dissection.

We present a case of a 55-year-old poorly controlled diabetic who presented to the hospital with facial pain, ophthalmoplegia, vision changes, and diabetic ketoacidosis and was diagnosed with rhinocerebral mucormycosis due to . He was started on liposomal amphotericin B and micafungin and went for nasal endoscopy and debridement, but the infection had progressed through the base of the skull and he received the maximum tolerated debridement. Posaconazole was added and discontinued due to elevated liver chemistry tests and was replaced with oral isavuconazole.

Cannabis is the most commonly consumed recreational drug in the world. As more states legalize cannabis use in some form, the incidence of cannabinoid hyperemesis syndrome (CHS) is expected to rise. CHS is a constellation of symptoms including severe cyclical nausea and vomiting and epigastric or periumbilical abdominal pain as a result of long-term cannabis use.

Intraoperative pathology consultation plays an important role in the management of surgical patients and is also a measure for quality control in surgical pathology. A retrospective study was conducted to review intraoperative consultation during a 5-year period at a single institution. There were 19,145 intraoperative pathology consultation cases, including 19,026 concordant cases, 71 (<1%) frozen discordant cases, 11 (<1%) gross discordant cases, and 37 (<1%) deferred cases.

The Art of SAPIEN 3 Transcatheter Mitral Valve Replacement in Valve-in-Ring and Valve-in-Mitral-Annular-Calcification Procedures.

The SAPIEN 3 is the only transcatheter heart valve commercially available for compassionate transcatheter mitral valve replacement in patients with previous mitral surgical rings and mitral annular calcification (valve in ring [VIR] and valve in mitral annular calcification [VIM]). Reported outcomes have been inconsistent or poor. The review provides an overview of the authors' approach to achieve largely consistent results despite the intrinsic limitations of SAPIEN 3 VIM and VIR.

Successful Minimally Invasive Management of a Gastroduodenal Artery Pseudoaneurysm Causing Extrinsic Bile Duct Compression.

Gastroduodenal artery (GDA) pseudoaneurysms are rare clinical entities that typically develop in the setting of chronic inflammation of the pancreas, although idiopathic pseudoaneurysms can occur. Although GDA pseudoaneurysms carry the risk of rupture with resultant hemorrhage, they seldom are reported to cause biliary obstruction. We report a unique case of biliary obstruction secondary to extrinsic compression of the bile duct by a GDA pseudoaneurysm successfully managed by nonoperative means.

This study aims to investigate the mortality variations of COVID-19 from different hospital settings during different pandemic phases.

Methods: We retrospectively included adult (≥18 years) patients who visited emergency departments (ED) of five hospitals in the state of Texas and who were diagnosed with COVID-19 between March-November 2020.

Reasons for canakinumab initiation among patients with periodic fever syndromes: a retrospective medical chart review from the United States.

Background: Although canakinumab has demonstrated efficacy in multiple trials in patients with periodic fever syndromes (PFS), the evidence on initiation of canakinumab among PFS patients in real world setting is not well understood. We aimed to characterize the reasons for canakinumab initiation among patients with PFS, specifically, cryopyrin-associated periodic syndrome (CAPS), hyperimmunoglobulin D syndrome/mevalonate kinase deficiency (HIDS/MKD), TNF receptor-associated periodic syndrome (TRAPS) and familial Mediterranean fever (FMF).

Her medical history included an uncomplicated gastric lap band surgery 9 years ago and subsequent removal of lap band after 6 years. She had a Roux-en-Y gastric bypass and cholecystectomy 5 months prior to the current presentation.

Nonpenetrating trauma resulting in hemopericardium presenting as syncope.

Hemopericardium is a life-threatening condition that needs to be treated promptly to prevent serious complications. It typically results from penetrating trauma directly into the pericardial sac resulting in cardiac tamponade and death if not promptly treated. High clinical suspicion should prompt further workup even in the absence of significant injury.
